Purpose The primary aim of the current article is to provide a brief review of the literature regarding the effects of noise exposure on the vestibular and balance control systems. Although the deleterious effects of noise on the auditory system are widely known and continue to be an active area of research, much less is known regarding the effects of noise on the peripheral vestibular system. Audiologists with working knowledge of how both systems interact and overlap are better prepared to provide comprehensive care to more patients as assessment of both the auditory and vestibular systems has been in the audiologists' scope of practice since 1992. Method A narrative review summarizes salient findings from the archival literature. Results Temporary and permanent effects on vestibular system function have been documented in multiple studies. Hearing conservation, vestibular impairment, and fall risk reduction may be more intimately related than previously considered. Conclusions A full appreciation of both the vestibular and auditory systems is necessary to address the growing and aging noise-exposed population. More cross-system studies are needed to further define the complex relationship between the auditory and vestibular systems to improve comprehensive patient care.

AbstractComplex systems are challenging to control because the system responds to the controller in a nonlinear fashion, often incorporating feedback mechanisms. Interdependence of systems poses additional difficulties, as cross-system connections enable malicious activity to spread between layers, increasing systemic risk. In this paper we explore the conditions for an optimal control of cascading failures in a system of interdependent networks. Specifically, we study the Bak–Tang–Wiesenfeld sandpile model incorporating a control mechanism, which affects the frequency of cascades occurring in individual layers. This modification allows us to explore sandpile-like dynamics near the critical state, with supercritical region corresponding to infrequent large cascades and subcritical zone being characterized by frequent small avalanches. Topological coupling between networks introduces dependence of control settings adopted in respective layers, causing the control strategy of a given layer to be influenced by choices made in other connected networks. We find that the optimal control strategy for a layer operating in a supercritical regime is to be coupled to a layer operating in a subcritical zone, since such condition corresponds to reduced probability of inflicted avalanches. However this condition describes a parasitic relation, in which only one layer benefits. Second optimal configuration is a mutualistic one, where both layers adopt the same control strategy. Our results provide valuable insights into dynamics of cascading failures and and its control in interdependent complex systems.

According to the business characteristics and technical requirements of heavy military vehicles digital collaborative development, the paper proposes the dynamic federal collaborative development method for heavy military vehicles. To realize the method, the key is constructing the three-tier dynamic federal collaborative development environment. So the paper focuses on the research of the business, function and technical architecture of dynamic federal collaborative development environment. Applying the Web Service-Based cross-system collaborative flow controlling method and developing the integrated middleware, the three-tier federal collaborative environment realizes tightly integration. Finally, the application demonstration of a type-product inter-enterprise collaborative development based on the developed digital collaborative development platform verifies the effectiveness and feasibility of the method.

BACKGROUND Rapid implementation of telemedicine for cancer care during COVID-19 required innovative and adaptive solutions among healthcare workers. OBJECTIVE The objective of this qualitative study was to explore healthcare workers’ experiences with telemedicine implementation during COVID-19. METHODS We conducted semi-structured interviews with 40 oncology healthcare workers who implemented telemedicine during COVID-19. The interviews were recorded, transcribed verbatim, and analyzed for themes using Dedoose software (Version 4.12). RESULTS Approximately half of participants were physicians (55%) and one quarter of participants were APPs (25%). Other participants included social workers (n=3), psychologists (n=2), dieticians (n=2), and a pharmacist. Five key themes were identified: 1) establishing and maintaining patient-provider relationships, 2) coordinating care with other providers and informal caregivers, 3) adapting in-person assessments for telemedicine, 4) developing workflows and allocating resources, and 5) future recommendations. Participants described innovative strategies for implementing telemedicine, such as coordinating inter-disciplinary visits with multiple providers. Healthcare workers discussed key challenges, such as workflow integration, lack of physical exam and biometric data, and overcoming the digital divide. Participants recommended policy advocacy to support telemedicine (e.g., medical licensure policies) and monitoring how telemedicine affects patient outcomes and healthcare delivery. CONCLUSIONS To support the growth of telemedicine, implementation strategies are needed to ensure providers and patients have the tools necessary to effectively engage in telemedicine. At the same time, cancer care organizations will need to engage in advocacy to ensure policies are supportive of oncology telemedicine and develop systems to monitor the impact of telemedicine on patient outcomes, healthcare quality, costs, and equity.
