The aim. To increase the effectiveness of treatment of dry eye syndrome in patients with Sjogren’s syndrome with preservative-free drops which contain 0.4 % sodium hyaluronate and nutraceutical complexes nutraceuticals. Materials and methods. The study was conducted on the basis of the Filatov Institute of Eye Diseases and Tissue Therapy of the National Academy of Medical Sciences of Ukraine. The study included 64 patients aged 34 to 58 years (128 eyes) with dry eye syndrome accompanied by Sjogren’s syndrome. Examination of patients included: assessment of complaints, visometry, refractometry, bacterial culture of the conjunctiva and nasopharynx, biomicroscopy, ophthalmoscopy, Schirmer and Norn tests, assessment of "comfort of application of drops" on the questionnaire. This study is an extension of the study previously conducted at the premises of the Filatov Institute of Eye Diseases and Tissue Therapy of The National Academy of Medical Sciences of Ukraine on the effectiveness of preservative-free eye drops containing 0.4 % hyaluronic acid in the treatment of Sjogren’s syndrome. Results. Prior to the appointment of comprehensive treatment with instillations, the Norn test showed 6.55 s (SD 1.57) with a minimum of 3 s, a maximum of 9 s, and a median of 6.5 s. After 3 months of treatment the Norn test score in the 1st group averaged 14.66 (SD 1.07) vs. 13.86 (SD 1.07) in the 2nd group, but the difference was not statistically significant (p=1.0). The mean number of required instillations per day was 4.1 (SD 1.04) in the 1st group and 5.27 (SD 1.11) in the 2nd group. Conclusion. Instillations of preservative-free drops containing sodium hyaluronate 0.4 % (in a bottle with an innovative ophthalmic squeeze dispenser (OSD)) in combination with a nutraceutical complex in the treatment of dry eye syndrome in patients with Sjogren’s syndrome is effective and allows achieving a stable and long-term effect. Purpose: To evaluate the compliance with 0.1% cyclosporine A in dry eye patients with Sjögren's syndrome and identify factors affecting the compliance. Methods: This study included 67 dry eye patients with Sjögren's syndrome who had used 0.1% cyclosporine A for over 6 months. Clinical parameters including sex, age, the number of eye drops used, side effects, the use of steroid eye drops, and the visit interval were investigated. Additionally, the ocular surface disease index, tear break-up time (TBUT), Schirmer test score, and corneal fluorescein staining (CFS) score were assessed. Patients were classified into compliant and non-compliant groups, using a criterion of 50% compliance; relevant factors were evaluated accordingly. Results: Of the 67 patients, 36 (53.7%) were classified into the compliant group and 31 (46.3%) into the non-compliant group. Compared to the non-compliant group, the compliant group showed a longer TBUT (p = 0.03) and a lower CFS score (p = 0.04) at 6 months of follow-up. Altogether, 39 subjects (58.2%) experienced side effects, of which the most common was stinging pain (71.8%). In the multivariate analysis, severe dry-eye symptoms (p = 0.03), non-use of the steroid eye drops (p = 0.02), and longintervals between exam vitists (p = 0.02) were identified as factors reducing compliance. Conclusions: Factors related to a decrease in the compliance with 0.1% cyclosporine A in dry-eye patients with Sjögren's syndrome, included severe dry eye symptoms, long visit intervals, and non-use of steroid eye drops. Taking these factors into consideration may help increase patient compliance.

The study included 14 patients aged 34 to 59 years (28 eyes) with dry eye syndrome concomitant with Sjogren’s syndrome. The examination of patients included: assessment of complaints, visometry, refractometry, culture tank from the conjunctiva, biomicroscopy, ophthalmoscopy, Schirmer’s test and Norn’s test, assessment of the “comfort of using drops” according to the questionnaire developed for this study. Results. Before the appointment of instillations, Norn’s test was 6.5 seconds (SD 1.56) with a minimum of 3 seconds and a maximum of 9 seconds, a median of 6.5 seconds. After a month of treatment while taking drops, Norn’s test in the first group averaged 14.86 (SD 1.07) versus 13.86 (SD 1.07), but the difference was not statistically significant (p=1.0). The average number of instillations required per day for the first group was 4.14 (SD 1.06) and for the second group it was: 5.29 (SD 1.11). Conclusion. Instillations of non-preservative drops containing sodium hyaluronate 0.4% (in a bottle equipped with an innovative reusable push-type ophthalmic dispenser (OSD) in the treatment of dry eye syndrome in patients with Sjogren’s syndrome are quite effective and very convenient to use.
