Double-angle shear connections with short outstanding legs

This paper describes an experimental study on double-angle shear connections with a narrow outstanding leg width from 31 to 37 mm. When connection angles are welded to a supporting column with a face width less than 152 mm, a common practice is either to weld the angles to the column by flare bevel groove welds or shorten the outstanding legs to accommodate fillet welds. This practice reduces the rotational capacity of the connections; and therefore confirmation of this practice is required. Three groups, comprising 12 full-scale connection specimens, were tested. Each group consisted of four specimens having three, four, five, and six bolts, respectively. Group A used 9.5 mm thick angles and fillet welds, group B used 6.4 mm thick angles and fillet welds, and group C used 9.5 mm thick angles and flare bevel groove welds. The test results demonstrated that both rotational and shear capacities of the connections are satisfactory when either of the two details are used.

Nowadays, people are more vulnerable to metabolic disorders due to their faulty dietary and behavioural habits. One such disorder is Vatarakta which causes functional impairment due to involvement of Sandhi (joints). It is manifested by Ruk, Toda, Sparsha asahatva, Shopha, Raga, Daha and Stabdhata in Sandhi. Vatarakta can be correlated with Hyperuricaemia or Gout due to similarity in their clinical features. Hyperuricaemia is defined as abnormally high level of uric acid in blood (i.e. >6mg/dl in female and >7mg/dl in male). On the other hand, Gout is an inflammatory response to monosodium urate crystals formed secondary to hyperuricaemia. Aims and objectives: 1. To evaluate the effectiveness of Trikarshika kwatha and lifestyle modification in the management of Vatarakta. 2. To compare the effects of Trikarshika kwatha with and without lifestyle modification in the management of Vatarakta. Materials and methods: Raw herbs of the research formulation were collected after proper identification and Kwatha was prepared for oral administration. For the clinical study, total 60 patients were selected on the basis of selection criteria. Selected patients were randomly divided into two groups. (i) Group A: 30 patients were treated with Trikarshika kwatha. (ii) Group B: 30 patients were treated with Trikarshika kwatha along with Lifestyle modification. Individual patient was treated for 45 days along with follow up at the interval of every 15 days. To assess the effectiveness of treatment, scoring pattern was followed for subjective and objective parameters. They were assessed before and after treatment. The collected data were analysed statistically by using Paired t-test. Results: On the basis of all statistical data, it can be said that patients of Group B showed better results in all parameters in comparison to patients of Group A. Conclusion: Both Trikarshika kwatha and Lifestyle modification are affective but Trikarshika kwatha with Lifestyle modification is more effective than Trikarshika kwatha without Lifestyle modification in the management of Vatarakta.

Aim: To compare the frequency of epigastric pain and uterotonic effect of an equivalent dose of oxytocin administered as an intravenous bolus versus intravenous infusion during elective LSCS under spinal anesthesia. Methodology: We recruited 98 parturients undergoing elective LSCS under spinal anesthesia for this prospective quasi experimental study and divided them into two groups. Group-A received 5 IU of oxytocin as bolus intravenous (IV) injection in 5 sec (bolus group, n= 48), and Group-B (infusion group, n= 50) received 5 IU of oxytocin as an infusion over 5 min. Any complaint of epigastric pain by the patients was noted and its frequency was compared between the two groups. The uterine tone was assessed as adequate or inadequate by an obstetrician. The data were entered into SPSS version 22. Patient demographic data were analyzed with independent samples T-test and the study data were analyzed with Chi‑square test and presented as n (%). p < 0.05 was considered statistically significant. Results: Epigastric pain was noted in 25 (52.03%) out of 48 parturients in Group-A and 15 (30%) out of 50 in Group-B (p = 0.026). There was no significant difference in the uterotonic effect of oxytocin between the two groups (p = 0.736). Conclusion:  We conclude that oxytocin infusion is associated with lower frequency of epigastric pain in elective LSCS when compared to intravenous bolus of an equivalent dose of oxytocin, However, the effect on uterine contractions was adequate with both methods. Objective: To determine effect of citrullus lanatus juice on hemoglobin and Serum Glutamic Pyruvic Transaminase (SGPT) level. Study design and setting: It was an experimental study conducted on healthy rabbits for sixty days in the research department of pharmacy University of Karachi. Methodology: Current study was planned to evaluate effect of Citrullus lanatus juice on red blood cells, hemoglobin and liver enzyme. 60 days study was performed at 2 different doses i.e 3and 6ml/kg on rabbits. These rabbits were from either gender and were divided into 3 groups their weight ranges from 1200 to 1800 grams. Group A is control group (Normal saline 6ml/kg), Group B is treated group (3 ml/kg), Group C is treated group (6ml/kg). After taking mean of all values they are compared with control group. Significance of mean can be estimated by Tukes Post Hoc Test. P<0.05 estimated as significant. Results: It was found that count of red blood cells rises significantly along with rise in hemoglobin level. As far as liver enzyme serum glutamate pyruvate transaminase (SGPT) was concerned its concentration decreases slightly. Conclusion: Citrullus lanatus juice contain ingredients which are important for RBC hemoglobin and synthesis .It also contains important antioxidants that have organoprotective role due to which SGPT level decreases even in healthy animals as compared to control groups

