Is it necessary to perform eye examination for patients with cutaneous atypical nevi?

Background Suppression is associated with binocular vision conditions such as amblyopia and strabismus. Commercial methods of testing fusion often only measure central fusion or suppression at near. The purpose of this pilot study was to assess a new iPad picture fusion test that assesses foveal and central fusion at near. Methods Participants aged 5 years and older presenting for eye examination at The Ohio State University College of Optometry were enrolled. Results from visual acuity, dry and wet refraction/retinoscopy, stereopsis and cover testing were recorded from the patient chart. The iPad picture fusion test, Worth four-dot, Worth type test with foveal letter targets, and Polarized four-dot were performed by one examiner in a randomized order at 40 cm. Testing was repeated with the anaglyphic filters reversed. Crosstabulation and McNemar chi-square analysis were used to compare the results between fusion testing devices. Results Of the fifty participants (mean age = 17.5), twelve reported suppression and one reported diplopia. Testability was excellent for all tests (98% to 100%). There were no significant differences between tests in reported results (P ≥ 0.22 for all comparisons). No difference in reported fusion or suppression status was observed with change in orientation of the anaglyphic filters. Six participants reported foveal suppression alone at near which was not identified with Worth four-dot at near. Conclusion The iPad picture fusion test provided excellent testability and agreement with commonly used tests of fusion and allowed testing of both central and foveal fusion at near. Nearly half (46%) of participants with suppression reported foveal suppression, supporting the importance of testing for foveal suppression.

Cytohistological comparisons to the account of the clinical data and revision of cytological and histological preparations on a material received from 21 patients are made. Cytomorphological features of juvenile nevi (Spitz-nevus, Reed-nevus), dysplastic and atypical nevi and early forms of melanoma are described. The establishment at cytological examination of good-quality character of melanocytic defeats at the account of the clinical data could be the basis for appointment laser therapy. At revealing of atypical melanocytes in cytological preparations patients should be referred to specialized oncological institutions for surgical excision of tumor with the subsequent histological examination. Thus cytological examination could be used in dermatological practice as a method of screening pre-malignant melanocytic tumors and skin melanoma.

Objective: The objective of the study was to compare cytokine levels in the vitreous body of patients with proliferative diabetic retinopathy (PDR) undergoing posterior vitrectomy. Patients and methods: The study included 39 patients (39 eyes) undergoing pars plana vitrectomy (PPV). Patients were divided into three groups: patients with proliferative diabetic retinopathy (PDR) without aflibercept injection prior to the surgery, PDR patients administered aflibercept injection prior to the surgery, and patients without diabetes mellitus (control group). All patients underwent a comprehensive eye examination one day before and 3 weeks after the surgery, including measurements of: best-corrected visual acuity (BVCA) and intraocular pressure (IOP), slit-lamp examination and spectral domain optical coherence tomography (SOCT). Concentrations of cytokines: IL-6, IL-8, IL-12p70, TNF, IL-10, IL-1β were measured in the vitreous body of patients with BD™ Cytometric Bead Array (CBA) Human Inflammatory Cytokines Kit. Results: PDR patients who received pretreatment with aflibercept injection showed significantly lower concentrations of IL-12p70, TNF, IL-10 and IL-1β in the vitreous body compared to the control group. Meanwhile, patients without prior aflibercept injection had a significantly higher concentration of IL-8. There was also a significant positive correlation between IOP before PPV and IL-8 concentration in both PDR patients’ groups. Conclusion: Findings of our study suggest an important role of IL-8 in the development of severe PDR. Aflibercept administration on the day before elective vitrectomy facilitated the surgery.

