Paper goal is to determine complications after urgent surgical treatment of patient with penile fractures, by using circumferential-degloving technique (degloving penile skin to root of penis). In period between 1998-2006 year, 23 patients have been treated as urgent cases with clinically proven penis fracture, age between 18 and 35. Patients were treated in 3 medical centers in Bosnia and Herzegovina(Sarajevo, Tuzla and Zenica). All 23 (100%) patients were injured during sexual intercourse. In the case of 20 (86,9%) patients partial rupture of corpus cavernosum was verified. Two (8,6%) patients had a complete rupture of urethra and it was primary sutured. In 22 cases (95,6%) spontaneous erection appeared, and in the case of 2 (8,6%) patients penile curvature was verified. Urgent surgical treatment is the best therapy choice, which enables preservation of erection in great number of cases in patients with penile fracture.

Novel Applications

Ad hoc networks enable network creation on the fly without support of any predefined infrastructure. The spontaneous erection of networks in anytime and anywhere fashion enables development of various novel applications based on ad hoc networks. However, ad hoc networks present several new challenges. Different research proposals have came forward to resolve these challenges. This chapter provides a survey of current issues, solutions, and research trends in wireless ad hoc networks. Even though various surveys are already available on the topic, rapid developments in recent years call for an updated account. The chapter has been organized as follows. In the first part of the chapter, various ad hoc network issues arising at different layers of TCP/IP protocol stack are presented. An overview of research proposals to address each of these issues is also provided. The second part of the chapter investigates various emerging models of ad hoc networks, discusses their distinctive properties, and highlights various research issues arising due to these properties. The authors specifically provide discussion on ad hoc grids, ad hoc clouds, wireless mesh networks, and cognitive radio ad hoc networks. The chapter ends with a presenting summary of the current research on ad hoc networks, ignored research areas and directions for further research.

The Impact

Due to the lack of hospitals with adequate technical platform on one hand, and high diagnostic costs that cannot be afforded most of the population, the incidence of prostate cancer in Cameroon has increased and has great impact on people’s health. Aim: This work was undertaken with the objective to determine the impact of prostate cancer on erectile dysfunction and how to manage it. Methodology: Sampling of the population was done in a comprehensive and non-probabilistic manner at the Urology Department of Ngaoundere Islamic Hospital, Ngaoundere, Cameroon, between June 2018 and November 2019. Of the 75 patients received, 50 of them participated in this study. Biopsies were taken from these patients to determine and confirm the form and stage of cancer followed by PSA assays. After the diagnosis was revealed, the testosterone assay was carried out in order to evaluate erectile functioning in the patients who equally completed a survey form made available to them in order to get an idea of their health history, the type of treatment followed and their lifestyle. Results: The mean age of the patients was 67 years, with a predominance in the 60-70 age range. 85% of the patients had a Gleason score greater than or equal to 8. Of these patients, 42% had low testosterone levels (< 2.3 ng/mL), resulting to lack of morning erection (66.6%), loss of sexual desire (43.9%), difficulty having a spontaneous erection (88%). On the other hand, erectile dysfunction was revealed in the prostate cancer patients with low testosterone levels, with a history of hypertention (16.6%), diabetes (28.5%) alcohol consumption (44%), tobacco smoking (41%) and having undergone as prostate cancer treatment involving transurethral resection of the prostate (80%) and orchiectomy (20%). Conclusion: The major cause of erectile dysfunction observed in patients suffering from prostate cancer in Northen Cameroon can be attributed to the evolution of the disease, as well as the health history of the patients (diabetes, hypertension).

The article presents a clinical case from the practice of a patient with craniopharyngioma. The man of reproductive age with diencephalic obesity (BMI 35 kg/m2), recurrent craniopharyngioma, a long history of endocrine disorders (panhypopituitarism, including secondary hypogonadism, with the corresponding sexual function disorders and the lack of sexual activity), with visual disorders and psychiatric symptoms in the early postoperative period after shunting surgery and reducing the volume of the craniopharyngioma cyst, erection conditions arose against the background of episodes of disorders of consciousness within the framework of sleep dissociation. After the operation (installation of the Ommaya system), the patient had a state of spontaneous erection lasting up to 30 minutes against the background of dream-oneiric states of impaired consciousness with erotic experiences. These states were observed for 3 nights, the patient remembered the experiences and events that occurred to him in a dream, and could tell about them to others on the next morning. The identity of the patient remained intact, he was active in the department, ordered in behavior; memory for current events and new information was intact. These disorders in the patient did not require specialized treatment, regressed independently on the 6th day of the postoperative period.

