ABSTRACT Objective: The aim of this study was to investigate the impact of erosive tooth wear on the Oral Health-Related Quality of life (OHRQoL) of preschool children. Methods: Dental examinations were conducted on 815 children aged 3-4 years during the Children’s Vaccination National Day when their parents were also invited to answer the Brazilian Early Childhood Oral Health Impact Scale (B-ECOHIS). ETW prevalence and severity were measured using a modified version of the O’Brien index (1994). Data collected included socioeconomic factors and child's variables. OHRQoL was measured through B-ECOHIS domains and total score. Poisson regression was used to associate ETW to the outcome and this association was adjusted for dental caries and dental trauma. Results: The proportion of children who had at least 1 ETW tooth was 51.2%. Most erosive lesions were confined to enamel (42.7%). The multivariate adjusted model showed that child's age (children aged 4year-old) was associated to a negative impact on the symptom domain (RR=1.70; p=0.010), functional limitation domain (RR=1.85; p=0.005) and total B-ECOHIS score (RR= 1.63; p=0.006). Families with 2 or more children in the house have a negative impact on the self-image/social interaction domain (RR=5.41; p=0.043). ETW was not associated to total B-ECOHIS scores (RR= 0.79; p=0.163) and individual domains. Conclusion: Erosive tooth wear does not affect the OHRQoL in this sample of preschool children.

Abstract Objectives To analyse possible changes in oral health-related quality of life (OHRQoL) before and after dental treatment under dental general anaesthesia (DGA) among Austrian preschool children. Methods A consecutive sample of 89 parents of children aged 2 to 5 years, suffering from early childhood caries (ECC) and scheduled for DGA, were recruited from two locations in Austria (Vienna and Salzburg). Parents self-completed the German version of the ECOHIS before (baseline) and 4 weeks (T4) after their child’s dental treatment. The ECOHIS consists of 13 questions and is divided into two main parts, namely, the child impact section (9 items) and the family impact section (4 items). Results A total of 80 children (89%) completed a sufficient number ECOHIS questions at baseline and the follow-up assessment after 4 weeks. “Pain in the teeth, mouth, and jaws” and “difficulty eating some foods” from the child section and parents’ ratings of “feeling upset” and “guilty” were the most frequently reported impacts at baseline. The ECOHIS total score decreased significantly from a mean of 14.60 to 9.89 (p < 0.001) after DGA treatment, revealing a large effect size for the child (0.8) section, family (0.6) section, and the total score (0.8). Parents rated their child’s overall and oral health significantly higher after the DGA treatment (p < 0.001). Conclusions Significant improvements in oral health-related quality of life were observed 4 weeks after DGA in children suffering from ECC. Clinical relevance ECC has an impact on OHRQoL. Rehabilitation under general anaesthesia makes a sustainable improvement.

Objetivo: O objetivo deste estudo foi compreender a percepção materna sobre a qualidade de vida de crianças, portadoras de cárie na primeira infância (CPI). Material e Método: O objeto de estudo foram seis mães de crianças em tratamento na Clínica de Odontologia da Universidade de Ribeirão Preto. Realizou-se entrevista semi-estruturada gravada. Os dados foram analisados segundo a técnica de Análise de Conteúdo proposta por Bardin e utilizando-se os conceitos teóricos oriundos de Bronfembrenner para avaliar aspectos da CPI que interferem no pleno desenvolvimento da pessoa desde a infância. Depreendeu-se dos dados, 03 categorias empíricas: 1ª - “o dente natural é tudo”; 2ª - “ Procura o dentista quando tem dor” e 3ª - “A mãe tem responsabilidade no aparecimento da cárie porque não limpa a boca da criança”. Resultados: Os resultados demonstraram que a influência dos aspectos sócio-econômicos e educacionais das mães tornam as crianças mais vulneráveis à CPI. As mães compreendem a importância dos dentes na saúde bucal, porém, não atribuem a presença da CPI a predispor as crianças ao desenvolvimento de problemas sistêmicos e afetar o desenvolvimento biopsicossocial. As mães apontaram falhas no atendimento das crianças e de acesso ao serviço público, assim como, o despreparo dos profissionais do serviço público e privado. Abstract Background : Preschool years are a critical period in the development of a healthy child. The consequences of poor oral health in preschool children reach beyond dental problems, with oral health-related quality of life (OHRQoL) being associated with overall systematic health as well as one’s quality of life. The purpose of this study was to assess the prevalence of dental caries and its impacts on the OHRQoL in a sample of preschool children in Kisarawe. Methods: A cross-sectional based study was conducted in 2017. A total of 1,106 preschool children completed a face-to-face interview, using a translated Kiswahili version of the Michigan Oral Health-related Quality of Life Scale (MOHRQoL) –Child Version (2003), and underwent clinical oral examination using WHO (1997) criteria. Results: The decayed component was the most prevalent (dft = 2.08) and the Significant Caries Index (SiC) was 5.54 double of the (dft), showing polarization of dental caries in the studied population. After adjusting for appropriate covariates, preschool children of age 5 and 6 years old were more likely to have decayed tooth [Adjusted OR = 3.02, (95% CI =2.01-4.54)] and [Adjusted OR=2.23, (95% CI= 1.55-3.20)] respectively. Preschool children without visible plaque on the buccal surface of upper anterior teeth were less likely to have decayed teeth [Adjusted OR=0.21, (95% CI=0.09-0.45)]. Regarding measurements of oral health-related quality of life using the MOHRQoL, only preschool children who reported on ‘do your teeth hurt you now?’ and ‘do kids make fun of your teeth?’ were more likely to have a decayed tooth [Adjusted OR=1.74, (95% CI=1.12-2.71)] and [Adjusted OR=1.87, (95% CI=1.11-3.15)], respectively. Conclusion: Findings from this study suggest that dental caries affects a significant portion of preschool children and, was associated with poor oral hygiene. The overall impacts of dental caries prevalence to OHRQoL were low in this sample of preschool children . Children having caries (independent variable) were shown to report more frequently that ‘do your teeth hurt you now?’ and ‘do kids make fun of your teeth?’ were more likely to have a decayed tooth among preschool children in Kisarawe, Tanzania.
