BackgroundAlpacas are being more frequently presented to veterinarians in the UK. It is important to validate whether published normal ocular parameters are consistent with the alpaca population in the UK.MethodsOphthalmic examinations were performed on healthy alpacas (Vicugna pacos) from three farms in East Anglia, UK.ResultsOn direct ophthalmoscopy of 35 alpacas, there was a 50 per cent prevalence of opacities within the lens in alpacas older than two years old (n=8/16). There was a 36.8 per cent prevalence of persistent hyaloid arteries in alpacas under two years old (n=7/19). The mean Schirmer tear test-1 value was 20.0 ±6 mm/minute (n=40). The mean intraocular pressure measured by rebound tonometry was 17.2 ±5.5 mmHg (n=46), and applanation tonometry resulted in statistically similar values (P=0.30; n=25). There was a significant variation in intraocular pressure throughout a 24-hour period (n=8). Fluorescein dye was not detected at the nostrils of any of the alpacas which underwent a Jones test to assess nasolacrimal duct patency (n=8).ConclusionThe ophthalmic findings appear largely consistent with previously published values from North America and continental Europe. Variations include the large range of measurements obtained and evidence of diurnal variation in intraocular pressure.

Purpose: Goldmann applanation tonometry (GAT) is considered the gold standard for Intraocular Pressure (IOP) measurement. It has the disadvantages of being a contact device, need for a slit-lamp, non-portability and need of a skilled examiner. Many hospitals are using a Non Contact Tonometry (NCT) as a screening device to save clinician time, however the usefulness is not proved in terms of reliability. This study was aimed to determine the usefulness of the Air-puff tonometer (TONOREF NIDEK II, NIDEK CO., LTD., JAPAN) over a GAT in a tertiary care center. Design: Cross-sectional Study Methods: This was a cross-sectional, non interventional observational study conducted on 224 eyes (right eye) from 224 patients. All patients underwent the IOP measurement with both methods and a central corneal thickness (CCT) measured. The data was analyzed using SPSS 20.0 software. Results: The mean age of the patients was 40.3±11.29 years. There was a statistically significant difference (p<0.001) between the mean NCT and GAT readings which persisted even after correction for central corneal thickness. The correlation between NCT and GAT using Pearson’s correlation coefficient was strong irrespective of the corrections for their corneal thickness (r = 0.751 and 0.718 for uncorrected and corrected values respectively). The correlation of the individual clinicians for the readings varied from moderate to strong. The ROC curve showed the best sensitivity and specificity to occur at around 13 to 14 mmHg. Conclusion: NCT seems to overestimate the IOP at low ranges as compared to the GAT and underestimate at higher ranges. The crossover of the values is seen between 12 to 13 mmHg.  The clinician should do an individualized analysis of his/her GAT measurements to the readings of the NCT machine at the clinic to obtain clinician specific nomogram.

Introduction: Posterior capsular opacification is a common complication after cataract surgery. Neodynium Yttrium Aluminium Garnet laser capsulotomy is still the preferred treatment for posterior capsular opacification. This study was done to determine the use of Brimonidine eye drop in preventing the rise of intraocular pressure post-Neodynium Yttrium Aluminium Garnet laser capsulotomy. Methods: A descriptive cross-sectional study was conducted in Lumbini eye institute and research center, Bhairahawa, Nepal, in 200 eyes with posterior capsular opacification using Brimonidine from Feb 1, 2019, to July 30, 2019. The Institutional Review Committee approved the study with approval number 0237. A convenient sampling method was used. Pre-capsulotomy best-corrected visual acuity, slit-lamp examination of the anterior segment, and dilated fundus examination were done. Intraocular pressure was measured with Goldmann Applanation Tonometer. Post capsulotomy patients were evaluated after one hour, two hours, and two weeks for intraocular pressure and any complications. The statistical analysis was done using Statistical Package of Social Sciences version 20.0 statistical analysis software. The descriptive statistical analysis of the study was done after the collection of the data. Results: Mean age of patients at presentation was 61.61±SD 1.09. The mean intraocular pressure following Neodynium Yttrium aluminum garnet laser capsulotomy using brimonidine at 1 hour was 12.73±3.3 mmHg.and two hours was 11.98±3.2 mmHg. The mean energy per pulse was 2.3±SD 0.3 mJ. The mean duration of posterior capsular opacification from cataract surgery was 22.28 weeks. Conclusions: Neodynium Yttrium Aluminium Garnet laser capsulotomy had lower intraocular pressure after the Brimonidine eye drop procedure. The maximum mean reduction in intraocular pressure was observed after two hours.

