Risk Indicators for Major Hazards on Offshore Installations

The Petroleum Safety Authority Norway took in 1999 the initiative to develop an approach in order to assess trends and status for the risk levels in the Norwegian offshore petroleum industry, covering major hazards, as well as occupational hazards, health hazards and risk perception, HES culture, motivation and communication. The results are expressed on a national level as well on an installation level, where appropriate. Lagging as well as leading indicators are used. There are individual indicators for a number of barrier elements, including fire and gas detection, a number of safety valves, active fire protection and mustering of personnel. Lessons learned from the project are discussed followed by a discussion of how these lessons may be utilised by individual companies to develop risk indicators for individual installations as well as company wide indicators.

 This paper reviews and analyses the present status of private forests and tenure administration in light of existing legal, policy, and regulatory frameworks in Nepal. Additionally, the present status of private forests, as well as the scenarios of timber harvesting, transportation, marketing, and their administration are thoroughly revised. Provisions regarding forests and trees on private land and their basis are examined and implications are articulated for potential policy improvements for enhanced tenure security. It is shown that robust national-level policies and legal frameworks exist, and that there is an increasing trend of timber flows to markets from private forests over the past five years. However, there is still skepticism, mistrust and fear amongst private forest owners, saw millers, and forest administration that prevents the full use of the bundle of rights that legal and policy provisions have promised. An unusually slow pace of private forest registration, lengthy and multi stage processes for obtaining harvesting and transportation permits, and official bans on important commercial species, among others, are found to be the factors that most hinder the private forest owners’ and tree growers’ interests, and their rights and obligations with respect to the management and use of their private forest resources. It is concluded that a simplified permitting process along with programmatic support would promote and help to grow private forestry and that Nepal’s experience and lessons learned from community forest implementation would be a great asset to move towards this end. Connecting community forest user groups for organised and cooperative action, and mobilising their institutional strength and accumulated funds for pro-farmer technical and regulatory support would allow farmers to intensify tree plantations and forest management. Further steps are required to convince policymakers and secure necessary budgetary support to this end..

Healthcare workers (HCWs) are at increased risk of infection from SARS-CoV-2 and other disease pathogens, which take a disproportionate toll on HCWs, with substantial cost to health systems. Improved infection prevention and control (IPC) programmes can protect HCWs, especially in resource-limited settings where the health workforce is scarcest, and ensure patient safety and continuity of essential health services. In response to the COVID-19 pandemic, we collaborated with ministries of health and development partners to implement an emergency initiative for HCWs at the primary health facility level in 22 African countries. Between April 2020 and January 2021, the initiative trained 42 058 front-line HCWs from 8444 health facilities, supported longitudinal supervision and monitoring visits guided by a standardised monitoring tool, and provided resources including personal protective equipment (PPE). We documented significant short-term improvements in IPC performance, but gaps remain. Suspected HCW infections peaked at 41.5% among HCWs screened at monitored facilities in July 2020 during the first wave of the pandemic in Africa. Disease-specific emergency responses are not the optimal approach. Comprehensive, sustainable IPC programmes are needed. IPC needs to be incorporated into all HCW training programmes and combined with supportive supervision and mentorship. Strengthened data systems on IPC are needed to guide improvements at the health facility level and to inform policy development at the national level, along with investments in infrastructure and sustainable supplies of PPE. Multimodal strategies to improve IPC are critical to make health facilities safer and to protect HCWs and the communities they serve.

Abstract The 2014 Wood Review is a report reviewing UK offshore oil and gas recovery and its regulation, led by Sir Ian Wood. The report identifies and addresses key challenges in the UK petroleum industry, among them the lack of a strong regulatory body and a decommissioning strategy. The UK petroleum industry is mature, and Norway may benefit from UK's experiences in decommissioning. The article investigates the applicability of the Wood Review recommendations for decommissioning in Norway. The analysis of the recommendations in the Wood Review is carried out by a SWOT-analysis of the general recommendations with a high potential impact on decommissioning as well as the five recommendations specific to decommissioning. The recommendations in the Wood Review were broadly accepted by UK authorities and formed the basis for numerous initiatives aimed at improving policies and practices in UK decommissioning. The key initiatives are presented to illustrate how the Wood Review recommendations has been interpreted. A summary of the key differences between the petroleum industries and the regulatory authorities in Norway and the UK is provided for background. Decommissioning in Norway face similar challenges to those identified in the Wood Review. The analysis indicates that several of the UK initiatives following the recommendations in the Wood Review has the potential of improving decommissioning in Norway. Differences in regulatory regimes between the regions may complicate the implementation of some of the initiatives following the Wood Review in Norway. In most cases only minor changes to regulations and/or practices are required. Recent UK initiatives with a high impact on decommissioning include increased focus on sharing of information and lessons learned, increased collaboration, the development of a decommissioning strategy, benchmarking of decommissioning cost estimates for all projects and the development and publishing of annual UK decommissioning cost estimates. There are indications that the Norwegian Petroleum Directorate (NPD) and the Norwegian Ministry of Petroleum and Energy (MPE) are falling behind their UK counterparts in key areas. Norway has limited experience with decommissioning, and scrupulous analysis of lessons learned in other regions is essential. Decommissioning of Norwegian offshore infrastructure is a major undertaking and even minor improvements may have a substantial impact on personnel risk, risk to the environment or the total decommissioning expenditure. The Norwegian regulatory regime has been an integral part of the Norwegian petroleum industry's success in previous decades, and changes to the regime require careful deliberation. The recent implementation of initiatives aimed at improving decommissioning regulations and practices in the UK represents a unique learning opportunity for Norwegian authorities. The analysis suggest that Norway may benefit from adopting some of the UK initiatives following the Wood Review recommendations.

