The objective of the study is to show significance of desynchronosis laboratory markers in risk assessment of metabolic syndrome (MS) development. Materials and Methods. There were examined 98 men, aged 43-88, diagnosed with dyscirculatory encephalopathy showing one and more risk factors for development of cardiovascular diseases. They were divided into 2 groups according to the international guidelines of 2009: with MS (n = 61) and without MS (n = 37). Parameters of fats, glucose metabolism, inflammatory mediators, fat tissue metabolism markers, melatonin metabolite excretion (6-sulfatoxymelatonin) were defined in blood serum and urine. Results. The article presents data on changes in leptin, adiponectin, PAI-1, testosterone production and 6-sulfatoxymela-tonin excretion in patients with MS. There are calculated threshold values of these markers definitely increasing MS risk and logistic regression equation which allows assessing MS risk for an individual patient. Conclusion. Detected disorders of melatonin synthesis diurnal dynamics in patients with MS and interconnection between melatonin production and adiponectin, leptin, PAI-1, testosterone synthesis allow considering these parameters as desynchronosis markers significant for MS development.

Severe Acute Respiratory Syndrome Coronavirus-2 (SARS-CoV-2) has been declared a global pandemic. Our goal was to determine whether co-infections with respiratory polyomaviruses, such as Karolinska Institutet polyomavirus (KIPyV) and Washington University polyomavirus (WUPyV) occur in SARS-CoV-2 infected patients. Oropharyngeal swabs from 150 individuals, 112 symptomatic COVID-19 patients and 38 healthcare workers not infected by SARS-CoV-2, were collected from March 2020 through May 2020 and tested for KIPyV and WUPyV DNA presence. Of the 112 SARS-CoV-2 positive patients, 27 (24.1%) were co-infected with KIPyV, 5 (4.5%) were positive for WUPyV, and 3 (2.7%) were infected simultaneously by KIPyV and WUPyV. Neither KIPyV nor WUPyV DNA was detected in samples of healthcare workers. Significant correlations were found in patients co-infected with SARS-CoV-2 and KIPyV (p < 0.05) and between SARS-CoV-2 cycle threshold values and KIPyV, WUPyV and KIPyV and WUPyV concurrently detected (p < 0.05). These results suggest that KIPyV and WUPyV may behave as opportunistic respiratory pathogens. Additional investigations are needed to understand the epidemiology and the prevalence of respiratory polyomavirus in COVID-19 patients and whether KIPyV and WUPyV could potentially drive viral interference or influence disease outcomes by upregulating SARS-CoV-2 replicative potential.

Radon is a naturally occurring radioactive gas present in the hydrosphere, lithosphere and atmosphere abundantly. Its ionizing radiation provides the largest human internal exposure by inhalation and ingestion to natural sources, constituting a serious health hazard. The contribution to total exposure is mainly due to inhalation, as ingestion by food or drinking water is typically very small. However, because of public health concerns, the contributions from all these sources are limited by regulations and remedial action should be taken in the event that the defined threshold values are overcome. In this paper, the first campaign of measurements to control the radon activity concentration in drinking water from public water supplies in the province of Salerno, south Italy, is described. The results represent a main reference for the area, as it was never investigated before. The purpose of this survey was to contribute to data compilation concerning the presence of radon-222 in groundwater in the Campania region and to determine the associated risk for different age groups. The maximum radon activity concentrations and the related total annual public effective dose turned out to be lower than the threshold values (100 Bq/l and 0.1 mSv/y, respectively) indicated by international guidelines and the national regulation, showing that the health risks for public consumption can be considered negligible.

BackgroundDespite the necessity, there is no reliable biomarker to predict disease severity and prognosis of patients with COVID-19. The currently published prediction models are not fully applicable to clinical use.ObjectivesTo identify predictive biomarkers of COVID-19 severity and to justify their threshold values for the stratification of the risk of deterioration that would require transferring to the intensive care unit (ICU).MethodsThe study cohort (560 subjects) included all consecutive patients admitted to Dubai Mediclinic Parkview Hospital from February to May 2020 with COVID-19 confirmed by the PCR. The challenge of finding the cut-off thresholds was the unbalanced dataset (eg, the disproportion in the number of 72 patients admitted to ICU vs 488 non-severe cases). Therefore, we customised supervised machine learning (ML) algorithm in terms of threshold value used to predict worsening.ResultsWith the default thresholds returned by the ML estimator, the performance of the models was low. It was improved by setting the cut-off level to the 25th percentile for lymphocyte count and the 75th percentile for other features. The study justified the following threshold values of the laboratory tests done on admission: lymphocyte count <2.59×109/L, and the upper levels for total bilirubin 11.9 μmol/L, alanine aminotransferase 43 U/L, aspartate aminotransferase 32 U/L, D-dimer 0.7 mg/L, activated partial thromboplastin time (aPTT) 39.9 s, creatine kinase 247 U/L, C reactive protein (CRP) 14.3 mg/L, lactate dehydrogenase 246 U/L, troponin 0.037 ng/mL, ferritin 498 ng/mL and fibrinogen 446 mg/dL.ConclusionThe performance of the neural network trained with top valuable tests (aPTT, CRP and fibrinogen) is admissible (area under the curve (AUC) 0.86; 95% CI 0.486 to 0.884; p<0.001) and comparable with the model trained with all the tests (AUC 0.90; 95% CI 0.812 to 0.902; p<0.001). AbstractThis study assesses the extent to which the two main Configurational Comparative Methods (CCMs), i.e. Qualitative Comparative Analysis (QCA) and Coincidence Analysis (CNA), produce different models. It further explains how this non-identity is due to the different algorithms upon which both methods are based, namely QCA’s Quine–McCluskey algorithm and the CNA algorithm. I offer an overview of the fundamental differences between QCA and CNA and demonstrate both underlying algorithms on three data sets of ascending proximity to real-world data. Subsequent simulation studies in scenarios of varying sample sizes and degrees of noise in the data show high overall ratios of non-identity between the QCA parsimonious solution and the CNA atomic solution for varying analytical choices, i.e. different consistency and coverage threshold values and ways to derive QCA’s parsimonious solution. Clarity on the contrasts between the two methods is supposed to enable scholars to make more informed decisions on their methodological approaches, enhance their understanding of what is happening behind the results generated by the software packages, and better navigate the interpretation of results. Clarity on the non-identity between the underlying algorithms and their consequences for the results is supposed to provide a basis for a methodological discussion about which method and which variants thereof are more successful in deriving which search target.
