Objective: The present study aims to analyze implementation consequences on active search for incidents related to blood transfusion at Onofre Lopes University Hospital (HUOL) and establish a blood transfusion profile in this facility. Methods: Blood transfusion and TIs registered on Hemotherapy Core at HUOL were counted through SPSS 20, comparing IT/1000 blood transfusion averages between 2012 and 2014. T Test of Student was used to compare data and chi-square (X²) and relative risk calculation to associate the use of blood components and risk to develop TI. Results: An increase of TI numbers at HUOL was shown by active searching and its equivalence to reference French and Brazilian services, liked to ANVISA sentinel network. Thus, there was a change in the average rate from 1.86 TI/1000 blood transfusions in 2012 to 5.36 TI/1000 blood transfusions in 2013 and 5.86 TI/1000 in 2014 (p = 0.001). It was also observed that the red blood cell concentrate is the fraction with the highest risk of occurrence of TIs (p = 0.003) and the greatest chance of causing any type of TI in relation to the other blood products, RR = 1.848 (95% CI; 1.042 - 3.266). It was also seen that the infusion of platelet concentrate is related to the allergic reaction (p <0.01), and greater risk compared to other blood components, RR = 2.746 (95% CI; 1.477 - 5.107). Conclusion: This study demonstrates active Hemovigilance importance on Tis subnotifications decrease.

Background: Inventory of blood/blood components suffer due to outdating of Rh Negative units. Rationale of this article is strategy of transfusion of O Rh Negative red cells to all. A Rh Negative red cells can be given to A Rh Positive, AB Rh Positive. B Rh Negative red cells can be given to B Rh Positive, AB Rh Positive. AB Rh Negative red cells to AB Rh Positive recipients. AB Rh Negative FFP/PRP can be given to all. Objective was to avoid out dating of Rh Negatives by studying the percentage of outdated Rh Negative units amongst the all outdated.Methods: This was 9 years observational, retrospective, cross sectional and descriptive study conducted at tertiary care hospital. Outdated units of Rh Negative blood and components were analysed from the year wise discard registers of blood bank. Percentage of Rh Negative units within all outdated units were calculated.Results: 198 Rh Negative units of whole blood and blood components within all outdated units was 29.11%. Out of 198 the 20 Rh Negative blood components were discarded.Conclusions: Adopt type and screen protocol to prevent outdating. Avoid to bleed the rare blood groups. Audit by hospital transfusion committee and implementation of MSBOS. Track O Rh Negative red cells transfusion to Rh Positive as quality indicator. Track/review transfusion of O Rh negative red cells to Non O Rh negative recipients. Track AB Rh Negative FFP/PRP transfusions to all.

SummaryTwo important factors concerning the management of hemophilia in France are considered.The supply of factors VIII and IX for replacement therapy meets the current demand but as the demand increases with the development of self-infusion programs, the production will also have to increase. This can only be done through more effective use of all of the blood components and will require careful evaluation of the needs of each patient.Programs which teach self-infusion and the other aspects of home care are gradually allowing the hemophiliac and his family better understanding of the disease. These programs are developing slowly in France but must be expanded to improve the general care of all French hemophiliacs.

Recently non-fermenting Gram negative rods (NFGNR) are playing an important role in healthcare associated infections. This observational study in a tertiary care hospital of Dhaka city conducted during 01August 2007 to 30 June 2013 found that 34.8% isolated organisms from patients with healthcare associated infections were NFGNR. Majority (74.3 %) of these infections were occurring inside critical care areas. Pseudomonas and Acinetobacter together constituted 79.6% of the total NFGNR whereas Burkholderia cephacia complex (15.4%), Stenotrophomonas (4.3%) and Chryseobacterium species (0.7%) combined constituted remaining 20.4%. Out of total NFGNRs, Pseudomonas was responsible for highest number of catheter associated urinary tract infections (55.6%), ventilator associated pneumonia (46.3%), respiratory tract infection (65.8%) and surgical site infection (70.6%). Blood stream infection was predominantly caused by Burkholderia cephacia complex (33.5%) and Acinetobacter spp. (39.5%).
