The Fukushima nuclear accident in March 2011 posed major threats to public health. In response, medical professionals have tried to communicate the risks to residents. To investigate forms of risk communication and to share lessons learned, we reviewed medical professionals’ activities in Fukushima Prefecture from the prefectural level to the individual level: public communication through Fukushima Health Management Surveys, a Yorozu (“general”) health consultation project, communications of radiological conditions and health promotion in Iitate and Kawauchi villages, dialogues based on whole-body counter, and science communications through online media. The activities generally started with radiation risks, mainly through group-based discussions, but gradually shifted to face-to-face communications to address comprehensive health risks to individuals and well-being. The activities were intended to support residents’ decisions and to promote public health in a participatory manner. This article highlights the need for a systematic evaluation of ongoing risk communication practices, and a wider application of successful approaches for Fukushima recovery and for better preparedness for future disasters.

Objective The aim of the present study was to develop a positive organisational scholarship in health care approach to health management, informed by health managers and health professionals’ experiences of brilliance in health care delivery. Methods A sample of postgraduate students with professional and/or management experience within a health service was invited to share their experiences of brilliant health services via online discussions and a survey running on the SurveyMonkey platform. A lexical analysis of student contributions was conducted using the individual as the unit of analysis. Results Using lexical analysis, the examination of themes in the concept map, the relationships between themes and the relationships between concepts identified ‘care’ as the most important concept in recognising brilliance in health care, followed by the concepts of ‘staff’ and ‘patient’. Conclusions The research presents empirical material to support the emergence of an evidence-based health professional perspective of brilliance in health management. The findings support other studies that have drawn on both quantitative and qualitative materials to explore brilliance in health care. Pockets of brilliance have been previously identified as catalysts for changing health care systems. Both quality, seen as driven from the outside, and excellence, driven from within individuals, are necessary to produce brilliance. What is known about the topic? The quest for brilliance in health care is not easy but essential to reinvigorating and energising health professionals to pursue the highest possible standards of health care delivery. What does this paper add? Using an innovative methodology, the present study identified the key drivers that health care professionals believe are vital to moving in the direction of identifying brilliant performance. What are the implications for practitioners? This work presents evidence on the perceptions of leadership and management practices associated with brilliant health management. Lessons learned from exceptionally well-delivered services contain different templates for change than those dealing with failures, errors, misconduct and the resulting negativity.

This report summarizes baseline and midterm findings of an intervention pilot study conducted by Horizons, LINKAGES, National Food and Nutrition Commission, District Health Management Team, Hope Humana, and the Zambia Integrated Health Program in Ndola District, Zambia. The aim of the research is to investigate how integrating services for the prevention of mother-to-child transmission (PMCT) of HIV into low-resource maternal and child health (MCH) and antenatal care (ANC) settings influences women’s ability to make and implement informed decisions about HIV. The intervention strengthened routine services and introduced HIV voluntary counseling and testing and PMCT counseling and services into the MCH/ANC setting. Exclusive breastfeeding was promoted for all mothers with infants below six months of age.

Surveillance for possible international infectious disease threats to the Olympic and Paralympic Games in London, United Kingdom, was conducted from 2 July to 12 September 2012 by a collaborative team comprising representatives from the Health Protection Agency (Public Health England since April 2013), the European Centre for Disease Prevention and Control and the National Travel Health Network and Centre. Team members enhanced their usual international surveillance activities and undertook joint risk assessments of incidents identified as relevant through an agreed set of criteria designed for the Games and using tools developed for this purpose. Although team members responded to a range of international disease incidents as part of their routine roles during this period, no incident was identified that represented a threat to the Games. Six incidents were highlighted by the team that were likely to attract media attention and hence could generate political and public concern. Responding to such concern is an important aspect of the overall public health management of mass gathering events. The lessons learned about the process and outcomes of the enhanced international surveillance will help inform planning by future hosts of similar events.

