Severity of hemorrhage and rate of bleeding are fundamental factors in the outcomes of trauma. Intravenous administration of fluid is the basic treatment to maintain blood pressure until bleeding is controlled. The main guideline, used almost worldwide, Advanced Trauma Life Support, established by the American College of Surgeons in 1976, calls for aggressive administration of intravenous fluids, primarily crystalloid solutions. Several other guidelines, such as Prehospital Trauma Life Support, Trauma Evaluation and Management, and Advanced Trauma Operative Management, are applied according to a patient’s current condition. However, the ideal strategy remains unclear. With permissive hypotension, also known as hypotensive resuscitation, fluid administration is less aggressive. The available models of permissive hypotension are based on hypotheses in hypovolemic physiology and restricted clinical trials in animals. Before these models can be used in patients, randomized, controlled clinical trials are necessary.

Este artigo teve como objetivo identificar os cuidados de enfermagem necessários para construir um protocolo de atendimento a pacientes adultos traumatizados, aerorremovidos, específico aos períodos durante e após o voo, baseado nos princípios do Prehospital Trauma Life Support. Trata-se de uma pesquisa qualitativa convergente-assistencial, realizada na Divisão de Operações Aéreas em São José, Santa Catarina, no período de abril a junho de 2010. Para a construção do protocolo foram utilizadas entrevista individual e encontros em grupo. Participaram oito enfermeiros que atuam no serviço. O protocolo contempla cuidados relativos à segurança em voo, avaliação constante do paciente durante a remoção e na entrega ao destino final. O protocolo propiciará uma prática de cuidado mais segura aos pacientes aerorremovidos. Sugere-se outros estudos voltados para a atuação do enfermeiro, como a criação de novos protocolos que orientem a assistência no ambiente aeroespacial.

Aim: identify and collectively select empirical indicators of Basic Human Needs (BHN) in the context of Mobile Prehospital Care (MPC). Method: This is a descriptive research of the methodological type, using a quantitative approach, carried out at the Mobile Emergency Care Service (SAMU), in Curitiba-PR-Brazil, between 2014 and 2015. It was developed in two stages: the first, through consultation in the scientific literature related to MPC and through consultation in the international protocols Advanced Cardiac Life Support (ACLS) and Prehospital Trauma Life Support (PHTLS); the second, through a questionnaire applied to the 34 nurses participating in the research. Results: 138 empirical indicators were obtained. Of these, 96 were considered relevant and, among these, the regrouping and reorganization were carried out, totaling 69 indicators. Conclusion: The identification of the empirical indicators allowed the selection of the most relevant signs and symptoms in the context of MPC.

Teve-se como objetivo apresentar um protocolo de cuidados de enfermagem na pré-remoção aeroespacial de pacientes adultos vítimas de trauma. Trata-se de uma pesquisa qualitativa, convergente assistencial, realizada na Divisão de Operações Aéreas, em São José-SC, entre abril a junho de 2010. Baseou-se nos princípios do Prehospital Trauma Life Support. Utilizou-se de entrevista individual e de encontros em grupo. Participaram os oito enfermeiros do serviço. O protocolo contempla os cuidados relativos à segurança no embarque; desembarque; avaliação da cena; avaliação primária; avaliação secundária e estabilização do paciente. O protocolo propiciará uma prática de cuidado mais segura aos pacientes aerorremovidos.
