Orang dengan diabetes mellitus memiliki peningkatan mengembangkan sejumlah masalah kesehatan yang mengancam jiwa. Kadar glukosa darah yang tinggi dapat menyebabkan kerusakan pembuluh darah umum yang dapat mempengaruhi jantung, mata, ginjal, saraf, dan dapat mengakibatkan berbagai komplikasi. Sedangkan pola makan merupakan asupan makanan yang memberikan berbagai macam jumlah, jadwal dan jenis makanan yang didapatkan seseorang. Pengaturan pola makan yang tidak tepat dapat meningkatkan kadar glukosa dalam darah sehingga seseorang rentang terkena penyakit diabetes melitus. Penelitian ini bertujuan untuk mengetahui hubungan antara pola makan terhadap kejadian diabetes mellitus. Jenis penelitian yang digunakan adalah non experiment dengan metode survey analitik. Dengan menggunakan pendekatan cross sectional. Populasi dalam penelitian ini yaitu 47 responden. Sampel diambil menggunakan tehnik total sampling. Berdasarkan hasil penelitian menunjukkan bahwa dari total 47 responden terdapat 21 responden yang memiliki pola makan baik (44.7 %). Dimana Pola makan baik yang tidak menderita sebanyak 13 responden (27.7%), sedangkan pola makan baik yang  menderita sebanyak 8 responden (17.0%). Kemudian 26 responden yang memiliki pola makan kurang baik (55.3%). Dimana pola makan kurang baik yang tidak menderita sebanyak 8 responden (17.0%), sedangkan pola makan kurang baik yang  menderita sebanyak 18  responden (38.3%). Setelah dilakukan uji statistic dengan menggunakan uji chi-square test maka berdasarkan hasil fisher’s exact test didapatkan nilai p = 0,033 yang menunjukkan p<0,05, maka dengan demikian dapat disimpulkan bahwa Ha diterima dan H0 ditolak atau ada hubungan yang signifikan antara pola makan dengan kejadian diabetes mellitus di wilayah kerja puskesmas kassi-kassi kota Makassar.

Early Initiation of Breastfeeding (IMD) is an opportunity given to babies immediately after birth by placing the baby on the mother's stomach, then the baby is allowed to find the mother's nipple and breastfeed within the first hour after birth. The UNICEF organization Says that IMD is so important for babies that it calls it the baby's first vaccine. The purpose of this study was to determine the factors related to the implementation of Early Breastfeeding Initiation (IMD) at Butar Puskesmas, Pagaran District, Tapanulu Utara Regency in 2020. This type of research is a descriptive quantitative study using a cross sectional design or approach looking at the relationship between the independent and dependent variables. The total population of 30 people was used with a total sampling technique in which the entire population was sampled so that the sample size was 30 people. Data were collected by means of a questionnaire, then the data were processed using the chi-square test. The results of the study using the chi-square test with a confidence level of 95% (α = 0.05) showed that there was no relationship between the type of labor and the results of the Fisher's Exact Test with a confidence level of 95% and df = 1 obtained x2 count (0.597) <x2. table (3,841), there is a relationship of knowledge df = 2 obtained x2 count (10.425)> x2 table 5,991, there is a relationship between husband's support for Fisher's Exact Test with a confidence level of 95% (α = 0.05) and df = 1 obtained p value = 0.000 ( p <0.05), there is a relationship between support for health workers and df = 2, it is obtained x2 count (8,450)> n x2 table (5,991). It is expected that respondents will know the information provided by health workers or from the mass media about the provision and implementation of Early Breastfeeding Initiation (IMD).

