An Engineering Approach for Quantitative Analysis of the Lengthwise Strokes in Massage Therapies

Massage therapies are widely used for improving and restoring the function of human tissues. It is generally accepted that such therapies promote human health and well-being by several possible mechanisms, including increase in blood flow and parasympathetic activity, release of relaxation hormones, and inhibition of muscle tension, neuromuscular excitability, and stress hormones. Nonetheless, most of the purported beneficial/adverse effects of massage are based on anecdotal experiences, providing little insight on its effectiveness or the mechanisms underlying its usefulness. Furthermore, most studies to date have not quantitatively demonstrated the efficacy of massage on human health. This might be due to the lack of appropriate tools necessary for the application of quantitatively controlled loading and for the evaluation of the subsequent responses. To address this issue, we developed a device that applies compression in lengthwise strokes to the soft tissues of the New Zealand white rabbit, thereby mimicking the rubbing and effleurage techniques of massage. This device permits control of the magnitude and frequency of mechanical load applied to the rabbit’s hind limb for various durations. The measurement of tissue compliance and the viscoelastic properties as a function of loading parameters was also demonstrated. Findings of this study suggest that this device offers a quantitative analysis of the applied loads on the tissue to determine an optimal range of loading conditions required for the safe and effective use of massage therapies.

Physical therapies using mechanical loadings are widely used for improving and recovering the physical activities of human tissues. It is generally accepted that such therapies promote health and well-being by many mechanisms, including fastening muscle blood flow, parasympathetic activity, releasing relaxation hormones and inhibiting muscle tension, neuromuscular excitability and stress hormones. Nonetheless, most of current research in this area is based on statistics and thus qualitative, preventing the in-depth study of the effectiveness of these therapies. It is partially due to the lack of appropriate tools for quantitative loading and in situ tissue evaluation. To address this, we developed a medical device that resembles the mechanical motions and loadings that occur in massage therapies by applying combinations of compressive and shear loadings to the subject tissues. This device consists of a loading wheel, a force sensor, a pneumatic actuator, a control system and a data acquisition system. In this work, mechanical forces were applied to the lower limbs of rabbits with controllable magnitudes, frequencies and durations. The changes of mechanical properties of the subjects, including the compliance and the viscosity, were in situ measured as a function of the loading dose, and correlated to the results from biomolecular assay. This device can quickly identify the optimal sets of loading parameters which lead to high effectiveness, and thus provide guidance to practitioners to design their therapies. It is also expected to shed light on the fundamental study of biomechanical forces in regulation of the physiologic conditions of cells and tissues.

To address this, we develop a biomechanical device to mimic massage therapies by applying controllable mechanical forces to animal tissues during cyclic mechanical motions. The device can apply compressive loads normal to the tissue surface and generate lengthwise motion along the tissue surface. Mechanical forces are applied with controllable magnitudes, frequencies and durations. Tissue mechanical response is recorded and correlated to the loading parameters. The changes of bulk tissue compliance and viscoelastic properties under various loading conditions are evaluated. The improvement of tissue functions and inhibition of muscle inflammation are examined. The results show that the peak torque production increased after massage, which suggests the recovery of muscle functions. A reduced number of infiltrating leukocytes is also observed in the subject muscle fibers after massage. Findings of this study suggest that the biomechanical device offers a quantitative analysis of massage actions, which will help to determine the optimal range of loading conditions required for safe and effective use of massage therapies.

The late Dr. Fazlur Rahman, Harold H. Swift Distinguished Service Professor of Islamic Thought at the Oriental Institute of the University of Chicago, has written this book as number seven in the series on Health/Medicine and the Faith Traditions. This series has been sponsored as an interfaith program by The Park Ridge Center, an Institute for the study of health, faith, and ethics. Professor Rahman has stated that his study is "an attempt to portray the relationship of Islam as a system of faith and as a tradition to human health and health care: What value does Islam attach to human well-being-spiritual, mental, and physical-and what inspiration has it given Muslims to realize that value?" (xiii). Although he makes it quite clear that he has not attempted to write a history of medicine in Islam, readers will find considerable depth in his treatment of the historical development of medicine under the influence of Islamic traditions. The book begins with a general historical introduction to Islam, meant primarily for readers with limited background and understanding of Islam. Following the introduction are six chapters devoted to the concepts of wellness and illness in Islamic thought, the religious valuation of medicine in Islam, an overview of Prophetic Medicine, Islamic approaches to medical care and medical ethics, and the relationship of the concepts of birth, contraception, abortion, sexuality, and death to well-being in Islamic culture. The basis for Dr. Rahman's study rests on the explication of the concepts of well-being, illness, suffering, and destiny in the Islamic worldview. He describes Islam as a system of faith with strong traditions linking that faith with concepts of human health and systems for providing health care. He explains the value which Islam attaches to human spiritual, mental, and physical well-being. Aspects of spiritual medicine in the Islamic tradition are explained. The dietary Jaws and other orthodox restrictions are described as part of Prophetic Medicine. The religious valuation of medicine based on the Hadith is compared and contrasted with that found in the scientific medical tradition. The history of institutionalized medical care in the Islamic World is traced to awqaf, pious endowments used to support health services, hospices, mosques, and educational institutions. More than 80% of the people in the USA and Canada live in cities. Urban development replaces natural environments with built environments resulting in limited access to outdoor environments which are critical to human health and well-being. In addition, many urban open spaces are unused because of poor design. This paper describes case studies where traditional landscape architectural design approaches would have compromised design success, while evidence-based landscape architecture (EBLA) resulted in a successful product. Examples range from school-yard design that provides safe levels of solar radiation for children, to neighborhood parks and sidewalks that encourage people to walk and enjoy nearby nature. Common characteristics for integrating EBLA into private, public, and academic landscape architecture practice are outlined along with a discussion of some of the opportunities and barriers to implementation.