Broadband deployment is a necessity nowadays. It could help each country, municipality and region to grow and offer better quality of life to the citizens. Today, the emphasis on the development of broadband networks is on fixed Fibre To The Home solutions The lessons learned from countries that are leaders in broadband penetration and Fibre To The Home deployment could be proven very useful for under-served communities, regions and countries where the broadband penetration is low. Therefore, this chapter summarises the lessons learned from implementing (a) country-wide strategies formulated at the national level, and (b) local strategies formulated by the municipalities. Concerning the role of national and local governments, it should be noted that nowadays it is very urgent the involvement of government in the development of broadband infrastructure. Proposed noteworthy remarkable cases are Japan, South Korea and Singapore.

Introduction:Patan Hospital, located in Kathmandu Valley, Nepal is a 400-bed hospital that has a long history of responding to natural disasters. Hospital personnel have worked with the Ministry of Health (MOH) and the World Health Organization (WHO) to develop standardized disaster response plans that were implemented in multiple hospital systems after the earthquake of 2015. These plans focused primarily on traumatic events but did not account for epidemics despite the prevalence of infectious diseases in Nepal.Aim:To develop and test a robust epidemic/pandemic response plan at Patan Hospital in Kathmandu that would be generalizable to other hospitals nationwide.Methods:Using the existing disaster plan in conjunction with public health and disaster medicine experts,we developed an epidemic response plan focusing on communication and coordination (between the hospital and MOH, among hospital administration and staff), logistics and supplies including personal protective equipment (PPE), and personnel and hospital incident command (IC) training. After development, we tested the plan using a high-fidelity, real-time simulation across the entire hospital and the hospital IC using actors and in conjunction with the MOH and WHO. We adjusted the plan based on lessons learned from this exercise.Results:Lessons learned from the high-fidelity simulation included the following: uncovering patient flow issues to avoid contamination/infection; layout issues with the isolation area, specifically accounting for donning/doffing of PPE; more sustained duration of response compared to a natural disaster with implications for staffing and supplies; communication difficulties unique to epidemics; need for national and regional surveillance and inter-facility planning and communication. We adjusted our plan accordingly and created a generalizable plan that can be deployed at an inter-facility and national level.Discussion:We learned that this process is feasible in resource-poor hospital systems. Challenges discovered in this process can lead to better national and system-wide preparedness.

PurposeThis study provides a safety prewarning mechanism, which includes a comprehensive risk assessment model and a safety prewarning system. The comprehensive risk assessment model is capable of assessing nine safety indicators, which can be categorised into workers’ behaviour, environment and machine-related safety indicators, and the model is embedded in the safety prewarning system. The safety prewarning system can automatically extract safety information from surveillance cameras based on computer vision, assess risks based on the embedded comprehensive risk assessment model, categorise risks into five levels and provide timely suggestions.Design/methodology/approachFirstly, the comprehensive risk assessment model is constructed by adopting grey multihierarchical analysis method. The method combines the Analytic Hierarchy Process (AHP) and the grey clustering evaluation in the grey theory. Expert knowledge, obtained through the questionnaire approach, contributes to set weights of risk indicators and evaluate risks. Secondly, a safety prewarning system is developed, including data acquisition layer, data processing layer and prewarning layer. Computer vision is applied in the system to automatically extract real-time safety information from the surveillance cameras. The safety information is then processed through the comprehensive risk assessment model and categorized into five risk levels. A case study is presented to verify the proposed mechanism.FindingsThrough a case study, the result shows that the proposed mechanism is capable of analyzing integrated human-machine-environment risk, timely categorising risks into five risk levels and providing potential suggestions.Originality/valueThe comprehensive risk assessment model is capable of assessing nine risk indicators, identifying three types of entities, workers, environment and machine on the construction site, presenting the integrated risk based on nine indicators. The proposed mechanism, which adopts expert knowledge through Building Information Modeling (BIM) safety simulation and extracts safety information based on computer vision, can perform a dynamic real-time risk analysis, categorize risks into five risk levels and provide potential suggestions to corresponding risk owners. The proposed mechanism can allow the project manager to take timely actions.

This study assesses greenhouse gas emission (GHG) reductions and sustainable development impacts connected to the nationally appropriate mitigation action (NAMA) on sustainable charcoal production in Mozambique. The analysis covers the results of the ex-ante assessment of the NAMA potential contribution to the achievement of Mozambique’s Nationally Determined Contribution (NDC) target. The expected impacts show that the proposed actions tied to NAMA (e.g., introducing improved kilns, sustainable forest management, briquetting of charcoal waste and torrefaction) can cumulatively lead to emission reductions amounting to 314,521 ± 45,138 t CO2-eq (119% reduction) by 2025, and 442,706 ± 26,766 t CO2-eq (113% reduction) by 2030 at the national level, compared to a business as usual scenario. This shifting represents a transformation of the charcoal sector from a net source of emissions to net carbon sequestrating. The analysis also identifies a wide range of sustainable development cobenefits, including increased income, improved gender equity, job creation (23% increase by 2025, and 15% decrease by 2030), and increased revenue (USD 825,000 by 2025, and USD 1.6 million by 2030). The assessment process concluded that unless robust data collection, processing, and sharing is put in place, a full assessment of all direct and indirect environmental, social, and economic impacts cannot be comprehensively reported. Finally, we highlight the lessons learned and specific barriers for a robust monitoring, reporting, and verification (MRV) of the given policy under the current MRV set up and available capacities.
