Anomia is a frequent and persistent symptom of poststroke aphasia, resulting from damage to areas of the brain involved in language production. Cortical neuroplasticity plays a significant role in language recovery following stroke and can be facilitated by behavioral speech and language therapy. Recent research suggests that complementing therapy with neurostimulation techniques may enhance functional gains, even amongst those with chronic aphasia. The current review focuses on the use of transcranial Direct Current Stimulation (tDCS) as an adjunct to naming therapy for individuals with chronic poststroke aphasia. Our survey of the literature indicates that combining therapy with anodal (excitatory) stimulation to the left hemisphere and/or cathodal (inhibitory) stimulation to the right hemisphere can increase both naming accuracy and speed when compared to the effects of therapy alone. However, the benefits of tDCS as a complement to therapy have not been yet systematically investigated with respect to site and polarity of stimulation. Recommendations for future research to help determine optimal protocols for combined therapy and tDCS are outlined.

AbstractIn this manuscript we consider the problem of relating functional connectivity measurements viewed as statistical distributions to outcomes. We demonstrate the utility of using the distribution of connectivity on a study of resting state functional magnetic resonance imaging association with an intervention. Specifically, we consider 47 primary progressive aphasia (PPA) patients with various levels of language abilities. These patients were randomly assigned to two treatment arms, tDCS (transcranial direct-current stimulation and language therapy) vs sham (language therapy only), in a clinical trial. We propose a novel approach to analyze the effect of direct stimulation on functional connectivity. We estimate the density of correlations among the regions of interest (ROIs) and study the difference in the density post-intervention between treatment arms. We discover that it is the tail of the density, rather than the mean or lower order moments of the distribution, that demonstrates a significant impact in the classification. This approach has several benefits. Among them, it drastically reduces the number of multiple comparisons compared to edge-wise analysis. In addition, it allows for the investigation of the impact of functional connectivity on the outcomes where the connectivity is not geometrically localized.

AbstractObjective:Aphasia recovery depends on neural reorganization, which can be enhanced by speech-language therapy and noninvasive brain stimulation. Several studies suggested that transcranial direct current stimulation (tDCS) associated with speech-language therapy may improve verbal performance evaluated by analytic tests, but none focused on spontaneous speech. We explored the effect of bihemispheric tDCS on spontaneous speech in patients with poststroke aphasia.Methods:In this multicentric controlled randomized cross-over double-blind study, we included 10 patients with poststroke aphasia (4 had aphasia >6 months and 6 with aphasia <6 months). We combined the sessions of speech-language therapy and bihemispheric tDCS (2 mA, 20 min). After three baseline speech evaluations (1/week), two different conditions were randomly consecutively proposed: active and sham tDCS over 3 weeks with 1 week of washout in between. The main outcome measure was the number of different nouns used in 2 min to answer the question “what is your job.”Results:There was no significant difference between conditions concerning the main outcome measure (p = .47) nor in the number of verbs, adjectives, adverbs, pronouns, repetitions, blank ideas, ideas, utterances with grammatical errors or paraphasias used. Other cognitive functions (verbal working memory, neglect, or verbal fluency) were not significantly improved in the tDCS group. No adverse events occurred.Conclusion:Our results differed from previous studies using tDCS to improve naming in patients with poststroke aphasia possibly due to bihemispheric stimulation, rarely used previously. The duration of the rehabilitation period was short given the linguistic complexity of the measure. This negative result should be confirmed by larger studies with ecological measures.

Zusammenfassung Gegenstand und Ziel: Die transkranielle Gleichstromstimulation (transcranial direct current stimulation, tDCS) wird als neues nicht invasives Hirnstimulationsverfahren für die Behandlung neurologischer und psychiatrischer Erkrankungen untersucht. Mittlerweile gibt es Fallberichte und eine randomisierte, kontrollierte Pilotstudie zur Behandlung akustischer Halluzinationen sowie der Negativsymptomatik bei Schizophrenie. Fallbericht: Im vorliegenden Fallbericht wurde ein Patient mit therapieresistenter Symptomatik bei paranoider Schizophrenie drei Wochen lang mit anodaler tDCS über dem linken dorsolateralen und kathodaler tDCS über dem rechten dorsolateralen Kortex behandelt. Ergebnisse: Es zeigte sich eine Verbesserung von Affektivität und Kognition, die Positivsymptome blieben jedoch nahezu unverändert. Klinische Relevanz: Die bifrontale tDCS stellt eine nebenwirkungsarme neue Behandlungsform dar und sollte in der Anwendung bei Erkrankungen aus dem schizophrenen Formenkreis weiter untersucht werden.