Context: Gentamicin is an antibiotic used in gram-negative infection. It is an nephrotoxic agent. Proximal tubular epithelial cells are selectively sensitive to gentamicin. Objective: To observe the toxic effect of gentamicin on epithelial cells of proximal convoluted tubule in Long Evans rats. Study design: An experimental study. Plan and period of study: The study was carried out in the Department of Pharmacology of IPGMR Dhaka during the period of 1995 to 1996. Materials and methods: Eighteen mature long Evans rats were divided into three groups A, B & C. The rats of group A were treated with vehicle and group B and C were treated with gentamicin 50gm and 100gm respectively. Blood urea and serum creatinine were measured and microscopic damaged cells were counted. Result: There was significant increase in serum creatinin and blood urea in gentamicin treated groups. Microscopic examination of kidney showed necrosis of proximal tubular cells. Conclusion: It can be concluded from this study that gentamicin has toxic effects on renal tubule. Key words: Gentamicin. This study compares the effect of Hyalobarrier Gel and 4DryField® PH on the formation of postoperative adhesions after myomectomy in an experimental study on rabbit does. Thirty experimental animals were divided into three groups: group A – control, group B – use of 4DryField® PH, and group C – use of the Hyalobarrier Gel. A simulated myomectomy was performed in each group. The animals were euthanised 14 days after the procedure and the antiadhesive effect of the above-mentioned preparations was evaluated according to the criteria. Statistical evaluation clearly showed differences in the antiadhesive effect in the scoring systems for groups B and C compared to control group A at a high level of significance. We demonstrated an obvious, statistically confirmed antiadhesive effect of the Hyalobarrier Gel and 4DryField® PH in an experimental model of the rabbit uterus. Comparing both preparations, the Hyalobarrier Gel shows a higher level of effectiveness. Based on the results of the experiment, both preparations can be unambiguously recommended for routine practice.

Objective: To determine the abnormality of spermatozoa in the contralateral epididymis after unilateral testicular torsion. Material & method: Twenty wistar rats were divided into two groups i.e. Group B (sham procedure) Group A (torsio and orchiectomy 24 hours later), and contralateral epididymectomy was performed a month later. Spermatozoa in the contralateral epididymis are extracted and analyzed by an experienced biologist. Data were analyzed using Chi-square or Fischer exact test. Results: Sperm morphology changes in group B is higher than Group A (6,6% vs 0,5%, p = 0,009). Conclusion: Unilateral testicular torsion causes sperm abnormal morphology in the contralateral epididymis. Keywords: Unilateral testicular torsion, contralateral epididimal spermatozoa.

Objective: To compare the efficacy of steroid antibiotic wick with Ichthammol Glycerol wick in the management of Acute Otitis Externa in terms of tenderness and clearance of discharge/debris. Study Design: Quasi-experimental study. Place and Duration of Study: ENT Out Patient department of Combined Military Hospital Peshawar and Hayatabad Medical Complex Peshawar, from May to Nov 2018. Methodology: A total of 250 patients were included and divided into two groups of 125 each. After necessary suction clearance topical Ciprofloxacin/Dexamethasone (Cipotec-D) wick was placed in auditory canal of group A patients and topical Glycerol/Ichthammol wick was placed in group B. Follow up visits were done on 3rd and 7th day of starting the treatment. Results: Group A patients responded better in terms of tenderness (88%) however both groups had similar response in terms of discharge reduction (7.2% vs 6.4%). In terms of efficacy neither of the treatment proved more efficacious compared to the other (p-value 0.058). Conclusion: While steroid antibiotic wick is significantly more efficient in terms reducing tenderness, in terms of overall efficacy and discharge reduction Ichthammol/glycerol is equally effective.

