Three‐dimensional mixed longitudinal study of facial growth changes and variability of facial form in preschool children using stereophotogrammetry

The purpose of the present study was to describe three-dimensional developmental changes of facial asymmetry in children with an operated complete unilateral cleft lip and palate (UCLP) and in children without craniofacial anomalies (controls). Using stereophotogrammetry, three-dimensional coordinates for 16 bilateral and 10 midsagittal facial landmarks were determined for the UCLP group (n = 33) and the control group (n = 63) on two occasions. In this mixed-longitudinal study, the children were 4 to 12 years of age. Facial asymmetry and left-right dominance was measured and resolved for transverse, vertical, and sagittal components. Significant effects were analyzed with multivariate analyses of variance (MANOVA). We concluded that individuals with complete unilateral cleft lip and palate, as well as individuals without craniofacial anomalies, show an increase, during growth, in the amount of facial asymmetry in the basal region of the nose. In the region that is related to the cleft, children with complete unilateral cleft lip and palate do not show changes in the amount of facial asymmetry between the occasions. Regarding facial left-right dominance and variation in dominance, no demonstrable growth changes take place in individuals with complete cleft lip and palate, nor in individuals without craniofacial anomalies.

Helen Zarsky Reinherz (1923–2017), Professor at Simmons College, was a social work pioneer known for her independence, work ethic, and intellect. She was considered a pioneer in the field of social work during her 43-year career as a beloved professor and faculty member at Simmons College. But it is her work as the principal investigator on the groundbreaking Simmons Longitudinal Study focusing on preschool children and adolescents that was considered her most impactful contribution to the field of social work.

Objective: To investigate whether timing of hard palate repair had a significant effect on facial growth in patients with unilateral cleft lip and palate (UCLP). Design: Retrospective longitudinal study. Setting: Sri Lankan Cleft Lip and Palate Project. Patients: A total of 104 patients with nonsyndromic UCLP who had hard palate repair by age 13 years, with their 290 cephalometric radiographs taken after lip and palate repair. Main Outcome Measures: Clinical notes were used to record surgical treatment histories. Cephalometry was used to determine facial morphology and growth rate. Results: Timing of hard palate repair had a significant effect on the length and protrusion of the alveolar maxilla (PMP-A and SNA, respectively) and the anteroposterior alveolar jaw relation (ANB) at age 20 years but not on their growth rates. Conclusion: Timing of hard palate repair significantly affects the growth of the maxilla in patients with UCLP. Late hard palate repair has a smaller adverse effect than does early hard palate repair on the growth of the maxilla. This timing effect primarily affects the anteroposterior development of the maxillary dentoalveolus and is attributed to the development being undisturbed before closure of the hard palate.

Abstract Background: With the aid of implants, Björk identified the two-dimensional mandibular stable structures in cephalogram during facial growth. However, we don't know the three-dimensional stable structures exactly. The purpose of this study was to identify the most stable mandibular landmarks in growing patients using three-dimensional images.Methods: The sample was comprised of two cone-beam computed tomography (CBCT) scans taken about 4.6 years apart in 20 growing patients between the ages of 12.5 (T1) to 17.1 years (T2). After head orientation, landmarks were located on the chin (Pog), internal symphysis (Points C, D and E), and mandibular canals, which included the mental foramina (MF and MFA) and mandibular foramina (MdF). The linear distance change between Point C and these landmarks was measured on each CBCT to test stability through time. The reliability of the suggested stable landmarks was also evaluated. Results: The total distance changes between Point C and points D, E, Pog, MF, and MFA were all less than 1.0 mm from T1 to T2. The reliability measures of these landmarks, which were measured by the Cronbach alpha, were above 0.94 in all three dimensions for each landmark. From T1 to T2, distance changes from Point C to the right and left mandibular foramina were respectively 3.39±3.29 mm and 3.03±2.83 mm. Conclusions: During a growth period that averaged 4.6-years, ranging from 11.2 to 19.8 years, the structures that appeared relatively stable and could be used in mandibular regional superimposition included Pog, landmarks on the inferior part of the internal symphysis, and the mental foramen. The centers of the mandibular foramina, the starting points of the mandibular canal, underwent significant changes in the transverse and sagittal dimensions.