Abstract Background Goldmann applanation tonometry (GAT) has been the gold standard for intraocular pressure(IOP) measurement ,since its appearance in clinical practice around 50 years ago.(1) In spite of being almost unchallenged, the last few years have become a sustained search for a new standard method for IOP measurement,. One such recently marketed instrument is the Ocular Response Analyzer (ORA), which able to detect the corneal biomechanics. Aim of the Work is to compare the IOP measurement estimated by Goldmann applanation tonometer to that of ORA and detect the effect of state of refraction, corneal topography and central corneal thickness (CCT) on these measurements. Patients and Methods This cross-sectional study was done from March 2018 to October 2018 on 65 eyes of patients visiting the outpatient clinic. Results The mean GAT IOP was 15.938 ± 6.041 while the mean ORA (IOPcc) and (IOPg) were 19.711 ± 7.59 and 17.242 ± 7.35 mm Hg respectively. There is a strong positive relationship between GAT IOP & ORA IOPg measurement (r = 0.880 – p = &lt;0.001*). Also finding a weak yet significant correlation between IOPg and CCT (r = 0.385, p = 0.001). None of the pressure measurements was affected by refraction or corneal curvature significantly. Conclusion In conclusion, our results suggest that mean IOPs obtained by ORA were significantly higher than that of GAT with different influencing factors that are not completely understood. caution has to be sought when using the ORA, the values obtained ought not to be used interchangeably with the values obtained by 1 GAT, despite the presence of a positive correlation between these values. This underlines the importance of using one and only method of evaluation of the IOP for every patient in successive follow-up visits.

Background: High dose of corticosteroids are required by patients with idiopathic nephrotic syndrome for long duration due to repeated relapse. The aims of this study were to analyze the ocular and extra-ocular complications of steroids on children with nephrotic syndrome. Methods: This cross-sectional study was conducted at Dhaka Shishu (Children) Hospital from September 2017 to September 2019. Children with nephrotic syndrome between 1-16 years of age, who received steroid for at least three months, participated in this study. Total 70 children were enrolled. Detailed history was taken from parents regarding initial episode, drug history, visual problem, headache, hypertension, weight gain, psychosis, bone pain, epigastric pain and excessive growth of body hair. Comprehensive opthalmologic assessment including visual acquity, intraocular pressure and cataract were performed. Detail of renal histopathology and treatment regimen in each patient was noted. Results: A total of 70 patients were included, 61.4% were male and 38.6% were female. Median age at the time of examination was 60 months (range 17 to 216 months). The mean duration of disease was 33.71±21.12 months. The mean cumulative steroid dose at the time of examination was 8485.91±7326.83 mg. Fourteen (20%) patients had posterior subcapsular cataract. Among 70 patients, we could do visual acuity for 25 (35.7%) patients, intraocular pressure (IOP) in mm for 51 (72.85%) patients. Among them raised IOP was found in five (9.8%) patients. In right eye mean IOP was 14.16±3.57 and for left eye 14.77±3.55 mm. Renal biopsy was performed in 15 (21.43%) cases. Among the biopsy report, it was found that mesangial proliferative glomerulonephritis (MPGN) in 8 (53.33%), focal segmental glomerulonephritis (FSGS) in 2 (13.33%) and minimal change in 5 (33.34%) cases. Among all cases, 12 (17.14%) patients developed Cushingoid facies, 3 (4.2%) patients had hypertrichosis, 3 (4.2%) patients had central obesity and 4 (5.7%) patients had buffalo hump but none had hypertension, diabetes mellitus, epigastric pain, bone pain or psychosis. Conclusion: Cataract formation was the most frequent ocular complication after large dose of oral corticosteroid therapy. The present study emphasizes the need for regular eye screening. Cushingoid facies was the most frequent among extra-ocular complications. Birdem Med J 2021; 11(1): 7-10

