Development of evidence-based nursing-sensitive quality indicators for emergency nursing: A Delphi study

Introduction: Determining the research priorities in Evidence-Based Nursing Care is considered as a regular method to provide substantive information for developing the nursing science and taking evidence-based practical steps but such research is currently minimal. Therefore, a Delphi survey was conducted to determine the research priorities in Evidence-Based Nursing Care in the North of Iran. Methods: A three-round Delphi study was conducted to identify research priorities in Evidence-Based Nursing Care. The study comprised of research nurses, who have attended a Research Methodology course and did have published research. A Delphi expert panel consists of a group of credible experts within the field attending in consensus meetings to determine the nursing research top priorities. Open-ended questions were analyzed using thematic content analysis. Analysis of questionnaires 2 and 3 entailed an examination of the mean scores for each priority ranked on the five-point Likert scale. Results: Round 1 had 42 themes across 174 topics, identified by analyzing the data. In Round 2, respondents ranked all themes and 30 themes were selected. Finally, in Round 3, the top five research priorities emerged as the following: Assurance in health care, nurse-patient communication, pain management, taking care of the patients under mechanical ventilation and drug care. Conclusions: Determining research priorities in the Evidence-Based Nursing Care would optimize the level of utilization of the results of the studies by nurses and subsequently could improve the delivery of evidence-based nursing services; Prioritization will also help in directing resources to the essential research needs.

Background Ineffective communication procedures create openings for errors when health care professionals fail to transfer complete, consistent information. Deficient or absent clinical handovers, or failures to transfer information, responsibility, and accountability, can have severe consequences for hospitalized patients. Clinical handovers are practiced every day, in many ways, in all institutional health care settings. Objective This study aimed to design an evidence-based, nursing handover standard for inpatients for use at shift changes or internal transfers between hospital wards. Methods We carried out a modified, multiround, web-based, Delphi data collection survey of an anonymized panel sample of 264 nurse experts working at a multisite public hospital in Switzerland. Each survey round was built on responses from the previous one. The surveys ended with a focus group discussion consisting of a randomly selected panel of participants to explain why items for the evidence-based clinical nursing handover standard were selected or not selected. Items had to achieve a consensus of ≥70% for selection and inclusion. Results The study presents the items selected by consensus for an evidence-based nursing handover standard for inpatients for use at shift changes or internal transfers. It also presents the reasons why survey items were or were not included. Conclusions This modified Delphi survey method enabled us to develop a consensus- and evidence-based nursing handover standard now being trialed at shift changes and the internal transfers of inpatients at our multisite public hospital in Switzerland.

Hintergrund: Hinter der konkreten Anwendung von Forschungsergebnissen in der pflegerischen Praxis steht ein mehrdimensionaler Prozess. Derzeit liegen in Österreich noch keine Ergebnisse vor, die den Einfluss auf und den Zusammenhang mit Forschungsanwendung in Krankenhäusern erklären. Ziel: Ziel der Studie war, Einflüsse und Zusammenhänge von persönlicher Einstellung zu Forschungsanwendung, Verfügbarkeit von Forschungsergebnissen und institutioneller Unterstützung von Pflegekräften in Spitälern in Österreich in Bezug auf die Forschungsanwendung zu untersuchen. Methode: Im Rahmen eines nicht-experimentellen quantitativen Querschnittdesigns wurde 2011 eine multizentrische Studie (n = 10) durchgeführt. Ergebnisse: Die Stichprobe umfasst 178 diplomierte Gesundheits- und Krankenpflegepersonen, die mittels Fragebogen befragt wurden. Die multiplen Regressionsanalysen zeigen, dass eine positive Einstellung zu Forschungsanwendung (β = 0,388; p < 0,001), die Verfügbarkeit von aufbereiteten Forschungsergebnissen (β = 0,470; p < 0,001) und eine adäquate institutionelle Unterstützung (β = 0,142; p < 0,050) einen signifikanten Einfluss auf die Anwendung von Forschungsergebnissen haben. Die Pfadanalyse belegt, dass der Besuch von Kursen zu Evidence-based Nursing die Einstellung gegenüber Forschungsanwendung in starkem Maße positiv beeinflusst (β = 0,464; p < 0,001). Schlussfolgerungen: Gesundheitseinrichtungen sind gesetzlich gefordert, vorhandene positive Einstellungen zu nutzen und Unterstützungsmöglichkeiten für eine wissenschaftsorientierte Pflegepraxis zu schaffen.