Σκοπός της παρούσας διδακτορικής διατριβής ήταν να διερευνήσει την κατανομή των HLA-DRB1, DQA1 και DQB1 αλληλομόρφων γονιδίων, των απλοτύπων και γονοτύπων στον ελληνικό παιδιατρικό πληθυσμό με κοιλιοκάκη, σε σύγκριση με τους υγιείς μάρτυρες, και να αξιολογήσει την επίδραση των HLA τάξης ΙΙ γονιδίων στην επιδεκτικότητα της νόσου. Παράλληλα, αναζητήθηκαν συσχετίσεις του HLA-γενετικού υποστρώματος των ασθενών με τα κλινικά, επιδημιολογικά, ορολογικά και ιστολογικά χαρακτηριστικά της νόσου.Μελετήθηκαν 118 παιδιά με κοιλιοκάκη ελληνικής καταγωγής. Oι συμπτωματικοί ασθενείς ταξινομήθηκαν σε 3 ομάδες ως εξής: ομάδα Α = DQB1*02 ομοζυγώτες, ομάδα Β = DQB1*02 ετεροζυγώτες και ομάδα Γ = DQB1*02 αρνητικοί ασθενείς. Ως ομάδα ελέγχου, για τη σύγκριση των αποτελεσμάτων της HLA τυποποίησης των ασθενών, χρησίμευσε η HLA τυποποίηση 120 υγιών ατόμων, δοτών αίματος και μυελού των οστών, μη συγγενών μεταξύ τους, ελληνικής καταγωγής και εθνικότητας. Η γονιδιακή HLA τυποποίηση πραγματοποιήθηκε με τις μεθόδους PCR-SSP και PCR-SSO. Η στατιστική ανάλυση περιελάμβανε τα Pearson’s Chi-Square test, Fisher’s exact test, Student’s t-test, Mann-Whitney και Kruskal-Wallis test. Ο βαθμός τη σχέσης ελέγχθηκε με ακριβή λογιστική παλινδρόμηση (exact logistic regression) και παρουσιάστηκε ως λόγος σχετικών πιθανοτήτων (odds ratios, OR). Tα αποτελέσματα έδειξαν στατιστικά αυξημένη συχνότητα των HLA-DQB1*02:01, DQB1*02:02, DQA1*02:01, DQA1*05:01, DRB1*03 και DRB1*07 και στατιστικά μειωμένη συχνότητα των HLA-DQB1*03:01, DQB1*05:01, DQB1*05:02, DQA1*01:01, DQA1*01:02, DQA1*01:04, DQA1*05:05, DRB1*01 και DRB1*16 στους ασθενείς σε σύγκριση με τους υγιείς μάρτυρες. Σε επίπεδο απλοτύπων, ο κύριος συσχετιζόμενος με την κοιλιοκάκη απλότυπος ήταν ο DR3-DQ2 και ακολούθησε ο DR7-DQ2. Το 95,8% των ασθενών με κοιλιοκάκη εκφράζει το DQ2 ή/και το DQ8 μόριο. Μόνο ένας στους 118 ασθενείς, που αντιστοιχεί σε πιθανότητα 0,8%, δεν εκφράζει κανένα από τα συσχετιζόμενα με την κοιλιοκάκη αλλήλια και είναι DQ2/DQ8/DQA1*05/DQB1*02 - αρνητικός. Τα αποτελέσματα της μελέτης έδειξαν μια στατιστικά σημαντική διαφορά στον τίτλο των ΕΜΑ αντισωμάτων μεταξύ των ομάδων Α και Γ. Αναλυτικά, οι τίτλοι των ΕΜΑ αντισωμάτων είναι σημαντικά υψηλότεροι στους HLA-DQB1*02 ομοζυγώτες σε σχέση με τους HLA-DQB1*02 αρνητικούς ασθενείς, γεγονός που πιθανόν να αντανακλά μια HLA-DQB1*02 δοσοεξαρτώμενη επίδραση στον τίτλο των ΕΜΑ αντισωμάτων.Συμπερασματικά, η παρούσα διατριβή περιγράφει για πρώτη φορά στον ελληνικό παιδιατρικό πληθυσμό τη συχνότητα εμφάνισης των HLA τάξης ΙΙ αλληλομόρφων γονιδίων, απλοτύπων και γονοτύπων, και επιβεβαιώνει τη συμβολή των HLA τάξης ΙΙ γονιδίων στη γενετική προδιάθεση της νόσου. Επιπλέον, παρέχει στοιχεία που υποδηλώνουν μια πιθανή HLΑ-DQB1*02 δοσοεξαρτώμενη επίδραση στον τίτλο των ΕΜΑ αντισωμάτων.

