As the leading cause of trauma-related mortality, blood loss due to hemorrhage is notoriously difficult to triage and manage. To enable timely and appropriate care for patients with trauma, this work elucidates the externally measurable physiological features of exsanguination, which were used to develop a globalized model for assessing blood volume status (BVS) or the relative severity of blood loss. These features were captured via both a multimodal wearable system and a catheter-based reference and used to accurately infer BVS in a porcine model of hemorrhage (n = 6). Ultimately, high-level features of cardiomechanical function were shown to strongly predict progression toward cardiovascular collapse and used to estimate BVS with a median error of 15.17 and 18.17% for the catheter-based and wearable systems, respectively. Exploring the nexus of biomedical theory and practice, these findings lay the groundwork for digital biomarkers of hemorrhage severity and warrant further study in human subjects.

A key aspect of work processes in service sector firms is the interconnection between tasks and performance. Relational coordination can play an important role in addressing the issues of coordinating organizational activities due to high level of interdependence complexity in service sector firms. Research has primarily supported the aspect that well devised high performance work systems (HPWS) can intensify organizational performance. There is a growing debate, however, with regard to understanding the “mechanism” linking HPWS and performance outcomes. Using relational coordination theory, this study examines a model that examine the effects of subsets of HPWS, such as motivation, skills and opportunity enhancing HR practices on relational coordination among employees working in reciprocal interdependent job settings. Data were gathered from multiple sources including managers and employees at individual, functional and unit levels to know their understanding in relation to HPWS and relational coordination (RC) in 218 bank branches in Pakistan. Data analysis via structural equation modelling, results suggest that HPWS predicted RC among officers at the unit level. The findings of the study have contributions to both, theory and practice.

BACKGROUND: The modern Internet of Things (IoT) makes small devices that can sense, process, interact, connect devices, and other sensors ready to understand the environment. IoT technologies and intelligent health apps have multiplied. The main challenges in the sports environment are playing without injuries and healthily. OBJECTIVE: In this paper the Internet of Things-based Smart Wearable System (IoT-SWS) is introduced for monitoring sports person activity to improve sports person health and performance in a healthy way. METHOD: Wearable systems are commonly used to capture individual sports details on a real-time basis. Collecting data from wearable devices and IoT technologies can help organizations learn how to optimize in-game strategies, identify opponents’ vulnerabilities, and make smarter draft choices and trading decisions for a sportsperson. RESULTS: The experimental result shows that IoT-SWS achieve the highest accuracy of 98.22% and efficient in predicting the sports person’s health to improve sports person performance reliably.

Formal music performance studies within university settings strive to prepare the next generation of performers and pedagogues for musical engagement beyond university. Yet literature suggests that these spaces of study do not always lead to a sense of readiness for potential professional worlds, due in part to a lack of opportunities for guided, in-depth, critical reflection that helps students connect theory and practice. This article articulates findings from a study that sought to consider the impact of deliberate opportunities for reflection in The Accademia Europea dell’Opera (AEDO), a university-affiliated summer opera intensive experiential learning program. Utilizing a communities of musical practice framework, researchers worked collaboratively to help participants engage in guided critical reflection as they developed high-level musical skills through rehearsals and performances. This article specifically considers the ways in which a ‘broker’ helped participants develop practices of reflection and personal agency both within and beyond this context.

Background COVID-19-related mortality in Belgium has drawn attention for two reasons: its high level, and a good completeness in reporting of deaths. An ad hoc surveillance was established to register COVID-19 death numbers in hospitals, long-term care facilities (LTCF) and the community. Belgium adopted broad inclusion criteria for the COVID-19 death notifications, also including possible cases, resulting in a robust correlation between COVID-19 and all-cause mortality. Aim To document and assess the COVID-19 mortality surveillance in Belgium. Methods We described the content and data flows of the registration and we assessed the situation as of 21 June 2020, 103 days after the first death attributable to COVID-19 in Belgium. We calculated the participation rate, the notification delay, the percentage of error detected, and the results of additional investigations. Results The participation rate was 100% for hospitals and 83% for nursing homes. Of all deaths, 85% were recorded within 2 calendar days: 11% within the same day, 41% after 1 day and 33% after 2 days, with a quicker notification in hospitals than in LTCF. Corrections of detected errors reduced the death toll by 5%. Conclusion Belgium implemented a rather complete surveillance of COVID-19 mortality, on account of a rapid investment of the hospitals and LTCF. LTCF could build on past experience of previous surveys and surveillance activities. The adoption of an extended definition of ‘COVID-19-related deaths’ in a context of limited testing capacity has provided timely information about the severity of the epidemic.

