New recommendations for hypertension (HTN) diagnosis and treatment highlight the role of self-care activities in managing blood pressure (BP). This cross-sectional study investigated the predictive validity of the Hypertension Self-Care Activity Level Effects (H-SCALE) measure and examined the relative and cumulative effects of HTN self-care adherence on BP. We pooled baseline data from three studies ( N = 79), resulting in a gender and racially balanced sample. Partial correlations determined the relative effects of individual self-care behaviors on BP. We modeled the relationship between adherence to self-care behaviors and BP control using logistic regression. Physical activity had the greatest correlation with systolic BP. Adherence to each additional self-care behavior increased the odds of systolic BP control by 88% (95% confidence interval (CI) = [1.20, 2.96]) and diastolic BP control by 74% (95% CI = [1.10, 2.75]). Results provide further evidence that the H-SCALE is a valid assessment tool and should be adopted by clinicians to aid in improving BP management.

Background: Despite the availability of effective drugs, blood pressure (BP) control rate is sub-optimal in individuals with hypertension in low- and middle-income countries (LMICs). The role of self-care in the management of BP is less studied in LMIC settings. Methods: We conducted a community-based, cross-sectional study in individuals with hypertension in Kollam district, Kerala. A multistage cluster sampling method was used for the selection of study participants. We measured self-care by using an adapted Hypertension Self-Care Activity Level Effects (H-SCALE) scale. Descriptive statistics were used to summarise the data and logistic regression analysis was conducted to identify factors associated with BP control. Results: In total, 690 individuals with hypertension (women=60%) and a mean age of 57±8 years participated in the study. More than half (54%) of the participants were adherent to anti-hypertensive medications. However, the adherence rate was much lower for the dietary approach to stop hypertension (DASH) diet (12.8%), recommended level of physical activity (24%) and weight management (11.4%). Overall BP control was achieved in two of five individuals (38.4%, 95% CI: 34.7-42.0%). Among self-care activities, adherence to medications (AOR: 1.8, 95% CI: 1.3-2.5), DASH diet (AOR: 1.5, 95% CI: 1.0-2.4), and non-smoking status (AOR: 3.3, 95% CI: 1.7-6.4) were associated with control of BP. Additionally, good family support to self-care (AOR: 1.9, 95% CI: 1.1-3.1) was associated with better control of BP. Conclusion: In individuals with hypertension, the BP control rate is achieved in two of five individuals. Adoption of self-care activities are sub-optimal. Both family support and adherence to self-care activities are associated with BP control.  Family based interventions to improve adherence to self-care activities could have a significant public health impact in achieving better population-level BP control rates in Kerala, India.

Noncommunicable Diseases (NCDs) merupakan salah satu penyebab kematian utama di dunia. Kurangnya aktivitas fisik dapat meningkatkan risiko penyakit ini. Meskipun telah banyak penelitian yang menyelidiki tentang hubungan aktivitas fisik dan faktor risiko penyakit kardiovaskuler, hanya terdapat beberapa penelitian di Indonesia yang menelaah tentang peran aktivitas fisik dalam perkembangan penyakit kardiovaskuler. Penelitian ini bertujuan untuk mendeskripsikan tentang tingkat aktivitas fisik dan menyelidiki hubungannya dengan faktor risiko penyakit kardiovaskuler pada karyawan kantor di lingkungan Universitas Sebelas Maret (UNS). Penelitian ini menggunakan desain penelitian potong lintang, dengan jumlah sampel sebanyak 44 orang. Data level aktivitas fisik diperoleh dari International Physical Activity Questionnaire (IPAQ). Sedangkan faktor-faktor risiko penyakit kardiovaskuler (gula darah, kolesterol, tekanan darah, lingkar pinggang, dan IMT) diukur dengan menggunakan alat ukur yang sesuai. Hasil penelitian menunjukkan bahwa level aktivitas fisik karyawan berkategori Rendah sebesar 6.82%, Sedang sebesar 72.73%, dan Tinggi sebesar 20.45%. Berdasarkan uji Pearson Correlation, tekanan darah baik sistolik dan diastolik mempunyai hubungan yang signifikan dengan level aktivitas fisik. Tidak ada hubungan yang signifikan antara level aktivitas fisik dengan faktor risiko lainnya dalam penelitian ini. Secara umum penelitian ini menunjukkan bahwa level aktivitas fisik karyawan di lingkungan UNS adalah berkategori sedang, dan tekanan darah mempunyai hubungan yang signifikan dengan aktivitas fisik. Physical activity level and cardiovascular risk factors among university employees AbstractNoncommunicable Diseases (NCDs) are one of the leading deaths in the world. Insufficiency of physical activity (PA) escalate the risk of NCDs. Although the reports of the studies investigating the relationship between PA and cardiovascular risk factors are abundance, only a few studies in Indonesia examined the role of PA in the development of cardiovascular diseases. The aim of this study was to describe the level of PA and investigate the relationships between PA and cardiovascular risk factors among UNS employees. This was a cross sectional study with a sample of 44 university employees. PA level was assessed using IPAQ. Meanwhile cardiovascular risk factors such as plasma glucose, cholesterol, blood pressure, waist circumference, and BMI were measured using an appropriate tool for each measure. The results revealed that participants with low level of PA (6.82%), moderate category (72.73%), and high category (20.45%). Pearson Correlation analysis showed blood pressure both systolic and diastolic had a significant association with PA level. There was no a significant association between PA level and others cardiovascular risk factors in this research. Generally, this study exhibited the level of PA among UNS employees in the moderate category, and blood pressure had a significant association with PA.