Faktor-Faktor yang Berhubungan dengan Kepatuhan Menjalani Rehabilitasi Medikpada Pasien Stroke (Studi di RSI Sunan Kudus )

ABSTRAKStroke merupakan penyebab kematian terbesar di dunia. Selain sebagai penyebab kematian juga menyebabkan kelumpuhan. Rehabilitasi medik menjadi sangat penting bagi penderita stroke agar dapat kembali normal atau meminimalkan cacat yang mungkin terjadi. Tujuan dari penelitian adalah untuk mengetahui faktor-faktor yang berhubungan dengan kepatuhan menjalani rehabilitasi medik pada pasien stroke di RSI Sunan Kudus. Jenis penelitian ini adalah survei analitik dengan pendekatan cross sectional. Jumlah sampelnya adalah 40 diambil dengan teknik simple random sampling. Data dianalisis dengan uji validitas dan reliabilitas dan analisis hipotesis penelitian menggunakan chi square test dan Fisher’s Exact test. Hasil penelitian menunjukkan bahwa faktor yang berhubungan dengan kepatuhan menjalani rehabilitasi medik pada pasien stroke adalah motivasi pasien  (p=0,017) dan dukungan keluarga  (p=0,001). Faktor yang tidak berhubungan dengan kepatuhan menjalani rehabilitasi medik pada pasien stroke adalah pengetahuan keluarga  (p=0,442), penghasilan (p=0,664), keterjangkauan akses  (p=0,726 ), status serangan  (p=1,000 ), serta pelayanan petugas kesehatan  (p=0,712). Saran yang diberikan khususnya bagi tenaga kesehatan bagian rehabilitasi medik agar lebih mendukung dalam memberikan informasi mengenai hal-hal yang terkait dengan stroke dan pentingnya rehabilitasi medik bagi pasien stroke.

Latar Belakang. Iodium merupakan mikronutrien penting terutama bagi perkembangan otak janin dan anak. Iodium berperan untuk proses pertumbuhan dan perkembangan sebagian besar organ terutama otak. Konsumsi iodium yang rendah dalam jangka panjang merupakan salah satu faktor yang dapat menyebabkan gangguan akibat kekurangan iodium (GAKI). Tingkat pengetahuan mengenai GAKI dan garam beriodium berpengaruh terhadap ketersediaan dan praktik penggunaan garam beriodium. Pemerintah telah mengupayakan penanggulangan GAKI melalui fortifikasi garam dengan iodium. Tujuan. Penelitian ini bertujuan untuk menganalisis pengetahuan ibu tentang iodium dengan ketersediaan garam beriodium di rumah tangga dan faktor yang memengaruhinya. Metode. Penelitian ini merupakan penelitian cross-sectional. Data diambil dari 198 rumah tangga menggunakan teknik proportional stratified random sampling. Penilaian pengetahuan ibu dilakukan dengan menggunakan kuesioner dalam bentuk pertanyaan tertutup. Sementara itu, penilaian ketersediaan garam diperoleh dengan pengujian kandungan iodium (KIO3). Uji statistik yang digunakan adalah chi-square test/fisher’s exact test dan Mann Whitney U/Kruskal Wallis untuk mengetahui hubungan antar variabel. Hasil. Sebagian besar responden tinggal di daerah dataran tinggi (74,2%), berpendidikan SD (47,5%) dan bekerja sebagai petani (41,4%). Karakteristik lokasi geografi tempat tinggal responden berhubungan dengan pengetahuan responden mengenai GAKI serta dampak dan faktor risiko GAKI (p=0,023 dan p<0,001), sedangkan pekerjaan responden berhubungan dengan pengetahuan mengenai dampak dan faktor risiko GAKI (p=0,020). Tidak ada hubungan antara pengetahuan dengan pemenuhan syarat mutu kandungan KIO3 pada garam yang digunakan di rumah tangga. Namun ada kecenderungan responden yang mempunyai garam dengan KIO3 sesuai, lebih banyak pada responden dengan pengetahuan yang baik. Kesimpulan. Responden dengan pengetahuan baik lebih banyak yang memiliki garam dengan kadar iodium sesuai standar. Perlu adanya program edukasi mengenai GAKI, penggunaan dan penyimpanan garam beriodium, serta faktor penyebab penurunan kualitas garam di rumah tangga.