Background: Anti-inflammatory role of Aloe vera gel is well established. Diclofenac is extensively used for acute and chronic inflammation. The present study was conducted to compare dried Aloe vera gel and diclofenac effects on sodium and potassium balance in hypertensive rats. Material and Methods: This experimental study was conducted at Sargodha Medical College from May to November 2016. Twenty-four healthy male Sprague Dawley rats 7-8 weeks of age were included in study. Any unhealthy-looking rat was excluded from the study. Rats were equally and randomly divided into four groups Normal control (group A), Model control (group B), Aloe vera (group C) & Diclofenac (group D). Hypertension was induced by a 20 % sucrose diet in all groups except group A in 8 weeks’ time. Group B, C & D received distilled water and Aloe vera dried gel 400 mg/kg & diclofenac powder 12 mg/kg body weight respectively orally between 8 to 10 weeks. Serum and urine analysis was performed for hematocrit, sodium, and potassium concentrations at zero, eight and ten weeks. Twenty-four-hour urinary sodium excretion was calculated. Data was analyzed using Graph Pad Prism version 6. Result: After 2-week administration of aloe vera and diclofenac powder, serum potassium significantly decreased in Group C (p <0.001) while increased in Group B and D (p <0.001) as compared to group A. Urinary sodium concentration and excretion increased significantly in Group C (p <0.01) as compared to Group A whereas result of Group D was insignificant. No significant change in serum sodium and hematocrit of any group was observed. Conclusion: Aloe vera causes less sodium retention than diclofenac but decreases serum potassium contrary to the effect of diclofenac in hypertensive rats.

We evaluated the outcome of a community-based early intervention and habilitation for children with cerebral palsy (CP) in Bangladesh. Children registered on the Bangladesh CP Register (BCPR) were recruited in two groups for this study: Group A received a comprehensive six-month long community-based caregiver-led intervention program at the “Shishu Shorgo” (Bengali title, which translates to ‘Children’s Heaven’) Early Intervention and Rehabilitation Centres developed to support participants from the BCPR. Group B received standard care. A quasi-experimental study was conducted. Data were obtained at baseline, at the end of the program (i.e., 6 months), and at a 12-month follow-up. Outcome measures for children included gross motor functional measure (GMFM-66), Communication Function Classification System (CFCS), and Viking Speech Scale (VSS) and, for adult caregivers, the depression, anxiety, and stress scale (DASS 21). Between October 2016 and March 2017, 156 children with CP were recruited (77 in Group A and 79 in Group B). The total score of GMFM-66, CFCS level, and VSS level significantly improved statistically in Group A (p < 0.05 for all) and deteriorated in Group B (p < 0.001, p = 0.095, p = 0.232). The intervention showed promising outcomes particularly for children with CP under five years of age. There is a need for caregiver-led community-based programs for children with CP in LMICs.

Objective: To compare clinical efficacy of diacerein-ginger with diacerein alone in treating knee osteoarthritis. Duration and place of study: It was a randomized clinical trial conducted from 21st September 2018 to 31stMarch 2019, in medical OPD of a private hospital in Karachi. Methodology: 60 diagnosed patients of knee osteoarthritis were included in this study. Male and female patients 50 years of age, fulfilling the inclusion criteria and after written informed consent experienced a wash-out period of 72 hours. These patients were systematically randomized into 2 groups each having 30 members. Group A received capsule Diacerein 50mg + capsule Ginger 550 mg twice daily and group B received capsule Diacerein 50mg twice daily, for 12 weeks. Parameters checked at 0, 6 and 12 weeks were: Western Ontario and McMaster Universities Osteoarthritis (WOMAC) index, pain at rest and movement (Visual Analogue Scale). Comparison of the two groups was done by independent t-test. Results: Among 60 patients; 20 (33.33 %) were males and 40 (66.66%) were females. 4 patients in group A and 4 in B, dropped out during the study. Comparison of group A with group B in WOMAC and pain (at rest and movement) scores showed insignificant difference at day 0 before prescription of the drugs. However comparison showed highly significant difference (P-value < 0.001) between the two groups in WOMAC, pain at rest and movement scores at the end of 6th and 12th weeks of intervention. Conclusion: Diacerein-Ginger is clinically more efficacious for management of knee OA than Diacerein alone