<p><strong>Background:</strong> Intraocular pressure (IOP) is one of the most important parameters in the diagnosis and treatment of glaucoma. Glaucoma has been established as the second leading cause of blindness. The treatment of glaucoma focuses mainly on lowering intraocular pressure (IOP). The target IOP is often set to a level 20% to 30% of IOP reduction, and consequent large IOP reduction beyond 30% or even 40% in cases of advanced glaucoma The different methods of tonometery are: Goldman Applanation tonometery, Noncontact (air-puff) tonometery, Perkins tonometery, Tonopen tonometery, Transpalpebral tonometery.</p><p><strong>Objective:</strong> To determine the frequency of accuracy of intraocular pressure (IOP) measured by non-contact (air puff) tonometer compared with Goldmann applanation tonometer.</p><p><strong>Methods:</strong> This was a non-interventional, cross sectional study conducted at a tertiary care centre of Dhaka, Bangladesh. consecutive subjects attending the BSMMU eye OPD were included in the study. IOP was measured by non-contact (air puff) tonometer and a slit lamp mounted GAT in all the subjects. The study samples were selected by convenience sampling who presented for check-up in the Eye Department of community ophthalmology, Bangabandhu Sheikh Mujib Medical University, Dhaka. Bangladesh. Results:A total of 120 eyes in 60 patients were studied. The mean age of the patients was 41.60 year. study population consisted of 24 (40 %) men and 36 (60 %) women. The mean intraocular pressure was 13.52 &amp;13.72 mmHg for GAT, and 16.64 &amp; 17.44 mmHg for Air puff respectively. The range of measurements by GAT was from 10 to 23 mmHg and by Air puff was 12 to 28mmHg. The difference between IOP measured by two instruments were statistically significant (p=0.000).</p><p><strong>Conclusion:</strong> Airpuff tonometer is quick, a non-contact method to measure intraocular pressure and is useful for screening purposes and postoperative case but the measurements should be confirmed with Goldmann applanation tonometer for accurate labelling of intraocular pressure.</p>

The current study was undertaken to establish reference values of intraocular pressure in clinically normal Turkish Shepherd Dogs with the rebound tonometer (TonoVet®) and the applanation tonometer (Tono-Pen Vet®). Twenty-eight dogs (14 male and 14 female, aged 9 months-9 years, 56 eyes) were used in this study. All animals were healthy and showed no abnormalities in ophthalmic examinations, which consisted of the Schirmer tear test, slit lamp bio-microscopy, and fluorescein staining. The animals were restrained manually without using systemic anesthetics or tranquilizers. IOP was measured in both eyes with a TonoVet® and a Tono-Pen Vet®. In normal dogs, the mean IOP values for all eyes were 17.63±3.34 mmHg according to the TonoVet® and 14.95±2.92 mmHg according to the Tono-Pen Vet®. There was no effect of sex, age, or eye side (right vs. left) on the results obtained with either the TonoVet® or the Tono-Pen Vet®. The mean IOP obtained with the rebound tonometer was 2-3 mmHg higher than that measured with the applanation tonometer. The reference data obtained in the present study may assist in diagnostic testing for ophthalmic diseases in Turkish Shepherd Dogs and may promote further studies in this area..

