The Psychological Aspects of Injury in Sport

The benefit of sports and physical activity are widely known. However, the risk of injury is an unfavourable consequence in physical training. Age, gender, injury history, body size, local anatomy and biomechanics, aerobic fitness, muscle strength, psychological and psychosocial factors as well as general mental ability are factor in the predisposition to injury. It sports, regardless of the best efforts of coaches and trainers, injuries do occur. One of the most important aspects in sport injury is the psychological aspect. Which include psychological variables as predictors of injury occurrence, athlete's psychological response to injury, psychological aspects of the rehabilitation process, and also psychological readiness to return to competition. Psychological factors of injury occurrence in sports can be predicted. Among the factors already studied are the stress responses, personality, history of stressors, coping resources, stte–trait anxiety, self–esteem, and mental toughness. With regard to the psychological response to injury, a few factors such as emotions and behaviour of the injured athletes have been identified. Studies on phychological aspects of the rehabilition process have indicated that injured athletes need a number of approaches that can be implemented such as communication skills and motivation techniques. Nevertheless, injured athletes need to consider some psychological readiness factors to return to competition like anxiety, fear and loss of confidence which subsequently may affect their performance when they return to competition. Context: A multidisciplinary approach is one of the many forms of professional practice that can be utilized by sports medicine professionals to provide care to injured athletes. While this approach has been empirically supported in the health care domain, studies supporting its utilization in the sport injury rehabilitation context—particularly at the high school level—are limited. Objective: To investigate former high school athletes’ experiences of a multidisciplinary model of care for sport injury rehabilitation. Design: Cross-sectional survey design. Setting: In-person, in a classroom setting at 2 Division I universities. Patients: A total of 186 former high school athletes. Main Outcome Measure: An author-constructed instrument developed using the multidisciplinary model of care for sport injury rehabilitation as a guide. Results: Family, athletic coaches, and athletic trainers were the closest professionals/individuals that injured athletes reported interacting with during sport injury rehabilitation. The data also revealed that these professionals/individuals had the closest and most direct relationships with the injured athletes. Conclusions: The findings from the present study provided support for the utilization of the multidisciplinary model of care for sport injury rehabilitation with high school athletes.

Equestrian sport is considered a high-risk environment for equine injury. Due to the close bond between horse and rider, it could be theorised that riders may be impacted psychologically by their horses’ injuries, as seen in athletic pairs and with companion animal ownership. The extensive time commitment and responsibility of care within equestrian sport means that horse riders’ day-to-day life is impacted in a way not seen in other sporting or leisure environments, thus providing a unique opportunity to investigate the psychological responses of riders to their horse’s injury. The aims of this study were to investigate the psychological responses that amateur riders experienced when their horses were injured. 308 amateur horse riders (16 male and 292 female, median age 25-30 category) completed the Psychological Response to Sport Injury Inventory (19-item) (PRSII) and questions regarding demographics, investment in equestrian sport, the horse’s injury and the length of rehabilitation. Devastation was significantly affected by the weekly time investment of riders (H(3)=8.255, P=0.041) and the length of ownership prior to the injury (H(2)=7.690, P=0.021). ‘Devastation’, ‘feeling cheated’, ‘restlessness’ and ‘isolation’ were all significantly affected by the length of rehabilitation for the horse (H(7)=70.825, P=0.000, H(7)=37.799, P=0.000, H(7)=37.799, P=0.004, and H(7)=27.486, P=0.000, respectively). These findings suggest that amateur horse riders are at risk of psychological distress when their horse becomes injured. Whilst the industry has developed strategies to support owners following euthanasia which are already in place, psychological support following horse injury may be necessary to buffer psychological ‘devastation’ within amateur horse owners.

