AbstractThis animal study aimed to elucidate the relationship of low-dose, narrow-band UVB at 308 nm with vitamin D synthesis. C57BL/6 female mice, at 3 weeks-of-age, were randomly divided into the following six groups (n = 6 at each time point of vitamin D measurement), which were: (1) normal diet without UVB irradiation; (2) VDd diet without UVB irradiation; and (3)–(6) VDd diet with 308 nm-UVB irradiation of 12.5, 25, 50, and 100 μω/cm2, respectively. All of the groups needing UVB irradiation received an exposure of 10 min per day, five days per week, and a duration of 3–5 weeks. The mice recovering from severe VDd (plasma total 25-hydroxyvitamin D level increasing from approximately 3 to over 30 ng/mL) only occurred in groups with a UVB irradiation dosage of either 50 or 100 μω/cm2. The optimal, estimated dosage for mice to recover from severe VDd was 355 mJ/cm2 within 3 weeks. Low-dose, narrow-band UVB irradiation at 308 nm is effective in improving VDd in mice. The results obtained, in addition to the especially small side effects of the above UVB irradiation formula, could be further translated to treating VDd-related disorders.

Vitiligo adalah kelainan kulit yang ditandai dengan hilangnya pigmentasi akibat hilangnya melanosit epidermis fungsional. Pajanan sinar matahari dapat memicu peningkatan reactive oxygen species (ROS) pada kulit vitiligo. Simple 1-week sunlight exposure recall (S1WSER) telah digunakan untuk mengetahui hubungan pajanan sinar matahari dengan kadar serum vitamin D yang berperan pada penyakit vitiligo. Penelitian ini bertujuan untuk mengetahui peran pajanan sinar matahari terhadap perubahan fungsi biologis kulit vitiligo. Subjek penelitian ini adalah pasien vitiligo yang menjalani fototerapi narrow band ultraviolet B (NB -UVB). Jumlah pajanan sinar matahari harian dinilai dengan S1WSER yang terdiri atas total body areas exposed by sunlight (TBAES) dan time exposed by sunlight (TTES). Fungsi biologis kulit yang diukur adalah trans epidermal water loss (TEWL) menggunakan tewameter dan hidrasi kulit menggunakan korneometri pada kulit lesi yang terpajan sinar matahari (sun exposed lesion/SEL), lesi yang tidak terpajan sinar matahari (non sun exposed lesion/ NSEL), kulit tanpa lesi yang terpajan sinar matahari (sun exposed non lesion/SENL), dan kulit tanpa lesi yang tidak terpajan sinar matahari (non sun exposed non lesion/NSENL). Hasil penelitian menunjukkan hubungan bermakna (p<0.05) antara TBAES dengan selisih nilai korneometri SEL dan NSEL dengan kekuatan korelasi lemah (r<0.6).Sinar matahari berperan dalam perubahan fungsi biologis kulit pada vitiligo, pada penelitian ini didapatkan korelasi positif antara TBAES dengan selisih nilai korneometri SEL dan NSEL.Kata kunci: vitiligo, fungsi biologis kulit, pajanan sinar matahari

Cholecalciferol (vitamin D) might play a physiological role in photo-induced melanogenesis in human skin. We estimated the levels of 25-hydroxy vitamin D [25(OH)D] before, during, and after Narrow Band Ultraviolet B (NBUVB) radiation in patients of vitiligo and their correlation with NBUVB induced pigmentation. Thirty patients of vitiligo and equal number of age and sex matched controls were recruited for the study. Vitiligo patients were treated with NBUVB thrice weekly for 12 weeks. [25(OH)D] levels and Vitiligo Area and Severity Index (VASI) were calculated at 0 (baseline), 6, and 12 weeks. Baseline [25(OH)D] levels were measured in controls. Significant reduction in VASI score was observed after 12 weeks of therapy. Comparison and correlation between mean improvement in VASI and [25(OH)D] levels at 12 weeks showed moderate correlation, and the results were statistically insignificant. Mean reduction in VASI and increase in [25(OH)D] levels after 12 weeks of NBUVB showed moderate correlation. Thus, vitamin D might play a significant role in photo-induced melanogenesis. However, there might be additional effects of the phototherapy on melanogenesis. The complete mechanism of NBUVB induced pigmentation in vitiligo needs to be elucidated.
