Fracture Injury Risk of the Restrained Mandible to Anterior–Posterior Blunt Impacts

Abstract This study describes the results of anterior–posterior impacts conducted on the mandibles of 22 male postmortem human subjects (PMHSs). The objective of this study was to develop an injury criterion for the mandible based on blunt impact while the jaw was restrained. Previous studies have attempted to characterize the injury risk of blunt impact to the mandible; however, due to the translation of the mandible during impact and a limited number of fractured specimens, previous studies were not able to produce an injury criterion. Blunt impact to a restrained mandible is relevant to a wide array of helmeted individuals, including the military population and sports that require helmets with chinstraps. Therefore, in this study, specimens were positioned with restrained jaws and impacted using a monorail drop tower with a gravity-driven cylindrical impactor. Nineteen of 22 specimens sustained at least one fracture during testing. Injury cases had an average impact energy of 15.0 ± 5.7 J (11.1 ± 4.2 ft-lb) and a fracture force of 2684 ± 726 N (603 ± 163 lbf). Results were used to generate an impactor force based injury criterion through survival analysis. Risk of injury was modeled using a Weibull distribution and a 50% risk of injury was found to occur at approximately 2834 N (637 lbf). The developed injury risk curve can be used to characterize injury to the restrained mandible for future testing and research studies, especially in the development of maxillofacial protective equipment.

Rugby is a demanding contact sport. In light of research, poor balance, reduced jumping ability, muscle strength, and incorrect landing patterns might contribute to the increased risk of injury in athletes. Investigating the relationship between tests assessing these abilities might not only allow for the skillful programming of preventive training but also helps in assessing the risk of injury to athletes. Thus, the main purpose of this study was to investigate the relationship between dynamic balance, vertical and horizontal jumps, and jump-landings movement patterns. Thirty-one healthy amateur adolescent rugby players (age: 14.3 ± 1.6 years, height 171.4 ± 9.7 cm, body mass 80 ± 26 kg) participated in the study. Data were collected by the Y-balance Test (YBT), Counter Movement Jump (CMJ), Single Leg Hop for Distance (SLHD), and Landing Error Score System (LESS). Significant positive correlations were found between SLHD both legs (SLHDb) and YBT Composite both legs (COMb) (r = 0.51, p = 0.0037) and between SLHDb and CMJ (r = 0.72, p < 0.0001). A relationship was also observed between the CMJ and YBT COMb test (r = 0.51, p = 0.006). Moderate positive correlations were found between the dominant legs in SLHD and the posterolateral (r = 0.40, p = 0.027), posteromedial (r = 0.43, p = 0.014), and composite (r = 0.48, p = 0.006) directions of the YBT. These results indicate that variables that are dependent on each other can support in the assessment of injury-risk and in enhancing sports performance of young athletes.

AbstractMale infertility is a condition that affects approximately 50% of infertile couples and about 30% of those couples have only male factor infertility identified. There has been speculation that male military service members may have an even greater lifetime prevalence of infertility as compared to the general population, but very few scientific publications focus on male factor infertility in the military population specifically. This review will discuss many of the unique considerations regarding male infertility in the military and highlight future opportunities for research. The military/federal health system has the potential to serve as a leader in both the delivery of health care for male factor infertility and in the clinical investigation into the etiologies of and treatments for male factor infertility.