Purpose: Although acute conjunctivitis has been listed from the beginning as a possible sign of COVID-19, the likelihood of this association remains unclear. The aim of this study was to investigate the relationship between COVID-19 and conjunctivitis. Methods: In this retrospective, observational study, we recruited all patients with signs and symptoms of acute conjunctivitis seen at the Eye Emergency Department (ED), Turin Eye Hospital, between 01/01/2020 and 12/05/2020 and cross-checked our data with the Piedmont Region online COVID-19 registry in the same period. Results: Among 10,065 patients seen at our ED during the timespan considered, 88 underwent a nasopharyngeal swab (NS) for SARS-CoV-2 detection within 4 weeks before/after our examination. On average, NS was performed −0.72 ± 1.8 weeks before/after eye examination. Of the 77 patients with a negative NS, 26 (33.8%) had a diagnosis of acute conjunctivitis, whereas the remaining 51 (66.2%) had other eye disorders. Among the 11 patients with COVID-19, 7 (63,6%) had a diagnosis of acute conjunctivitis. We found a non-statistically significant increase in NS positivity rate (21.2%) among cases examined at our ED for acute conjunctivitis, compared to the NS positivity rate (7.3%) in patients examined for all other eye conditions ( p = 0.092). The Odds Ratio of having a positive NS in patients with acute conjunctivitis was 3.43 (95% I.C. = 0.9–12.8, p = 0.06). Considering online-registry data of Turin population during the same time-span, among 2441 positive NS cases only 27 (1.1%) presented with acute conjunctivitis. Conclusion: Our results do not reveal a statistically significant correlation between COVID-19 and acute conjunctivitis. Synopsis The present study analyzes retrospectively data from a tertiary eye referral center to investigate the relationship between COVID-19 infection and conjunctivitis.

Children are primarily examined by paediatricians. In Nepal, child health is in low priority due to illiteracy, there is total lack of awareness about children’s eye health. The common avoidable causes of childhood blindness are refractive errors, amblyopia, retinopathy of prematurity (ROP), vitamin A deficiency (VAD), xerophthalmia, ophthalmia neonatorum (ON), congenital cataract, and retinoblastoma. Paediatricians could be the key persons for early referral of these children to a paediatric ophthalmologist. Paediatricians can send the patients for eye examination after birth within 6 weeks, at 6 months, at one year and before going to school. In conclusion, early screening and referral by the paediatricians to the paediatric ophthalmologist prevents children from being sightless. AbstractThe aim of this study is to find out the frequency of anisometropic amblyopia in children and to increase awareness about this disease. All children between the ages of 4 and 10 years who were attending 11 kindergarten and primary schools determined by provincial directorate for national education in the center of Malatya were included. A screening team including interns and a resident from the Department of Pediatrics have used Plusoptix A09 handheld photo-refractometer device for detecting amblyopia and its reasons. A total of 7,000 students were screened. The mean age of the students was 7.4 ± 2 years. Amblyopia was suspected in 357 cases with the Plusoptix S09 screening. After eye examinations performed by an ophthalmologist, 303 cases were found to have pathologic eye examination. Amblyopia was found in 67 (18.8%) of the 303 cases, and anisometropic amblyopia was found in 59 of 67 cases. As a result of the screening, sensitivity of Plusoptix A09 was found to be 88.1%, while its specificity was found to be 88.6%. Its positive predictive value was found to be 64.0% and its negative predictive value was found to be 97.0%. We believe that with screenings performed by health professionals who are given short-term training by using a device that can conduct remote measurements, detection of anisometropic amblyopia, which is the most frequent reason for amblyopia, can be done in a fast and relatively less expensive way and with the minimum number of specialist physician required. This way, children with amblyopia can reach the ophthalmologist earlier for treatment.

Background: Visual impairment is disproportionately distributed between rural and urban dwellers. Rural dwellers have limited access to the eye care services that are available in urban cities.Aim: The aim of this study was to determine the prevalence and causes of visual impairment in Dariyah, a rural community in the Qassim region, Saudi Arabia.Setting: This research comprised a population-based assessment of the vision and visual status of the inhabitants of Dariyah community.Methods: A cross-sectional descriptive study was conducted amongst the inhabitants of the community of Dariyah. A comprehensive eye examination consisting of visual acuity, ocular health examination, objective and subjective refraction was performed. The examinations were conducted by registered ophthalmologists and optometrists from the Qassim University. Vision impairments were categorised according to the International Classification of Diseases (ICD), Section 9D90, ‘Vision impairment including blindness’ (2018).Results: In all, 68 (24.5%) participants had vision impairment (using the better-seeing eye), with refractive errors being the main cause of vision impairment. Other causes of vision impairment included cataract (20, 7.2%), trichiasis (5, 1.8%) and glaucoma (2, 0.7%). Refractive errors were present in 193 (69.7%), with astigmatism being the commonest refractive error. About 60% of those with refractive errors presented without spectacles for correction, and 20% reported that they had never had an eye examination. Refractive errors could not be determined in 19 (6.9%) of the subjects because of conditions such as matured cataract and other ocular abnormalities.Conclusion: Uncorrected refractive error was found to be the leading cause of vision impairment in this rural community in Saudi Arabia.
