Object. Cigarette smoking is associated with aneurysmal subarachnoid hemorrhage (SAH) and subsequent vasospasm. The purpose of this study was to quantify this association. Methods. Nearly 3500 patients with SAH from North America and Europe have been enrolled in five different multicenter, controlled studies coordinated at the Neuroclinical Trials Center of the Virginia Neurological Institute at the University of Virginia. Among the prospective data gathered were whether the patient smoked at the time of their most recent SAH and the evolution of angiographic vasospasm. The rate of smoking in the patients enrolled in the studies was compared with the expected rate by using a chi-square statistic adjusted for age and gender, in the general population in the United States (U.S.) and Europe. In virtually all age and gender subgroups, and for the combined populations in the five clinical trials, patients with SAH reported current smoking rates 2.5 times higher than expected based on U.S. and European national surveys (p < 0.0001). Cigarette smoking was also associated with younger age at onset of SAH (5–10 years, p < 0.0001) and increased incidence of clinically confirmed vasospasm (p < 0.005). Conclusions. The findings of a significantly increased representation of current cigarette smokers in the study populations and significant association with younger age at the time of SAH and increased incidence of vasospasm concur with recent reports of smoking as a significant risk factor for ruptured aneurysms and subsequent vasospasm.

AbstractObjectivesChildren from different countries and with different ethnic backgrounds have a distinct pattern of central fat deposition. Therefore, it is essential to develop population-specific percentiles of waist circumference (WC), waist-to-height ratio (WHtR) and waist-to-height ratio exponent (WHtR (exp)) for the evaluation of central obesity. The objective of this study was to develop age-and-gender-specific smoothed WC and WHtR percentile curves for the Pakistani children and adolescents aged 2–18 years.MethodsA cross-sectional data-set from a multi-ethnic anthropometric survey was considered. A sample of 10,668 healthy subjects (boys = 51.92%; and girls = 48.08%), aged 2–18 years was studied. Height (cm) and WC (cm) of each subject was measured under standard procedure and WHtR & WHtR (exp) were calculated. Age-and-gender-specific smoothed curves were obtained using the lambda-mu-sigma (LMS) method and compared with percentile curves obtained from different countries.ResultsExcept few early ages, the WC values increased with age in both sexes. Both boys and girls had approximately similar WC during 6–11 years of age and after age of 11, the boys had larger WC than the girls had. For WHtR, the centile curves showed a continuous decrease by 16 years of age and then increased gradually. WHtR of the girls in various ages were having similar or higher than those of the boys. In comparison of WC 50th and 90th percentiles with other countries, it was found that except few ages, the Pakistani children had larger WC than the other reference populations and the results of WHtR were also comparable to the other nations.ConclusionsWe present new reference data of WC, WHtR and WHtR (exp) using a representative sample of the Pakistani children aged 2–18 years. These reference values can be used provisionally for early detection of central obesity and its associated risks in the Pakistani children.

It has been demonstrated that normative data of pinch strengths (PSs) vary among healthy populations. Data from 838 participants (420 women and 418 men) aged 15–96 years were analyzed to develop normative data of PSs for a healthy Turkish population and to analyze changes in the means according to the physical demands of participants’ jobs. Lateral, palmar, and tip-to-tip pinch strengths were measured in standard manners. Occupations were classified based on the descriptions in the Dictionary of Occupational Titles. Mean PSs peaked between the ages of 30–50 years in general and increased a small amount in parallel with the increasing strength ratings of the occupations. PS changes followed a curvilinear relationship to age. The differences between our findings and those of similar studies in this field emphasize the importance of using normative data specific to a particular population.

Abstract This study investigates the impact of male labor migration upon wives living among their husbands’ extended families in Tajikistan. It studies the risks and choices available to such wives in bargaining for remittances, with a particular focus on the risks that daughters-in-law (kelin in Tajik) undertake when negotiating remittances with their mothers-in-law. This paper explores age and gender-specific norms in Tajik transnational families and their minimal opportunities for kelins to bargain and negotiate the risks associated with making “claims” on remittances by using Deniz Kandiyoti’s “patriarchal bargain” and Bina Agarwal’s household bargain framework, as well as extensive fieldwork conducted in Tajikistan. The study concludes that international migration and remittances have had a complex impact on gender norms in Tajikistan, with emerging new forms of passive negotiation by kelins unlikely to undermine patriarchal gender norms in their favor.
