The objective of the research was to develop an advanced hazard management information system in air transportation system, which was a supportive tool for the implementation of safety management system and safety risk management. Firstly, hazard management process was outlined taking account of the regulator, the air operator/air navigation service provider/airdrome and their responsibilities, the process clearly showed the necessity of such a system. The system fulfills five basic functions: hazard log management, inspection service, alerting service, statistics service and system management, it matches the needs of daily hazard management. Furthermore, architecture of the system and key algorithms was discussed in the paper. By using web technology, i.e. JSP and MySQL, the hazard management information system was developed based on B/S structure. Application of the system will definitely facilitate safety risk management and make safety management more proactive and efficient.

In modern day’s institutions, risk management plays a crucial role as it aims to minimize the likelihood of adverse events and contributes to improve the quality of services delivery. In health care, an effective risk management is only possible if supported by information systems that can produce high quality measures and meaningful risk indicators. These indicators will then allow the healthcare organization to self-assess by identifying critical gaps and opportunities for improvement in several frontiers. Such an organizational thrust is not only warranted for competitiveness but also fundamental for the purpose of benchmarking, accreditation and certification. Additionally, monitoring of specific indicators is often required by the tutelage. However, the development of a risk management system can be an arduous process due to the inherent complexity of clinical systems. This paper presents an architecture for the implementation of a risk management information system, using as example the case of CHTMAD, a Portuguese hospital center.

The role of information technology in an organization is growing so fast. Vehicle Testing of Management Information System (VTMIS) is a system that uses information technology to serve users in motor vehicle administration in the Banyumas transportation service area. VTMIS at the Banyumas Transportation Department is an integrated information system for the motor vehicle testing process starting from the registration process, levy payment, and vehicle testing. The purpose of this study is to analyze the risk management at VTMIS in Banyumas Transportation Department using Control Objective for Information and Related Technology (COBIT 4.1) domain Plan and Organize (PO) 9. COBIT 4.1 is a framework for analyzing and ensuring information technology aligns with business management by calculating maturity levels. The data analysis results from the PO9 domain show that VTMIS risk management has a maturity level of 3.46. The maturity level of VTMIS at the Banyumas Transportation Department is at level 3, namely defined, meaning that procedures are in a position of standardization, documentation, and individual communication. The recommendation for VTMIS at the Banyumas Transportation Department needs to carry out risk management in a structured, massive and integrated manner