Irinotecan and Increased Triglyceride Level in Paraneoplastic Deep Venous Thrombosis (DVT): A Strange Link in An Old Problem

Abstract DVT is present in about 3–15% of cancer patients. Paraneoplastic thrombosis pathogenesis is multifactorial. Chemotherapy is frequently involved in DVT genesis as pro-inflammatory/pro-thrombotic factor. There are few data about incidence of DVT in patients treated with Irinotecan. Aim of our study is to define if thrombotic risk in patients treated with Irinotecan is increased and then which mechanism is probably involved. For this purpose we considered in our patients the complement fractions C3 and C4 and the immune circulating complex (ICC), because they activate macrophages and platelets and increase tissue factor level. Moreover we evaluated also total cholesterol and triglyceride level, cause linked with factor VII activation. We considered C3, C4, ICC, cholesterol and triglyceride level, dose density and chemotherapeutic agents used in 116 patients with solid neoplasms (62colon, 26lung, 18gastric,) and not receiving anticoagulant prophylaxis. Median age was 68.5 years (R 57–83). M/F ratio was 66/40. The threshold value of third quartile was chosen as risk cut-off (C3: 130mg/dl; C4: 32mg/dl; ICC: 2.9 mcg/ml; total cholesterol: 205mg/dl; triglycerides 120mg/dl). The statistical analysis was conducted with Yates corrected chi square test, Fisher’s exact test, Odds Ratio (OR), relative risk (RR). Twenty-four patients (20%) developed DVT. The patients treated with Irinotecan were 42. DVT occurred in 14 of these patients (32%) and in 10 of the 74 patients treated without Irinotecan (14%), with a Yates corrected chi square test of 4.6 (p=0.03), a Fisher’s exact test with p=0.03, an OR of 2.98 (95%CI 1.2–7.3) and a RR of 2.35 (95%CI 1.1– 4.8). C-Reactive Protein and Erythocyte Sedimentation Rate at Pearson’s test were not related with C3, C4, ICC, total cholesterol and triglyceride levels. All patients treated with Irinotecan had advanced stage colon cancer. High levels of triglycerides (&gt;120mg/dl) were present in 17 out of 42 patients treated with Irinotecan (40%) and in 17 out of 74 patients treated without Irinotecan (23%) with a Yates corrected chi square test of 3.1 (p=0.07), a Fisher’s exact test with p=0.05, an OR of 2.2 (95%CI 1–5) and a RR of 1.7 (95%CI 1–3). Although most patients developing DVT have an advanced stage colon cancer and so receiving Irinotecan, the influence of chemotherapy in DVT pathogenesis in our study cannot be excluded. Probably Irinotecan could contribute to DVT increasing triglyceride levels that are linked with in vivo factor VII activation. This relief suggests also that risk factors, other than classical, should be considered in the thrombosis onset; then other drugs (i.e. hypolipemizing drugs) could be useful in paraneoplastic DVT prevention. Nevertheless these data should be confirmed in a larger cohort of patients.
