In 16 patients entering a renal dialysis program and given tetracycline bone labels prior to 11th rib biopsy, tissue-level bone formation rates measured by quantitive histological analysis fell below normal in each case and on both the haversian and cortical–endosteal bone surfaces, averaging one-third of normal. On both envelopes there were increases in the relative and absolute amounts of osteoid seam perimeter in a unit amount of bone as well as in the total volume of osteoid in a unit amount of bone.

Resumo: Objetivou-se traçar o perfil epidemiológico do portador de IRC em tratamento hemodialítico na cidade de Teresina-PI. Tratou-se de um estudoquantitativo descritivo, realizado na unidade de nefrologia conveniada ao Sistema Único de Saúde (SUS), com 64 pacientes. A coleta de dados ocorreude março a maio do ano de 2010. Constatou-se que a maioria dos pacientes é do gênero masculino (67,19%), faixa etária de 40 a 59 anos (40,63%), sendo60,94% casados, residentes em Teresina (64,06%), com baixo nível de escolaridade, renda individual de 1 a 2 salários mínimos e com moradia própria.Em 69% a doença de base foi hipertensão arterial e diabetes; e as principais dificuldades encontradas foram os efeitos causados pelo tratamento eo transporte, influenciando na manutenção da terapia hemodialítica. Tais fatores servem para subsidiar decisões para melhorar a assistência a essespacientes juntamente com o enfermeiro atuando na prevenção das doenças que levam a IRC.Descritores: Insuficiência Renal Crônica, Diálise Renal, Avaliação em enfermagem.Epidemiological profile of renal failure patients submitted to hemodialysisAbstract: The objective was to outline the epidemiological profile of patients with CRF on hemodialysis in the city of Teresina-PI. Chronic renal failure is the progressive loss of function of kidney and patient requires a long treatment in the form of renal replacement therapy. Alanine aminopeptidase activity has been assayed in (70) patient’s sera samples of both sexes, aged (25-70) years. The sample were divided in to two groups, the first group(G1) is of (35) samples before dialysis. The second group(G2) is of (35) samples after dialysis, as well as (30) sample of healthy as control group(C). The result showed a significant increase (p> 0.001) in general in the enzyme Alanine aminopeptidase in the serum of chronic renal failure patients (116.0±3.4) U/L while the efficacy rate is (105.5±3.4) U/L in healthy persons. Goal of the research was partial purified of enzyme from sera patients with chronic renal failure. by using two techniques: The first gel filtration chromatography packed with spadix G50 and giving 3.45% yield and 0.964-fold purification. Two isoenzymes were obtained by using ion exchange chromatography and the purity degree of isoenzymes were 0.687(1.08%) and 2.539(1.04%) fold respectively. Also, the results showed a single band for isoenzymes following steps.