Purpose EC-135P2+ helicopters operated by Polish Medical Air Rescue are highly exposed to environmental particles entering engines when performing helicopter emergency medical services. This paper aims to assess the effectiveness of inlet barrier filters installed to protect the engines, including their impact on maintenance. Design/methodology/approach The organisation adopted a comprehensive set of measures to predict and limit the impact of dust ingestion including visual inspections, health management and engine trend monitoring based on ground power checks’ (GPC) results. Three alternative particle separation solutions were considered. Finally, helicopter inlets were modified to allow the selected filter system to be installed, which reduced the number of particles ingested by the engine and prevented from premature overhauls. Findings The analyses carried out enabled not only the selection of the optimal filtration solution and its seamless implementation into the fleet but also confirmed its efficiency. After installing the filters, engines’ lifetime is extended from 500 to 4,500 flight hours while operating costs and the number of maintenance tasks was reduced significantly. Originality/value Lessons learned from operational experience show that a well-matched particle separation system can mitigate accelerated engine deterioration even if the platform is continuously exposed to environmental particles. The remaining useful life of engines can be predicted using performance models and data from GPC.

Abstract Background Measles continues to be a threat to Australia. While post eradication risks are low, imported measles cases from overseas travellers who are non-immune can cause small outbreaks. This case report discusses the challenge of identifying wild-type measles in an individual who was recently vaccinated with measles - containing vaccine (MCV). Methods A positive polymerase chain reaction (PCR) result for measles for an adult who had recently received a measles -containing vaccine was notified. Investigation revealed no known epidemiological link, recent overseas travel or contact with recent measles cases during the incubation period. Results The results of the initial sequencing to distinguish between wild-type and vaccine-strain measles were inconclusive. A decision was made to re-run the genotyping, collect additional specimens, and quarantine the case until a definitive result was obtained. Sequencing and genotyping revealed that this indeed was a wild-type measles strain. Conclusions Changing epidemiology of measles means distinguishing between wild-type and vaccine-strain measles has become a new challenge. The reflection of the public health management of this case has provided is a valuable teaching tool for public health professionals globally, particularly in low incidence measles countries. Key messages The lack of an epidemiological link can create confusion for public health staff when investigating possible measles cases. Changes in the epidemiology of measles means distinguishing between wild type and vaccine strain measles is a challenge.

In the present study, we provide a retrospective genomic epidemiology analysis of the SARS-CoV-2 pandemic in the state of Rio de Janeiro, Brazil. We gathered publicly available data from GISAD and sequenced more 1,927 new genomes sampled periodically from March 2021 to June 2021 from 91 out of the 92 cities of the state. Our results showed that the pandemic was characterized by three different phases driven by a successive replacement of lineages. All stages occurred in distinct mortality and mobility contexts, with higher evidence of social distancing measures being observed in early pandemic and relaxed in the last two phases. Interestingly, we noticed that viral supercarriers accounted for the overwhelming majority of the circulating virus (> 90%) among symptomatic individuals in the state. Moreover, SARS-CoV-2 genomic surveillance also revealed the emergence and spread of two new variants (P.5 and P.1.2) firstly reported in this study. Altogether, our findings provided important lessons learned from the different epidemiological aspects of the SARS-CoV-2 dynamic in the state of Rio de Janeiro that have a strong potential to shape future decisions aiming to improve public health management and understanding mechanisms underlying virus dispersion.

Relatar as trajetórias percorridas, os aprendizados e as dificuldades da gestão de enfermagem durante a pandemia da COVID-19. Relato de experiência realizado através de estudos e discussões entre profissionais de enfermagem durante o período de março a outubro de 2020. Verificamos que o desenvolvimento e a aplicabilidade de protocolos para fluxo de atendimento foram fundamentais para que não houvesse transmissão cruzada, além de otimizar o atendimento dos casos suspeitos ou confirmados de COVID-19, o controle e gerenciamento dos equipamentos de proteção individual se fez necessário para que não houvesse falta. O apoio da equipe de enfermagem e o suporte psicológico oferecido aos profissionais foram fundamentais para uma assistência segura e de qualidade.