The cholinergic neurons in the laterodorsal tegmental nucleus (LDT) play a crucial role in the regulation of rapid eye movement (REM) sleep. Because penile erection occurs during REM sleep, the involvement of the LDT in penile erection was examined in unanesthetized head-restrained rats. To detect penile erection, corpus spongiosum of the penis (CSP) pressure was measured through a telemetric device with simultaneous bulbospongiosum (BS) muscle EMG recording through stainless wires. Electrical stimulation in and around the LDT induced the following three CSP pressure patterns: 1) a full erection pattern indistinguishable from the nonevoked or spontaneous erection, characterized by a slow increase in CSP pressure with additional sharp CSP peaks associated with BS muscle bursts, 2) a muscular pattern characterized by sharp CSP pressure peaks but in the absence of a vascular component, i.e., without an increase in baseline CSP pressure, and 3) a mixed-type response characterized by high-frequency CSP pressure peaks followed by a full erection response. Full erections were evoked in and around the LDT, including more medially and ventrally. The sites for inducing mixed-type events were intermingled with the sites that triggered full erections in the anterior half of the LDT, whereas they were separated in the posterior half. The sites for muscular responses were lateral to the sites for full erections. Finally, a CSP pressure response identical to micturition was evoked in and around the Barrington's nucleus and in the dorsal raphe nucleus. These results suggest that the LDT and surrounding region are involved in the regulation of penile erection. Moreover, different anatomical areas in the mesopontine tegmentum may have specific roles in the regulation of penile erection and micturition.

Abstract The synthetic steroid 7α-methyl-19-nortestosterone (MENT) is a potent androgen that is resistant to 5α-reductase. It thus has decreased activity at the prostate and may have advantages over testosterone-based regimens in long term treatment or as part of a male contraceptive. Administration to eugonadal men results in suppression of gonadotropins, but its ability to support androgen-dependent behavior has not been investigated. For sustained release administration, MENT acetate was used, because its diffusion characteristics were more suitable for use in implants. However, upon release the acetate is rapidly hydrolyzed, and MENT is the biologically active moiety in circulation. We studied the effects of MENT on sexual interest and activity, spontaneous erection, and mood states in comparison with testosterone enanthate (TE) in 20 Caucasian and Chinese hypogonadal men recruited in Edinburgh and Hong Kong (n = 10 in each center). Outcomes were measured using a combination of daily diaries, semistructured interviews, and questionnaires. Nocturnal penile tumescence (NPT) was also recorded in the Edinburgh group. After withdrawal of androgen replacement treatment (wash-out phase) for a minimum of 6 weeks, subjects were randomized to two groups in a cross-over design. Drug treatment regimens were of 6-week duration and consisted of two implants, each containing 115 mg MENT acetate, inserted sc into the upper arm and removed after 6 weeks and two injections of TE (200 mg, im) 3 weeks apart. MENT treatment resulted in stable plasma MENT concentrations of 1.4 ± 0.1 nmol/L after 3 weeks and 1.3 ± 0.1 nmol/L after 6 weeks (mean ± sem; all men). Nadir testosterone concentrations were 3.6 ± 0.6 nmol/L at the end of the wash-out phase and 9.4 ± 0.6 nmol/L 3 weeks after each injection. There were no differences in hormone concentrations between centers. There were no adverse toxicological effects. There were only minor differences between the two treatments. Both MENT and TE treatment resulted in significant increases in sexual interest and activity, spontaneous erection (both by self-report and NPT measurement), and increases in positive moods, with decreases in negative moods in the Edinburgh group. In the Hong Kong group, both treatments increased waking erection, with a trend toward increased sexual interest and activity. Mood states appeared to be less affected during the wash-out phase than in Edinburgh men and showed no significant response to either treatment. These results demonstrate that MENT has similar effects on sexual activity and mood states as testosterone in hypogonadal men. As NPT is a physiological androgen-dependant outcome, these data provide further evidence for the androgenicity of MENT. The lack of detected effect of either androgen in Hong Kong men other than on waking erection illustrates the importance of the cultural context of symptomatology and its measurement. The appropriate dose of MENT remains to be determined, but these results support its development as a potential androgen replacement therapy.

Introduction: Erectile dysfunction is a significant public health problem. It is estimated that more than 50% of men between the ages of 40 and 70 suffer from the same or intermittent illness. Case report: A 45 years old patient was admitted to the family medicine ambulance for erection problems over the previous 6 months He complained on the ubiquitous absence of morning erection, nocturnal erection and spontaneous erection satisfactory for sexual intercourse. Physical examination reveals an enlarged thyroid gland. Ultrasound detected a non-homogeneous, partially calcified, clearly circumscribed nodule 24 x 30 x 22 mm in the right lobe of the thyroid gland. A diagnosis of hypothyroidism was based on thyroid function test. The patient was referred to a nuclear medicine specialist for Fine needle aspiration biopsy. The cytological finding of the bioptate reveals follicular accumulations of macronuclear thyrocytes of hyperchromatic nuclei (TBSRTC IV follicular tumor) and right-sided lobectomy was performed. Levothyroxine therapy was initiated and six months after that erectile function was normalised. Conclusion: Early detection and adequate treatment of hypothyroidism in patients with erectile dysfunction could result in significant improvement in erectile function.