ABSTRACT Aim Periodontitis, a chronic infectious disease, affects most of the population at one time or the other and its expression is a combination of hosts, microbial agents, and environmental factors. Extensive literature exists for the relationship between periodontal disease and diabetes mellitus, cardiovascular diseases, and adverse pregnancy outcomes. Only a few studies performed in a limited number of patients have reported periodontal health status in chronic renal failure patients. Hence, the aim of the present study is to assess and compare the periodontal status of patients with chronic renal failure undergoing dialysis, predialysis with systemically healthy individuals. Materials and methods A total of 90 patients were divided into three groups. Group I: 30 renal dialysis patients. Group II: 30 predialysis patients. Control group comprised 30 systemically healthy patients who formed group III. Periodontal examination was carried out using oral hygiene index-simplified (OHI-S), plaque index (PI), gingival index (GI), probing depth, and clinical attachment loss. Results The results of the study showed that the periodontal status of patients with chronic renal failure undergoing dialysis (dialysis group) and patients with chronic renal failure not undergoing renal dialysis (predialysis) when compared with systemically healthy subjects showed significantly higher mean scores of OHI-S, PI, and clinical attachment loss. Conclusion Thus, patients with chronic renal failure showed poor oral hygiene and higher prevalence of periodontal disease. Clinical significance The dental community’s awareness of implications of poor health within chronic renal failure patients should be elevated. Treatment and control of anemia plays an important role in improving the quality and effectiveness of artificial kidney. The study was conducted on 61 patients who met the selection and were not included in the excluded criteria group. Collecting data from medical records using retrospective, descriptive, vertical follow-up method for 6 months. Data were processed using Microsoft Excel Office 2010 and R. The results showed that patients used both EPO alpha and EPO beta simultaneously. Rate of usage’s EPO beta accounted for 23%, alpha EPO was 77%. Lyophilized form (EPO alpha) is most commonly used. Starting from T3, Hb. averages treat. There were no abnormalities in blood pressure, white blood cell, platelets, and electrolytes. Abstract There are two primary concerns in the management of patients with chronic renal failure (CRF) under hemodialysis who are receiving rHEpo for anemia - determine the ideal dose of iron and predict the response to the rHEpo therapy used to treat their anemia. The classic parameters used for monitoring iron therapy in these patients are Transferrin Saturation (TSAT %) and Ferritin (Ferr) concentration. Recently some new red cell parameters have been proposed to support these existing markers as the new research use only (RUO) tests, called Low Hemoglobin Density % (@LHD%®) and Microcytic Anemia Factor (@Maf®), produced by the LH 700 Series hematology analyzers We also have analysed the best hematological parameters to support TSAT % and Ferritin and the best predictors to predict the response to therapy. A total of 30 adult renal dialysis patients receiving EPO for treatment of anemia secondary to end-stage renal disease were enrolled over a six month period from March to August, 2006. The patients were stable with regard to dialysis treatment for at least 3 months prior to entry into the study. Patients had frequent follow up during therapy, consisting of CBC, auto-differential, automated retic %, absolute #, IRF, MRV and RUO parameters. The testing schedule followed the hospital’s existing standard of care with CBC, WBC Differential and Reticulocyte measurements being performed monthly. Iron levels and iron binding capacity were measured before the beginning the study, and then monthly for the duration of the study. These and other chemistry tests were performed on the Beckman Coulter DxC and DxI chemistry and immunoassay instruments and included serum Ferritin, serum iron, TIBC, Transferrin, transferrin saturation, and CRP. A 1 ml aliquot of serum from each sample was frozen for possible additional testing later (soluble transferring receptor (sTfR), etc.). The patients’ anemias were classified using their ferritin and transferrin saturation according to the KDOQI guidelines as Absolute Iron Deficiency (AID), Functional Iron Deficiency (FID) and any iron deficiency: AID or FID (any ID). CRP results were considered in the diagnosis. A response to the rHEpo therapy was defined as an increase in Hemoglobin concentration of 1 g/dL in 2 months, with a minimum increase of 0.3 g/dL per month. The patients’ results were classified as responder or non-responder based on the delta Hgb and delta reticulocyte results from month to month There were 29.3% (41/140) classified as responders. Using a Mann-Whitney test, significant differences (p<0.05) were found for @LHD%, @MAF, MCH comparing patients with TSAT% <20% vs TSAT%>20% and @LHD% for patients with Ferritin <100 ng/mL vs Ferritin> 100 ng/mL) The best RBC parameters for detecting the types of anemias found in these patients were: Detection of IDA was @LHD%® (>4.7%) Detection of FID: MCH (<29.1 g/dL) and @LHD%® (>6.1%) Detection of any ID (AID or FID): @LHD%® (>4.7%) and MCH (<30 g/dL) The best parameter for the prediction of the response to therapy as indicated by an increase in Hgb is @Maf® (<9.2) with a ROC Area under the curve of 0.714. Red Cell parameters can complement the classic parameters used to detect Iron deficiency and predict the response to therapy. The value of these findings to the clinician is that when the classic chemistry parameters are borderline or indeterminate, these new red cell parameters can be use to arrive at a definitive diagnosis and ensure the correct treatment option. @ Research use only parameters

It has been suggested that small doses of PTH could exert an anabolic effect on bone and could be beneficial in the management of bone diseases secondary to low bone formation. The effects of 20 wk of continuous infusion of 0.05 U.kg-1.h-1 of 1-34 PTH on cellular, structural, and dynamic parameters of bone were studied in inbred beagles. This physiologic or near physiologic dose of PTH caused a small but significant rise in the concentration of serum calcium and a significant increment in the plasma concentration of 1,25(OH)2D. In addition, the sustained infusion of PTH was associated with a significant increment in volume and surface density of osteoid without a change in bone mass. Mineralization of osteoid was not altered, as evidenced by normal double tetracycline uptake and normal osteoid seam thickness. The enhanced osteoid production was not due to augmented bone formation by individual osteoblasts or basic remodeling units but rather to increased activation frequency resulting in an increased number of remodeling units. There was also augmented bone resorption at the tissue level. The data indicate that small doses of PTH do not have an anabolic effect on the skeleton because they are not associated with increased net bone formation. Rather, PTH administration is associated with an increase in coupled bone turnover.
