AbstractA 37-year-old male was referred to us about one hour after deliberate ingestion of two 3-gram aluminium phosphide (ALP) tablets. Three hours after admission, his blood pressure dropped to 85/55 mmHg, his heart rate increased to 120 bpm, O2 saturation dropped to 82 %, and the electrocardiogram showed junctional rhythm. We started whole blood exchange, and gross haematuria and jaundice ensued. However, his blood pressure increased, arrhythmia resolved itself, and he was extubated two days after the transfusion was completed. He was sent home seven days after admission completely symptom-free. We believe this treatment may be successfully applied in ALP-poisoned patients.

Background:: Hyperviscosity of blood secondary to polycythemia results in increased resistance to blood flow and decrease in delivery of oxygen. Objective:: To evaluate whether serum endocan, NSE and IMA levels can be compared in terms of endothelial injury/ dysfunction and neuronal damage in term neonates with polycythemia who underwent PET. Methods:: 38 symptomatic polycythemic newborns having PET and 38 healthy newborns were included in the study. Blood samples for endocan, NSE and IMA were taken at only postnatal 24 hours of age in the control group and in polycytemia group just before PET, at 24 and 72 hours after PET. Results:: The polycythemia group had higher serum endocan(1073,4 ± 644,8 vs. 378,8 ± 95,9ng/ml; p<0.05), IMA(1,32 ± 0,34 vs.0,601 ± 0,095absorbance unit; p<0.05) and NSE(44,7 ± 4,3 vs. 26,91 ± 7,12μg/l; p<0.05) levels than control group before the PET procedure. At 24 hours after PET, IMA(0,656 ± 0,07 vs. 0,601 ± 0,095absorbance unit; p<0.05) and endocan(510,9 ± 228,6 vs. 378,8 ± 95,9ng/ml; p<0.05) levels were closer to the control group, being still statistically significant higher. NSE levels decreased to control group levels having no difference between the PET and control groups at 24 hours after PET (28,98 ± 6,5 vs. 26,91 ± 7,12μg/l; p>0.05). At 72 hours after PET the polycythemia and control groups did not differ statistically for IMA, endocan and NSE levels (p>0.05). Conclusion:: Serum endocan and IMA levels can be used as a biomarker for endothelial damage / dysfunction and tissue hypoxia in infants with symptomatic polycytemia.

Volumes of plasma (PV), blood (BV), and red cells (RCV) were estimated within 32 hours of birth in 39 neonates with normal growth, 14 neonates with intrauterine growth retardation, and 20 neonates with macrosomia. Total PV, BV, and RCV increased linearly with birth weight and were unaffected by deviation in the quality of fetal growth. In proportion to body weight, PV/kg, BV/kg, and RCV/kg correlated neither with birth weight nor with the quality of intrauterine growth. Neonates with umbilical vein hematocrit (UV Hct) levels 51% to 60%, 61% to 65%, and 66% to 77% had progressively lower, but not statistically different, mean PV/kg (38.1 ± 4.49, 37.6 ± 5.41, and 34.8 ± 5.16 ml/kg, respectively). On the other hand, they had progressively higher mean BV/kg (90 ± 10.1 vs 101 ± 13.7 ml/kg, P &lt; .002, and vs 110 ± 19.0 ml/kg, P &lt; .001). They also had progressively higher mean RCV/kg (52 ± 7.4, 64 ± 8.7, and 75 ± 16.4 ml/kg, P &lt; .001). Although PV/kg did not correlate with UV Hct, both BV/kg and RCV/kg increased linearly with increasing UV Hct (r = .58 and r = .79, respectively). Volume estimates were repeated after partial exchange transfusion in 29 neonates. Mean UV Hct decreased from 63 ± 5.9% preexchange to 51 ± 5.2% postexchange (P &lt; .001), mean PV increased from 37.7 ± 5.56 to 47.6 ± 7.99 ml/kg (P &lt; .001) and mean RCV decreased from 67 ± 16.5 to 51 ± 12.3 ml/kg (P &lt; .001). Despite precautions to keep the partial exchange isovolemic, mean BV decreased from 105 ± 18.7 to 98 ± 18.0 ml/kg (P = .001) and the mean PV increase (10 ml/kg) was less than the mean RCV decrease (16 ml/kg). These data suggest that neonates with polycythemia have normal PV but their RCV and BV are elevated in direct proportion to UV Hct. "Isovolemic" partial exchange transfusion decreases UV Hct, RCV, and BV and increases PV.

Recently, several new cell separators have become available for high platelet low WBC contamination thrombocytapheresis. In their original configuration, these apparatuses can only collect plasma along with platelets, while no concurrent red cell collection can be carried out. In this study, we present the preliminary results of plasma and/or erythrothrombocytapheresis after the adaptation of the lines of two different cell separators, namely the Fresenius AS 204 and the Baxter Health Care Amicus. In addition, the preliminary results of multicomponent collection using the Dideco Excel will be presented; in the latter a system of plasma recirculation was introduced, to increase the purity and quality of the platelet product. The results of these studies show that erythrothrombocytapheresis can be carried out with all these apparatuses, but only with Dideco-Excel and the Fresenius AS 204 the procedure is totally automated. There are marginal differences in the quality of the RBC product in terms of percent hematocrit, from 65% with the Excel and AS 204 to ∼ 75% with the Amicus and in terms total content of hemoglobin which averages 58 g per RBC concentrate when RBC collection is carried out during the processing of ∼ 420 ml of whole blood. In terms of platelet collection, the differences are presently moderate, as the platelet yield and WBC contamination are not affected by the concurrent RBC collection. The average platelet yield was 3.98-4.1, and 4.01x1011 and the total WBC contamination was 1.2-2.9, and 1.9X105 rerspectively with the Amicus, the AS 204 and the Excel. These represent a new step in the direction of total apheresis for blood and blood component procurement.
