Periodontal disease is the most common chronic inflammatory disease seen in humans. It is a major public health concern, and in its severe form affects approximately 10.8% or 743 million people aged 15−99 worldwide. Trends such as the rise of smoking in developing countries, the obesity and diabetes epidemic, coupled with an ageing population with greater tooth retention, are all likely to increase the burden of periodontitis still further in the UK and worldwide. Consequences of periodontitis include hypermobility of teeth, tooth migration, drifting and eventual tooth loss. Tooth loss can directly affect the quality of life of a person in terms of reduced functional capacity, self-esteem and social relationships. CPD/Clinical Relevance: This article reports the prevalence of periodontal disease in the UK and worldwide, along with the consequences of periodontitis. The importance of timely diagnosis to avoid litigation is discussed, as is the importance of effective management of periodontitis in order to improve patients' oral health-related quality of life.

<p class="abstract"><span lang="EN-US">Severe periodontal disease often leads to tooth loss, necessitating prosthetic rehabilitation to restore function and aesthetics. The concept of perio-prosthetic treatment using extensive bridges of cross-arch design was introduced approximately 30 years ago. Long term follow-up studies have shown that teeth with reduced periodontal support can be used as abutments for extensive fixed prostheses, provided periodontal disease had been treated successfully and an effective recall program had been instituted to prevent periodontal disease recurrence. Implants along with cross arch bridges in controlled periodontitis subject have never been used together for complete full mouth rehabilitation.</span></p>

The effect of preventive oral habits is largely unexplored in older individuals. The purpose of this study was to evaluate the associations between home use of flossing and prevalence of periodontal disease and caries in older adults. Five-year incident tooth loss was also evaluated. Data on 686 individuals ≥65 y-old from the Piedmont 65+ Dental Study were examined including: 1) interproximal clinical attachment level (iCAL), 2) interproximal probing depth (iPD), 3) numbers of caries, and 4) missing teeth. Flossing behavior was evaluated according to the Periodontal Profile Class (PPC) system. Five-year follow-up data ( n = 375) was evaluated for incident tooth loss. Dichotomous and categorical variables were analyzed using Pearson chi-square tests as well as covariate-adjusted Cochran-Mantel-Haenszel tests. Multiple linear regression compared clinical parameters based on flossing behavior. Elderly flossers had lower (mean, SE) %iCAL≥3 mm (38.2, 2.38 vs. 48.8, 1.56) and %iPD≥4 mm (8.70, 1.41 vs. 14.4, 0.93) compared to nonflossers ( P ≤ 0.005). Flossers showed less coronal caries compared to nonflossers ( P = 0.02). Baseline number of missing teeth (mean, SE) was 11.5 (0.35) in nonflossers compared to 8.6 (0.53) in flossers ( P < 0.0001). Regular dental visitors had lower oral disease levels compared to episodic dental users. The majority of flossers classified into PPC-Stage I (health) whereas nonflossers classified as PPC-Stages V, VI, and VII (disease). At the 5-y follow-up visit, the average tooth loss for flossers was ~1 tooth compared to ~4 teeth lost for nonflossers ( P < 0.0001). Among all teeth, molars showed the highest benefit (>40%) for flossing behavior ( P = 0.0005). In conclusion, the extent of oral disease for older individuals was significantly less in flossers than in nonflossers. Flossers showed less periodontal disease, fewer dental caries, and loss of fewer teeth over a 5-y period. These findings further support flossing as an important oral hygiene behavior to prevent oral disease progression in older adults.

There is increasing recognition, made explicit in the new classification for periodontitis, that periodontitis is a lifelong disease that is not ‘cured’ but rather ‘managed’. This paper focuses on how the response to periodontal treatment is ideally measured and how decisions are made as to whether the treatment has been ‘successful’ or not. The roles of both the patient and practitioner in the maintenance of periodontal health for those patients who respond to initial therapy are crucial. Patients not responding to initial, non-surgical periodontal therapy also need to be appropriately managed, as outlined in this paper. CPD/Clinical Relevance: This paper highlights the importance of maintenance of periodontal health, as an integral part of the overall management of patients with periodontitis, in order to minimize further periodontal breakdown and eventual tooth loss.

Abstract Aim The aims of the present study are to determine the causes and pattern of loss of permanent teeth among patients attending a dental teaching institution in southern India. Methods and Materials Data collected from patients attending the outpatient wing of the Sri Sankara Dental College, Kerala, during a three month period was used for the study. The cause for extraction was classified as follows: (1) caries and its sequelae, (2) periodontal disease, (3) orthodontic purposes, (4) impactions, (5) prosthodontic purposes, and (6) other reasons. Results A total of 1791 permanent teeth were extracted of which 708 (39.5%) teeth were extracted due to caries and its sequelae, 508 (28.4%) due to periodontal disease, 347 (19.4%) for orthodontic purposes, 29 (1.6%) due to impactions, 155 (8.7%) for prosthodontic purposes, and 44 (2.5%) for other reasons. Conclusion The results of the present study suggest caries and periodontal disease are the major causes of tooth mortality in the study population. Clinical Significance Data regarding the causes of tooth loss indirectly provides invaluable information on the pattern of oral health in a population which can be utilized for planning public health policies designed to address the burden of oral diseases.