Coffee and green tea are two of the most widely consumed hot beverages in the world. Their respective bioavailability has been studied separately, but absorption of their respective bioactive phenolics has not been compared. In a randomised cross-over design, nine healthy subjects drank instant coffee and green tea. Blood samples were collected over 12 h and at 24 h to assess return to baseline. After green tea consumption, ( − )-epigallocatechin (EGC) was the major catechin, appearing rapidly in the plasma; ( − )-EGC gallate (EGCg) and ( − )-epicatechin (EC) were also present, but ( − )-EC gallate and C were not detected. Dihydroferulic acid and dihydrocaffeic acid were the major metabolites that appeared after coffee consumption with a long time needed to reach maximum plasma concentration, suggesting metabolism and absorption in the colon. Other phenolic acid equivalents (caffeic acid (CA), ferulic acid (FA) and isoferulic acid (iFA)) were detected earlier, and they peaked at lower concentrations. Summations of the plasma area under the curves (AUC) for the measured metabolites showed 1·7-fold more coffee-derived phenolic acids than green tea-derived catechins (P = 0·0014). Furthermore, we found a significant correlation between coffee metabolites based on AUC. Inter-individual differences were observed, but individuals with a high level of CA also showed a correspondingly high level of FA. However, no such correlation was observed between the tea catechins and coffee phenolic acids. Correlation between AUC and maximum plasma concentration was also significant for CA, FA and iFA and for EGCg. This implies that the mechanisms of absorption for these two classes of compounds are different, and that a high absorber of phenolic acids is not necessarily a high absorber of catechins.

Searching for items that are useful given current goals, or “target” recognition, requires observers to flexibly attend to certain object properties at the expense of others. This could involve focusing on the identity of an object while ignoring identity-preserving transformations such as changes in viewpoint or focusing on its current viewpoint while ignoring its identity. To effectively filter out variation due to the irrelevant dimension, performing either type of task is likely to require high-level, abstract search templates. Past work has found target recognition signals in areas of ventral visual cortex and in subregions of parietal and frontal cortex. However, target status in these tasks is typically associated with the identity of an object, rather than identity-orthogonal properties such as object viewpoint. In this study, we used a task that required subjects to identify novel object stimuli as targets according to either identity or viewpoint, each of which was not predictable from low-level properties such as shape. We performed functional MRI in human subjects of both sexes and measured the strength of target-match signals in areas of visual, parietal, and frontal cortex. Our multivariate analyses suggest that the multiple-demand (MD) network, including subregions of parietal and frontal cortex, encodes information about an object’s status as a target in the relevant dimension only, across changes in the irrelevant dimension. Furthermore, there was more target-related information in MD regions on correct compared with incorrect trials, suggesting a strong link between MD target signals and behavior. NEW & NOTEWORTHY Real-world target detection tasks, such as searching for a car in a crowded parking lot, require both flexibility and abstraction. We investigated the neural basis of these abilities using a task that required invariant representations of either object identity or viewpoint. Multivariate decoding analyses of our whole brain functional MRI data reveal that invariant target representations are most pronounced in frontal and parietal regions, and the strength of these representations is associated with behavioral performance.

Abstract The Commission has proposed a regulation ‘on clinical trials on medicinal products for human use’ to introduce one regulatory framework for clinical trials in the European Union. This regulation should replace the current clinical trials directive (2001/20/EC). In this article we describe and critically review the main provisions of the proposed regulation. We assess the consequences for a sound authorisation procedure of clinical trials and the level of protection for human subjects. We note that the proposed regulation is inconsistent with applicable international legal documents, such as the Biomedicine Convention and the Declaration of Helsinki. We conclude that the proposed regulation does not ensure a “high level of human health protection” — required by its legal basis in the TFEU — because it may force Member States concerned to accept a reporting Member States’ approval of — in their estimation — an unethical clinical trial.

This article argues that teenagers become fully capable of consenting to participation in most IRB-approved research involving human subjects at age 14, four years earlier than they are allowed to consent under virtually all states' laws, and, consequently, four years younger than they are able to consent under currently applicable federal regulations. In determining the age at which person is old enough to have decision-making authority, legal institutions look at the intellectual and emotional maturity of someone of the age of the decision-maker, the risks and benefits of allowing the decision to be made by someone of that age, and the risks and benefits of denying a person of that age the authority to make the decision. Given the high level of safety of participating in IRB approved research, the value of doing so for both the society and the teenage subject, and the psychological and neuropsychological research on the specific nature of emotional and intellectual development during the teen years, the balance comes out in favor of allowing younger teens, by the age of 14, authority to consent to participate as subjects in IRB approved research. The current process requiring both teen assent and parental permission should give way to a process that requires only a teen's consent.

As minimally invasive operations evolve, it is imperative to evaluate the advantages and risks involved. The aim of our study was to evaluate our institution's experience in incorporating a robotic platform for transhiatal esophagectomy (THE). Patients undergoing robotic THE were prospectively followed. Data are presented as median (mean ± SD). Forty-five patients were of 67 (67 ± 6.9) years and BMI 26 (27 ± 5.5) kg/m2. Nine per cent of operations were converted to “open,” but none in the last 25 operations. Operative duration of robotic THE was 334 (364 ± 108.8) minutes and estimated blood loss was 200 (217 ± 144.0) mL, which decreased with time ( P = 0.017). Length of stay was 8 (12 ± 11.1) days. Twenty per cent had respiratory failure requiring intubation that resolved, 4 per cent developed pneumonia, 11 per cent developed a surgical site infection, 2 per cent developed renal insufficiency, and 2 per cent developed a UTI. Two per cent (one patient) died within 30 days postoperatively, because of cardiac arrest. Our experience with robotic THE promotes robotic application because we endeavor to achieve high-level proficiency. With experience, we improved estimated blood loss and converted fewer transhiatal esophagectomies to “open.” Our length of hospital stay seems long but reflects the ill-health of patients, as does the variety of complications. Our data support the evolving future of THE, which will integrally include a robotic approach.