IntroductionIncreasing urban populations have led to the growth of informal settlements, with contaminated environments linked to poor human health through a range of interlinked pathways. Here, we describe the design and methods for the Revitalising Informal Settlements and their Environments (RISE) study, a transdisciplinary randomised trial evaluating impacts of an intervention to upgrade urban informal settlements in two Asia-Pacific countries.Methods and analysisRISE is a cluster randomised controlled trial among 12 settlements in Makassar, Indonesia, and 12 in Suva, Fiji. Six settlements in each country have been randomised to receive the intervention at the outset; the remainder will serve as controls and be offered intervention delivery after trial completion. The intervention involves a water-sensitive approach, delivering site-specific, modular, decentralised infrastructure primarily aimed at improving health by decreasing exposure to environmental faecal contamination. Consenting households within each informal settlement site have been enrolled, with longitudinal assessment to involve health and well-being surveys, and human and environmental sampling. Primary outcomes will be evaluated in children under 5 years of age and include prevalence and diversity of gastrointestinal pathogens, abundance and diversity of antimicrobial resistance (AMR) genes in gastrointestinal microorganisms and markers of gastrointestinal inflammation. Diverse secondary outcomes include changes in microbial contamination; abundance and diversity of pathogens and AMR genes in environmental samples; impacts on ecological biodiversity and microclimates; mosquito vector abundance; anthropometric assessments, nutrition markers and systemic inflammation in children; caregiver-reported and self-reported health symptoms and healthcare utilisation; and measures of individual and community psychological, emotional and economic well-being. Abstract Purpose of review Biodiversity underpins urban ecosystem functions that are essential for human health and well-being. Understanding how biodiversity relates to human health is a developing frontier for science, policy and practice. This article describes the beneficial, as well as harmful, aspects of biodiversity to human health in urban environments. Recent findings Recent research shows that contact with biodiversity of natural environments within towns and cities can be both positive and negative to human physical, mental and social health and well-being. For example, while viruses or pollen can be seriously harmful to human health, biodiverse ecosystems can promote positive health and well-being. On balance, these influences are positive. As biodiversity is declining at an unprecedented rate, research suggests that its loss could threaten the quality of life of all humans. Summary A key research gap is to understand—and evidence—the specific causal pathways through which biodiversity affects human health. A mechanistic understanding of pathways linking biodiversity to human health can facilitate the application of nature-based solutions in public health and influence policy. Research integration as well as cross-sector urban policy and planning development should harness opportunities to better identify linkages between biodiversity, climate and human health. Given its importance for human health, urban biodiversity conservation should be considered as public health investment.

Electric light in buildings may provide some health benefits; however, for most people these benefits are likely to be small. It is possible for electric lighting to cause health problems, if there is too little light or there is glare, but for the most part there is good guidance available and these problems can be avoided. The quality of the lit environment can have a psychological impact and this may in turn impact well-being. A starting point for this is perceived adequacy of illumination. Related lighting metrics are examined and a hypothetical explanation is suggested.

Nutraceuticals are a category of products more often associated with food but having pharmaceuticals property and characteristics. However, there is still no internationally accepted concept of these food-pharmaceutical properties, and their interpretation can differ from country to country. Nutraceuticals are used as part of dietary supplements in most countries. They can be phytochemicals which are biologically active and have health benefits. These can be supplied as a supplement and/or as a functional food to the customer. For human health and longevity, these materials are likely to play a vital role. Consumption of these items is typical without a therapeutic prescription and/or supervision by the vast majority of the public. The development of nutraceuticals can be achieved through many bioresources and organisms. This review article will discuss the current research on nutraceuticals from different biological sources and their potential use as an agent for improving human health and well-being, as well as the gaps and future perspective of research related to nutraceutical development.

The health sector response to dealing with the impacts of climate change on human health, whether mitigative or adaptive, is influenced by multiple factors and necessitates creative approaches drawing on resources across multiple sectors. This short communication presents the context in which adaptation to protect human health has been addressed to date and argues for a holistic, transdisciplinary, multisectoral and systems approach going forward. Such a novel health-climate approach requires broad thinking regarding geographies, ecologies and socio-economic policies, and demands that one prioritises services for vulnerable populations at higher risk. Actions to engage more sectors and systems in comprehensive health-climate governance are identified. Much like the World Health Organization’s ‘Health in All Policies’ approach, one should think health governance and climate change together in a transnational framework as a matter not only of health promotion and disease prevention, but of population security. In an African context, there is a need for continued cross-border efforts, through partnerships, blending climate change adaptation and disaster risk reduction, and long-term international financing, to contribute towards meeting sustainable development imperatives.