Abstract Background Measurement of intraocular pressure (IOP) is essential for glaucoma patients. Many factors such as central corneal thickness (CCT) can affect the accuracy of IOP measurement. The purpose of this study was to evaluate the agreement of IOP measured by non-contact tonometer (NCT), iCare pro rebound tonometer (iCare), and Goldmann applanation tonometer (GAT) in different IOP group. Methods This was a Hospital-based cross-sectional study. Two hundred subjects were enrolled in this study. All subjects underwent IOP measurement using an NCT–iCare–GAT sequence. Bland-Altman, Pearson correlation and intraclass correlation analysis were performed using SPSS 17.0 software. The influence of CCT on each IOP measurement methods was evaluated by linear regression analysis. Results The mean difference (Δ) of NCT–GAT did not differ from (Δ) iCare–GAT in IOP < 10 and 10–21 mmHg group. However, (Δ) NCT–GAT was significantly higher than (Δ) iCare–GAT in IOP 22–30 and > 30 mmHg group (P < 0.05). Bland–Altman analysis showed significant agreement between the three devices (P < 0.01). IOP measurements of the three methods were significantly correlated with CCT (P < 0.01). Conclusions ICare pro shows a higher agreement with GAT over a wide range of IOP compared with NCT. The consistency between the three tonometers was similar in a low and normal IOP range. However, NCT shows a greater overestimate of IOP in moderate and higher IOP group. The variability of IOP measurement affected by CCT is NCT > iCare pro > GAT.

Purpose: To compare the intraocular pressure (IOP) and central corneal thickness (CCT) of non-glaucomatous North-East (NE) Indian tribals and general Indians.  Materials and methods: In a prospective, cross-sectional study, the IOP and CCT of non-glaucomatous NE Indian tribals (n = 50) and non-glaucomatous general Indians (n = 50) were compared. Glaucoma was ruled out by history, detailed ocular examinations, and investigations. Results: There was very good correlation between the CCT and IOP values of right and left eyes (p = 0.940 and 0.847, respectively). The difference in the IOP values in the two groups was not statistically significant (p = 0.312 for Oculus Dexter [OD], p = 0.400 for Oculus Sinister [OS]). Similarly, the difference in the CCT values in the two groups was not statistically significant (p = 0.736 for OD and 0.613 for OS). The mean CCT and IOP OD for the whole population was 530.50 ± 35.42 μm and 13.80 ± 2.760 mmHg, respectively. By linear regression analysis, the IOP OD of the whole population had good correlation with CCT OD of the whole population (adjusted r2 = 0.084, p = 0.002), but not with age (adjusted r2=0.000, p=0.314) and sex (adjusted r2 = 0.010, p = 0.163). Similarly, CCT OD for the whole population did not have good correlation with age (adjusted r2 = -0.009, p = 0.762) and sex (adjusted r2 = -0.007, p = 0.603). Conclusions: In this study of individuals with normal corneas and without glaucoma, no racial variation was found in the CCT and IOP values of the two groups. The IOP OD of whole population had good correlation with CCT, but not with age and sex. There was good correlation between OD and OS values of IOP and CCT.

Background: Raised intaocular pressure is considered as a major risk factor for developing optic neuropathy. Intraocular pressure (IOP) depends on various systemic and local factors and it has been postulated that IOP increases proportionately with increase in systolic blood pressure and increase in degree of obesity. The aim of this study was to compare the Body Mass Index (BMI) and Intraocular Pressure (IOP) in adult population.Methods: Four hundred ninety nine participants (284 male, 215 female) aged between 20 to 70 years were included in the cross-sectional study. On the basis of BMI subjects were divided into four categories i.e. underweight, normal weight, overweight and obese as per revised body type classification for Indian population recommended by Ministry of Health and Diabetes foundation of India. The mean IOP difference was 14.6±1.8 as compared amongst the four categories.Results: The participants were divided into six categories according to age and the mean IOP of different age groups was calculated. The changes in the mean IOP of males was 15.8±2.5 mm of Hg and the mean IOP of females was 14.7±2.2 mm of Hg. The results of the current study were statistically significant p<0.01.Conclusions: By concluding the current study as well as in the previous published literature, the findings of the current study were statistically significant. IOP was positively correlated with BMI. However, the clinical significance cannot be confirmed by the minor deviation in the IOP as well as BMI of the participants.