Background Pancreatic and periampullary cancers are rare but have high mortality rates. The only hope for cure is surgical removal of the tumor. Following pancreatic surgery, the patients have a great deal of responsibility for managing their symptoms. Patients report a lack of sufficient knowledge of self-care and unmet supportive care needs. This necessitates a health care system responsive to these needs and health care professionals who pay close attention to symptoms. Person-centered care is widely encouraged and means a shift from a model in which the patient is the passive object of care to a model involving the patient as an active participant in their own care. To address the challenges in care following pancreatic cancer surgery, an interactive app (Interaktor) was developed in which patients regularly report symptoms and receive support for self-care. The app has been shown to reduce patients’ symptom burden and to increase their self-care activity levels following pancreaticoduodenectomy due to cancer. Objective The aim of the study was to describe how patients used the Interaktor app following pancreaticoduodenectomy due to cancer and their experience with doing so. Methods A total of 115 patients were invited to use Interaktor for 6 months following pancreaticoduodenectomy. Of those, 35 declined, 8 dropped out, and 46 did not meet the inclusion criteria after surgery, leaving 26 patients for inclusion in the analysis. The patients were instructed to report symptoms daily through the app for up to 6 months following surgery. In case of alerting symptoms, they were contacted by their nurse. Data on reported symptoms, alerts, and viewed self-care advice were logged and analyzed with descriptive statistics. Also, the patients were interviewed about their experiences, and the data were analyzed using thematic analysis. Results The patients’ median adherence to symptom reporting was 82%. Fatigue and pain were the most reported symptoms. Alerting symptoms were reported by 24 patients, and the most common alert was fever. There were variations in how many times the patients viewed the self-care advice (range 3-181 times). The most commonly viewed advice concerned pancreatic enzyme supplements. Through the interviews, the overarching theme was “Being seen as a person,” with the following 3 sub-themes: “Getting your voice heard,” “Having access to an extended arm of health care,” and “Learning about own health.” Conclusions Interaktor proved to be well accepted. It made patients feel reassured at home and offered support for self-care. The app facilitated person-centered care by its multiple features targeting individual supportive care needs and enabled participation in their own care. This supports our recent studies showing that patients using the app had less symptom burden and higher self-care activity levels than patients receiving only standard care.

Post prandial glucose (PPG) level describes the speed of glucose absorption after 2 hours of macronutrient consumption. By knowing this, we could get the big picture of insulin regulation function and macronutrient metabolism in our body. In elderly, age-related slower glucose metabolism leads to diabetes mellitus (DM) in older age. This study aimed to analyze the predictors of PPG level in diabetics elderly which consist of functional status, self-care activity, sleep quality, and stress level. Cross-sectional study design was applied in this study. There were 45 diabetic elderly participated by filling in study instruments. Pearson and Spearman Rank correlation test were used in data analysis (α<.05). Results showed that most respondents were female elderly, 60-74 years old, had DM for 1-5 years with no family history, and only 33.33% respondents reported regular consumption of oral anti diabetes (OAD). Hypertension was found to be frequent comorbidity. Statistical analysis results showed that functional status, self-care activity, sleep quality, and stress level were not significantly correlated with PPG level in diabetic elderly (all p>α), therefore these variables could not be PPG level predictors. Other factors may play a more important role in predicting PPG level in diabetic elderly.

The purpose of this study is to assess the effectiveness of a theory-based self-care intervention with the application of health literacy strategies in patients with high blood pressure and limited health literacy. This is a randomized controlled trial, with measurements at baseline and 1 and 3 months follow-up. 100 patients with high blood pressure and limited health literacy will be randomly allocated to either an intervention group or a usual care control group. We will mainly establish the intervention model based on the principal health belief model components. Patients randomized to the intervention group will receive four educational sessions during four weeks. Considering the limited health literacy level of the patients of the study, health literacy strategies will be used in educational material design for enhancing the quality of the intervention. In order to cover these strategies, we will design four standard animated comics and fact sheets with illustrations and photos consistent with the health belief model constructs and educational sessions’ topics. Data will be collected using some questionnaires and will be analyzed using the SPSS software. The findings of this study may assist with the development of a theoretical model for self-care intervention in patients with high blood pressure and limited health literacy.