In addition to the disruptive impact of sport injury on physical functioning, injury can have psychological effects on athletes. Consistent with contemporary models of psychological response to sport injury, aspects of psychological functioning that can be affected by sport injury include pain, cognition, emotion, and behavior. Part of the fabric of sport and ubiquitous even among “healthy” athletes, pain is a common consequence of sport injury. Postinjury pain is typically of the acute variety and can be exacerbated, at least temporarily, by surgery and some rehabilitation activities. Cognitive responses to sport injury include appraising the implications of the injury for one’s well-being and ability to manage the injury, making attributions for injury occurrence, using cognitive coping strategies, perceiving benefits of injury, and experiencing intrusive injury-related thoughts and images, increased perception of injury risk, reduced self-esteem and self-confidence, and diminished neurocognitive performance. Emotional responses to sport injury tend to progress from a preponderance of negative emotions (e.g., anger, confusion, depression, disappointment, fear, frustration) shortly after injury occurrence to a more positive emotional profile over the course of rehabilitation. A wide variety of personal and situational factors have been found to predict postinjury emotions. In terms of postinjury behavior, athletes have reported initiating coping strategies such as living their lives as normally as possible, distracting themselves, seeking social support, isolating themselves from others, learning about their injuries, adhering to the rehabilitation program, pursuing interests outside sport, consuming alcohol, taking recreational and/or performance-enhancing substances, and, in rare cases, attempting suicide. Psychological readiness to return to sport after injury is an emerging concept that cuts across cognitive, emotional, and behavioral responses to sport injury.

The study reported here is part of a continuing research program investigating the links between psychological stress responses and performance in a variety of settings. A battery of psychological and cognitive measures designed to assess stress perceptions, coping resources, and cognitive performance was administered at selected times in association with the daily test activities of smoke and decontamination platoon operations. During testing soldiers wore the full chemical protective ensemble including mask (MOPP IV). Canonical correlation analyses were computed to examine the relationship between the stress perception measures (predictor variables) and cognitive performance measures (outcome variables). The direction of the results indicates that as subjects experience an increase in their perception of the situation as stressful their corresponding performance declines. These results are consistent with the literature and indicate that the stress perception measures and performance measures used in this study are sensitive indicators of stress.

The purpose of this study was to explore religiosity/spirituality (R/S) in coping with sport injuries, based on predictions of the integrated model of psychological response to the sport injury and rehabilitation process. A concurrent mixed methods design framed an online survey incorporating quantitative measures of R/S identification and commitment, health locus of control for sport injury, and ways of coping with sport injury, as well as qualitative open-ended questions about mechanisms through which R/S affected and was affected by coping with sport injuries. Participants included 49 physically active adults who experienced sport injuries, with 37 identifying as R/S. Quantitative findings among R/S participants showed religious commitment was a predictor of God health locus of control and positive religious coping. Quantitative results relative to differences between R/S and no-R/S participants showed that those self-identified as R/S relied on a God health locus of control and utilized active coping more so than did those self-identified as no-R/S, whereas no-R/S participants relied more than did R/S participants on an internal health locus of control. Thematic analyses exploring qualitative data revealed three main themes characterizing mechanisms through which R/S affected and was affected by coping with sport injuries: positive, negative, and no R/S coping strategies and effects. Findings support the predictions of the integrated model of psychological response to the sport injury and rehabilitation process and provide evidence-bases for clinical and counseling interventions that reflect cultural competence in accommodating patient or client R/S during recovery from sport injury.

Psychological Predictors of Injury among Professional Soccer Players Objectives: Numerous empirical studies suggest that specific psychological factors influence the frequency and severity of sport injuries. The main purpose of the present study is to outline the psychological factors, which predict increased injury vulnerability among professional male soccer players in Denmark. Based on the Stress-Injury Model by Williams & Anderson (1998) it is hypothesized that low coping resources, high competitive trait anxiety, and history of previous injuries would be positively related to an increased risk of injury occurrence and severity. Methods: The soccer players (N = 87) were asked to report history of previous injuries within the last 12 months. Furthermore, 2 questionnaires were used; Competitive Trait Anxiety Test, and Athletic Coping Skills Inventory - 28 (ACSI-28). Injuries were prospectively recorded throughout a period of approximately 3 months by the team's medical staff (doctors and physiotherapists). Results: Study findings clearly suggest that history of previous injury and coping with adversity are the best predictors of injury occurrence. These factors explained between 7 % and 11 % of the total variance of injury occurrence and days lost due to injury respectively. Furthermore, the same variables were found very successful in prediction injury occurrence. Conclusions: The findings support the suggestions that psychological factors can be utilized as a predictive measurement to sport injuries, which should be considered by coaches and medical staff in order to reduce vulnerability to injury.