Purpose:To examine the association between combined session rating of perceived exertion (RPE) workload measures and injury risk in elite Gaelic footballers.Methods:Thirty-seven elite Gaelic footballers (mean ± SD age 24.2 ± 2.9 y) from 1 elite squad were involved in a single-season study. Weekly workload (session RPE multiplied by duration) and all time-loss injuries (including subsequent-wk injuries) were recorded during the period. Rolling weekly sums and wk-to-wk changes in workload were measured, enabling the calculation of the acute:chronic workload ratio by dividing acute workload (ie, 1-weekly workload) by chronic workload (ie, rolling-average 4-weekly workload). Workload measures were then modeled against data for all injuries sustained using a logistic-regression model. Odds ratios (ORs) were reported against a reference group.Results:High 1-weekly workloads (≥2770 arbitrary units [AU], OR = 1.63–6.75) were associated with significantly higher risk of injury than in a low-training-load reference group (<1250 AU). When exposed to spikes in workload (acute:chronic workload ratio >1.5), players with 1 y experience had a higher risk of injury (OR = 2.22) and players with 2–3 (OR = 0.20) and 4–6 y (OR = 0.24) of experience had a lower risk of injury. Players with poorer aerobic fitness (estimated from a 1-km time trial) had a higher injury risk than those with higher aerobic fitness (OR = 1.50–2.50). An acute:chronic workload ratio of (≥2.0) demonstrated the greatest risk of injury.Conclusions:These findings highlight an increased risk of injury for elite Gaelic football players with high (>2.0) acute:chronic workload ratios and high weekly workloads. A high aerobic capacity and playing experience appears to offer injury protection against rapid changes in workload and high acute:chronic workload ratios. Moderate workloads, coupled with moderate to high changes in the acute:chronic workload ratio, appear to be protective for Gaelic football players.

ABSTRACT Insulinoma, the prototype of endogenous hyperinsulinemic hypoglycemia, is a very rare condition, with an incidence of four cases per million person-years. Its rate of occurrence in the U.S. military population is unknown. Two cases of insulinomas involving active duty service members have been published. However, there has been no reported case of an insulinoma in a deployed service member. We report the case of a 21-year-old infantryman with clinical hypoglycemia of insidious onset, manifesting with overt neuroglycopenic symptoms during his deployment as a combatant soldier, and the ultimate diagnosis of an insulinoma as the underlying cause. The series of multiple clinical evaluations and the unique circumstances leading to the formal evaluation of the patient’s hypoglycemia and treatment are chronicled. The significance of neuroglycopenia and the diagnostic approach to any suspected case of hypoglycemia, the potential challenges and opportunities, and educational aspects of evaluation and management of the insulinoma are elaborated. The potential role of the Military Health System in facilitating the detection and treatment of this rare condition in the service member is discussed as well.

Abstract Objective: To analyse the prevalence and independent risk factors of varicocele in the soldiers population. Methods: The results of ultrasound examination and clinical physical examination in military outpatient clinic were collected and analysed in our hospital between July 2019 and July 2020. The results were compared with ordinary outpatient patients.Results: Finally, 1169 military patients and 818 general outpatients were included in this study. Totally 225 military cases (19.24%) and 119 outpatient cases (14.55%) developed varicocele. The incidence of varicocele in the military group was higher than that of the outpatients (P<0.05). The number of bilateral and single right varicocele in the two groups were 29, 14 and 13, 7, respectively. Military men were younger and had smaller testicular volumes than ordinary people (P<0.05). The analysis of the military population revealed that reduced left testicular volumes (P=0.000, OR=0.896), increased right testicular volumes (P=0.033, OR=1.052) and the disappearance of fluid around testicular (P=0.007, OR=0.505) were independent risk factors for varicocele. There was no correlation between testicular volume, age and the diameter of spermatic vein (P>0.05).Conclusions: The incidence of varicocele in the military population was 19.24%, and the rate was higher than ordinary patients. Reduced left testicular volumes, increased right testicular volumes and the testicular hydrocele disappears were independent risk factors.

Push-up is the simplest and most widely performed sport. Although simple, it also has a high risk of injury risk if done not in accordance with the rules. Push-up detector is a good push-up motion monitoring solution. In this way, nonstandard movements can be detected and corrected immediately. It has two motion sensors integrated with Arduino-based microcontroller. From this detector tool got the data of push-up result from sensor mounted. Sensor data will be displayed in the application in real-time. Quality function development is used to determine the criteria of the user. The sample data involved 200 participants who followed the testing of this tool and got 90% who can do the push-up correctly. Factors that affect the height, age, and weight. Tests conducted on adolescent boys aged 18-23 years. The results of this study is an application capable of monitoring each push-up movement to position in accordance with the provisions to minimize injuries resulting from movement errors.

