Comparison of FIB-4 index, NAFLD fibrosis score and BARD score for prediction of advanced fibrosis in adult patients with non-alcoholic fatty liver disease: A meta-analysis study

(1) Background: FibroTest™ is a multi-marker panel, suggested by guidelines as one of the surrogate markers with acceptable performance for detecting fibrosis in patients with non-alcoholic fatty liver disease (NAFLD). A number of studies evaluating this test have been published after publication of the guidelines. This study aims to produce summary estimates of FibroTest™ diagnostic accuracy. (2) Methods: Five databases were searched for studies that evaluated FibroTest™ against liver biopsy as the reference standard in NAFLD patients. Two authors independently screened the references, extracted data, and assessed the quality of included studies. Meta-analyses of the accuracy in detecting different levels of fibrosis were performed using the bivariate random-effects model and the linear mixed-effects multiple thresholds model. (3) Results: From ten included studies, seven were eligible for inclusion in our meta-analysis. Five studies were included in the meta-analysis of FibroTest™ in detecting advanced fibrosis and five in significant fibrosis, resulting in an AUC of 0.77 for both target conditions. The meta-analysis of three studies resulted in an AUC of 0.69 in detecting any fibrosis, while analysis of three other studies showed higher accuracy in cirrhosis (AUC: 0.92). (4) Conclusions: Our meta-analysis showed acceptable performance (AUC > 0.80) of FibroTest™ only in detecting cirrhosis. We observed more limited performance of the test in detecting significant and advanced fibrosis in NAFLD patients. Further primary studies with high methodological quality are required to validate the reliability of the test for detecting different fibrosis levels and to compare the performance of the test in different settings.

Non-alcoholic fatty liver disease (NAFLD) is one of the main causes of fibrosis. Liver biopsy remains the gold standard for the confirmation of fibrosis in NAFLD patients. Effective and non-invasive diagnosis of advanced fibrosis is essential to disease surveillance and treatment decisions. Herein we used routine medical test markers and logistic regression to differentiate early and advanced fibrosis in NAFLD patients from China, Malaysia, and India (n1 = 540, n2 = 147, and n3 = 97) who were confirmed by liver biopsy. Nine parameters, including age, body mass index, fasting blood glucose, presence of diabetes or impaired fasting glycemia, alanine aminotransferase, γ-glutamyl transferase, triglyceride, and aspartate transaminase/platelet count ratio, were selected by stepwise logistic regression, receiver operating characteristic curve (ROC), and hypothesis testing and were used for model construction. The area under the ROC curve (auROC) of the model was 0.82 for differentiating early and advanced fibrosis (sensitivity = 0.69, when specificity = 0.80) in the discovery set. Its diagnostic ability remained good in the two independent validation sets (auROC = 0.89 and 0.71) and was consistently superior to existing panels such as the FIB-4 and NAFLD fibrosis score. A web-based tool, LiveFbr, was developed for fast access to our model. The new model may serve as an attractive tool for fibrosis classification in NAFLD patients.

ObjectiveLiver biopsy is still needed for fibrosis staging in many patients with non-alcoholic fatty liver disease. The aims of this study were to evaluate the individual diagnostic performance of liver stiffness measurement by vibration controlled transient elastography (LSM-VCTE), Fibrosis-4 Index (FIB-4) and NAFLD (non-alcoholic fatty liver disease) Fibrosis Score (NFS) and to derive diagnostic strategies that could reduce the need for liver biopsies.DesignIndividual patient data meta-analysis of studies evaluating LSM-VCTE against liver histology was conducted. FIB-4 and NFS were computed where possible. Sensitivity, specificity and area under the receiver operating curve (AUROC) were calculated. Biomarkers were assessed individually and in sequential combinations.ResultsData were included from 37 primary studies (n=5735; 45% women; median age: 54 years; median body mass index: 30 kg/m2; 33% had type 2 diabetes; 30% had advanced fibrosis). AUROCs of individual LSM-VCTE, FIB-4 and NFS for advanced fibrosis were 0.85, 0.76 and 0.73. Sequential combination of FIB-4 cut-offs (<1.3; ≥2.67) followed by LSM-VCTE cut-offs (<8.0; ≥10.0 kPa) to rule-in or rule-out advanced fibrosis had sensitivity and specificity (95% CI) of 66% (63–68) and 86% (84–87) with 33% needing a biopsy to establish a final diagnosis. FIB-4 cut-offs (<1.3; ≥3.48) followed by LSM cut-offs (<8.0; ≥20.0 kPa) to rule out advanced fibrosis or rule in cirrhosis had a sensitivity of 38% (37–39) and specificity of 90% (89–91) with 19% needing biopsy.ConclusionSequential combinations of markers with a lower cut-off to rule-out advanced fibrosis and a higher cut-off to rule-in cirrhosis can reduce the need for liver biopsies.