Context Athletic trainers (ATs) have indicated a desire to better understand the motivations of athletes during rehabilitation. Self-determination theory offers an ideal lens for conceptualizing the antecedents, mediators, and consequences of motivated behavior. Objective To explore athletes' perceptions of ATs' influence on their basic psychological needs as well as their motivation during sport injury rehabilitation. Design Qualitative study. Setting National Collegiate Athletic Association Division I universities in the northwestern and southeastern United States. Patients or Other Participants A total of 10 injured and previously injured athletes (7 women, 3 men; mean age = 20.9 ± 2.0 years) active in a variety of sports. Data Collection and Analysis Participants completed semistructured interviews, which were transcribed verbatim. The data were analyzed using consensual qualitative research methods. Trustworthiness techniques (eg, bracketing interview, discussion of biases, member checking, external auditor) were used throughout the process. Results Four domains were constructed: (a) athletes' concerns about injury and rehabilitation, (b) ATs' feedback and athletes' perceptions of competence, (c) a person-centered approach from ATs and athletes' perceptions of autonomy, and (d) a connection between ATs' and athletes' perceptions of relatedness. Athletes' experiences were largely influenced by the degree to which they perceived that ATs satisfied their 3 basic psychological needs, which, in turn, was determined by the presence or absence of particular AT behaviors, such as providing encouragement (competence), soliciting input (autonomy), and building rapport (relatedness). Furthermore, the degree to which they perceived these basic psychological needs were fulfilled (or thwarted) affected their overall motivation during sport injury rehabilitation. Conclusions Self-determination theory is a promising framework for ATs to consider when addressing motivational challenges among injured athletes.

Psychological rehabilitation in response to physical injury is of primary concern to athletes, trainers, coaches, and sport psychologists. To date, there is little empirical research to shed light on this topic, as well as on the role of sport psychology practitioners in facilitating the prevention, rehabilitation, and recovery from athletic injuries. The purpose of this paper is to consolidate and report the information available on the nature of injuries and make suggestions concerning the application of sport psychology principles when working with injured athletes. Four major concerns are addressed with regard to current knowledge and practical implications: how injuries happen, how athletes respond to injuries, how psychological rehabilitation as well as physical recovery from injuries can be facilitated, and determining when injured athletes are psychologically ready to return to competition.

Background: While a large number of studies have investigated the anatomic, hormonal, and biomechanical risk factors related to musculoskeletal (MSK) injury risk, there is growing evidence to suggest that cognition is an important injury contributor in the athletic population. A systematic review of the available evidence regarding the influence of cognitive performance on MSK injury risk has yet to be published in the sports medicine literature. Purpose/Hypothesis: The purpose was to determine the effects of cognition on (1) MSK biomechanics during sports-specific tasks and (2) MSK injury occurrence in the athletic population. It was hypothesized that athletes with lower cognitive performance would demonstrate biomechanical patterns suggestive of MSK injury risk and that injured athletes would perform worse on baseline measures of cognition as compared with their noninjured counterparts. Study Design: Systematic review. Methods: PubMed and SPORTDiscus were searched from January 2000 to January 2020. Manual searches were performed on the reference lists of the included studies. A search of the literature was performed for studies published in English that reported MSK biomechanics as a function of cognitive performance and MSK injury occurrence after baseline measures of cognition. Two independent reviewers extracted pertinent study data in accordance with the PRISMA (Preferred Reporting Items for Systematic Reviews and Meta-Analyses) 2009 guidelines and assessed study quality using the Quality Assessment Tool for Observational Cohort and Cross-sectional Studies from the National Institutes of Health. A meta-analysis was not performed, owing to the heterogeneous nature of the study designs. Results: Ten studies met inclusion criteria: 4 cognition–MSK biomechanics studies and 6 cognition–MSK injury studies. All 4 cognition–MSK biomechanics studies demonstrated that worse performance on measures of cognition was associated with lower extremity MSK biomechanical patterns suggestive of greater risk for MSK injury. The majority of the cognition–MSK injury studies demonstrated that injured athletes significantly differed on baseline cognition measures versus matched controls or that cognitive performance was a significant predictor for subsequent MSK injury. Conclusion: Although the literature exploring cognitive contributions to MSK injury risk is still in its infancy, it is suggested that sports medicine personnel conduct baseline assessments of cognition—in particular, reaction time and working memory—to identify which athletes may be at elevated risk for future MSK injury.
