Bulimia nervosa is an eating disorder characterized by frequent bouts of binge eating accompanied by compensatory behaviour for preventing weight gain (purging). It is estimated that 3% to 5% of young women are affected by bulimia nervosa, and its prevalence is increasing. Bulimia nervosa afflicts both sexes and all races. It can lead to serious medical complications. The expression of the disease in the gastrointestinal tract may have a critical role in the diagnosis of bulimia nervosa. Physiological effects of bulimia nervosa on the gastrointestinal tract include dental caries and enamel erosion; enlargement of the parotid gland; esophagitis; changes in gastric capacity and gastric emptying; gastric necrosis; and alterations of the intestinal mucosa. Identification of any of these factors may aid in establishing an early diagnosis, which has been shown to increase the likelihood of recovery.

This chapter describes the diagnostic criteria used in clinical and research settings to define eating disorders and highlights important changes in the criteria introduced by DSM-5 in 2013. Emerging evidence suggests that the DSM-5 categories usefully capture a range of eating problems in need of clinical attention that were not well described in previous DSM editions. The chapter also reviews the characteristics of adolescents and young adults with eating disorders, their frequency in the general population, and their psychological comorbidities. Important medical complications associated with the disorders, especially among adolescents, are summarized, as well as approaches to their treatment. The chapter covers in greatest detail anorexia nervosa, bulimia nervosa, and binge-eating disorder.

Eating disorders affect about five million Americans every year. There are three different eating disorders: anorexia nervosa, bulimia nervosa, and binge eating disorder. Eating disorders are complex conditions deriving from a complex interplay of long-standing behavioural, emotional, psychological, interpersonal, and social factors. The neuronal circuits that control the ingestion of food are mainly related to catecholaminergic, serotoninergic, and peptidergic systems. In this respect, while serotonin, dopamine and prostaglandin promote the ingestion of food, by contrast, neuropeptide Y, noradrenaline, γ‎-aminobutyric acid (GABA), and opioid peptides inhibit food ingestion, thus causing the development of eating disorders (1). Eating disorders typically occur in adolescent girls or young women, although 5–15% of cases of anorexia nervosa and bulimia nervosa and 40% of cases of binge eating disorder occur in boys and men. Approximately 3% of young women are affected with these disorders, and probably twice that number has clinically important variants. Although early disorders mostly develop in adolescence or young adulthood, they can occur after the age of 40 years and are increasingly seen in young children (2). Eating disorders are more prevalent in industrialized societies than in nonindustrialized societies, and occur in all socioeconomic classes and major ethnic groups in the USA. About half of those who have anorexia nervosa or bulimia nervosa fully recover, approximately 30% have a partial recovery, and 20% have no substantial improvement in symptoms (2). The aim of this chapter is to give an overview of the endocrinology of eating disorders leading to excessive weight gain or excessive weight loss in humans. It is of note that despite the strong association between obesity and eating disorders, the increase in obesity is not followed by an increase in eating disorders (3).

Many persons with EDs receive pharmacotherapy, but pharmacotherapy research for EDs has lagged behind that for other major mental disorders. This chapter first provides a brief rationale for using medications in the treatment of EDs. It then reviews the data supporting the effectiveness of specific medications or medication classes in treating patients with anorexia nervosa (AN), bulimia nervosa (BN), binge eating disorder (BED), and other potentially important EDs, such as night eating syndrome (NES). It concludes by summarizing these data and suggesting future areas for research in the pharmacotherapy of EDs.

The epidemiology of eating disorders holds important clues for understanding factors that may contribute to their etiology. In addition, epidemiological findings speak to the public health significance of these deleterious syndromes. Information on course and outcome are important for clinicians to understand the prognosis associated with different disorders of eating and for treatment planning. This chapter reviews information on the epidemiology and course of anorexia nervosa, bulimia nervosa, and two forms of eating disorder not otherwise specified, binge eating disorder and purging disorder.

Cardiovascular disease remains a leading cause of mortality worldwide. Unresolved inflammation plays a critical role in cardiovascular diseases development. Specialized Pro-Resolving Mediators (SPMs), derived from long chain polyunsaturated fatty acids (LCPUFAs), enhances the host defense, by resolving the inflammation and tissue repair. In addition, SPMs also have anti-inflammatory properties. These physiological effects depend on the availability of LCPUFAs precursors and cellular metabolic balance. Most of the studies have focused on the impact of SPMs in adult cardiovascular health and diseases. In this review, we discuss LCPUFAs metabolism, SPMs, and their potential effect on cardiovascular health and diseases primarily focusing in neonates. A better understanding of the role of these SPMs in cardiovascular health and diseases in neonates could lead to the development of novel therapeutic approaches in cardiovascular dysfunction.