Εισαγωγή: Η μη αλκοολική λιπώδης νόσος του ήπατος (Non-alcoholic fatty liver disease, NAFLD) συνδυάζεται με την παχυσαρκία, την ινσουλινοαντίσταση και την υπερλιπιδαιμία. Στην παθογένειά της εμπλέκονται και ορμόνες (αδιπονεκτίνη, λεπτίνη) που εκκρίνονται από τον λιπώδη ιστό. Ωστόσο, δεν υπάρχει γι’ αυτήν αποτελεσματική θεραπεία. Καθώς η ίδια μοιράζεται κοινούς παθογενετικούς μηχανισμούς με την καρδιαγγειακή νόσο, αλλά και βάσει μελετών που υποστηρίζουν την ευεργετική επίδραση των στατινών στα αυξημένα επίπεδα τρανσαμινασών, διερευνήθηκε η επίδραση της χαμηλής (5mg) και υψηλής (40mg) δόσης ροσουβαστατίνης στη λιπώδη διήθηση, την 24ωρη αρτηριακή σκληρία και τις αδιποκίνες ορού σε ασθενείς με NAFLD. Ασθενείς και Μέθοδοι: Μελετήθηκαν ασθενείς με αυξημένα επίπεδα πυροσταφυλικής τρανσαμινάσης (SGPT) (χαμηλότερα όμως από το τριπλάσιο των ανώτερων φυσιολογικών ορίων) και/ή ηπατική στεάτωση στα εξωτερικά ιατρεία της Β’ Προπαιδευτικής Παθολογικής του Ιπποκρατείου Νοσοκομείου Θεσσαλονίκης, ενώ αποκλείστηκαν αυξημένη κατανάλωση αλκοόλ και άλλες αιτίες ηπατικής νόσου. Η ηπατική στεάτωση ελέγχθηκε με υπερηχογράφημα, και η πιθανότητα ίνωσης εκτιμήθηκε με τους δείκτες NAFLD fibrosis score (NFS) και Fibrosis-4 index (FIB-4). Η 24ωρη αρτηριακή σκληρία εκτιμήθηκε με συσκευή Mobil-O-Graph μέσω της ταχύτητας μετάδοσης σφυγμικού κύματος (Pulse Wave Velocity, PWV) και του δείκτη ενίσχυσης του σφυγμικού κύματος (Augmentation Index, AIx). Η αδιπονεκτίνη και η λεπτίνη μετρήθηκαν στον ορό με την τεχνική της μη ανταγωνιστικής ενζυμοσύνδετης ανοσοαπορρόφησης. Αποτελέσματα: Συμπεριλήφθηκαν 43 ασθενείς. Οι 21 τυχαιοποιήθηκαν να λάβουν 5mg ροσουβαστατίνη, ενώ 22 άτομα έλαβαν 40mg ροσουβαστατίνης και όλοι επανεκτιμήθηκαν μετά από 6 μήνες. Στην ομάδα της χαμηλής δόσης, τελικά, κανείς δεν είχε σοβαρή στεάτωση (ενώ αρχικά είχε το 4,7%) και φυσιολογικό ήπαρ ή ήπια στεάτωση είχε το 85,7% (ενώ αρχικά ήπια στεάτωση είχε το 52,4%) (p=0,045). Προχωρημένη ίνωση βάσει NFS αρχικά είχε το 9,5% και βάσει FIB-4 το 4,7%, όμως στο τέλος δεν είχε κανείς (p=0,003 και p=0,006, αντίστοιχα). Στην ομάδα της υψηλής δόσης, αρχικά, σοβαρή στεάτωση είχε το 9,1%, ενώ στο τέλος κανείς και ήπια στεάτωση ή φυσιολογικό ήπαρ είχε τελικά το 91% (έναντι 40,9% αρχικά) (p=0,012). Βάσει NFS, ενώ αρχικά η προχωρημένη ίνωση ήταν απίθανη για το 50%, στο τέλος ήταν για το 54,4% (p<0,001) και βάσει FIB-4, η πιθανότητα για προχωρημένη ίνωση από 13,6% μειώθηκε στο 4,6% (p<0,001). Σχετικά με την αρτηριακή σκληρία, η PWV δεν διέφερε σημαντικά στην έναρξη και στη λήξη της μελέτης για την ομάδα της χαμηλής δόσης, ενώ ο AIx παρουσίασε μείωση (από 27,47,2% σε 25,27,3%, p <0,001). Στην ομάδα που έλαβε την υψηλή δόση, τόσο η PWV όσο και ο AIx μειώθηκαν στατιστικώς σημαντικά (από 8,11,8 m/s σε 7,81,9 m/s, p=0,002 και από 27,49,0% σε 22,45,0%, p <0,001). Για την ομάδα της δόσης των 40 mg, η λεπτίνη μειώθηκε σημαντικά (από 18,19,0 ng/ml σε 14,38,9 ng/ml, p<0,001), ενώ η αδιπονεκτίνη παρουσίασε σημαντική αύξηση (από 5,33,7 μg/ml σε 8,64,3 μg/ml, p<0,001). Συμπεράσματα: Τα ευρήματα της παρούσας μελέτης υποδεικνύουν την ευεργετική επίδραση της ροσουβαστατίνης στη λιπώδη διήθηση του ήπατος. Και οι δύο δόσεις βελτιώνουν τη στεάτωση και την ίνωση, αλλά η υψηλότερη σε μεγαλύτερο βαθμό. Μόνο η μεγάλη δόση ελαττώνει την PWV, ενώ ο AIx ελαττώνεται και με τις δύο δόσεις, όμως υπάρχει τάση η ελάττωση να είναι μεγαλύτερη με την υψηλή δόση. Τέλος, η υψηλή δόση ελάττωσε σημαντικά την λεπτίνη και μείωσε την αδιπονεκτίνη.