Introduction The objective of this study is to assess change over time in the modern contraceptive prevalence rate (MCPR) and related variables among married women of reproductive age (15–49 years) in the military population in Kinshasa, Democratic Republic of Congo, compared to women in the non-military population, based on cross-sectional surveys in 2016 and 2019. Methods Data among women living in military camps were collected as a special study of contraceptive knowledge, use, and exposure to FP messaging, for comparison to women in the non-military population from the annual PMA2020 survey. Both used a two-stage cluster sampling design to randomly select participants. This analysis is limited to women married or in union. Bivariate and multivariate analysis was used to compare the military and non-military populations. Results The socio-demographic profile of women in the military camps differed between 2016 and 2019, which may reflect the more mobile nature of this population. In both populations, knowledge of modern contraceptive methods increased significantly. Similarly, use of a modern contraceptive method also increased significantly in both, though by 2019 women in the military camps were less likely to use modern contraception (24.9%) than their non-military counterparts (29.7%). Multivariate analysis showed no significant difference in the amount of increase in MCPR for the two populations. Among contraceptive users in both populations, the implant was the leading method. Potential effects of FP programming were evident in the military population: exposure to FP messaging increased (in comparison to a decrease among the non-military population). Moreover, women who had lived in the camps for 4+ years had a higher MCPR than those living in the camps for less than four years. Conclusions This study demonstrates the feasibility and importance of collecting data in military camps for better understanding contraceptive dynamics among this specialized population.

The survey realized in 1992 on a military population allowed to collect anthropometric data on 688 males and 328 females. Among 73 measurements and 3 index, 26 of them have been retained for the comparison with previous surveys. Generally used for dimensioning human body models these data represent somatic measurements of reference, as weight and stature and segmentary measurements of trunk and limbs. A comparison with previous data, collected on a equivalent military population in 1973, confirms the modifications along the time are so significant that they can be considered as a phenomenon of morphological evolution. Likewise, the modification of the academic levels, average age and socio-cultural structures in the populations are combined to increase the anthropometric variability. It appears the military population presents a morphological modification with an overall increase in weight, stature and correlated dimensions. Otherwise, a light decrease of the cormic index indicates that the morphological transformation influences on the body proportions, with an increase more notable for the lower limbs compared to the trunk. The collected anthropometric information allow to update the Individual Database of ERGODATA from which ergonomie recommendations and statistical and morphological models of the human body can be proposed.

Horseracing stable staff have a multifaceted role, acting as care givers, skilled athletes and equine experts, resulting in inherent high emotional, physical and mental demands. Despite these demands, and reports of significant injury risk to those working in racing at ground level, there has been no research investigating the psychological risk factors for injury in UK racing stable staff and how psychological responses to injury may affect long term mental health and wellbeing. With mental health at the forefront of industry consideration, this narrative review aimed to reflect on the current literature to propose occupational, life and social stressors that may be increasing the risk of injury in racing staff. In addition, this review aims to apply current injury theory to propose whether stable staff are at heightened risk of injury mismanagement. A literature search was undertaken to investigate the psychological aspects of injury, using five search engines and the following key words: injury, psychological responses, occupational, sport and/or athlete, equestrian, horseracing, jockey, risk of injury, rehabilitation, and injury minimisalisation. Articles were selected based on relevance to review aims, and research findings were synthesised under two primary areas: predictors of injury (working hours, job security, job control, life stressors, mental health and drug misuse) and responses to injury (positive and negative responses to injury, the role of social support, rehabilitation adherence, and injury minimalisation). Whilst there are limitations to narrative reviews, this study provides a novel interpretation of injury risk and responses specific to staff working in horseracing, where injury-related challenges are currently a key focus for sector employees, managerial staff and racing organisations. Recommendations for further research have been made to direct the advancement of knowledge in the field.
