We report a case of unilateral hydronephrosis following urethral catheterization in a patient with T6 complete paraplegia at the Physical Medicine and Rehabilitation Department in a tertiary care teaching hospital, India. Diagnosis was established by an abdominal ultrasound. The misplaced catheter tip was withdrawn from the ureteric orifice and hydronephrosis was resolved. Foley's catheterization, a widely practiced clinical procedure, is not without its attendant risks of an inadvertent placement in the ureter leading to transient hydronephrosis. Inadequate drainage through a catheter should thus alert one to this potentially hazardous complication that can be diagnosed by an early ultrasound. This complication can be avoided by gently tugging on the catheter after inflating the catheter bulb.

ABSTRACT Pharmaceutical procurement is a complex process that involves many steps, agencies, and manufacturers. Procurement lead time is defined as the time required to acquire the supplies and services and then placing them in the hands of the user. This study was done to study the lead time in drugs procurement at Government Medical College and Hospital, Chandigarh. It was aimed at calculating the average lead time in the procurement of drugs for year 2009—2010 in Pharmacy Department. Observational study along with retrospective record analysis was done to calculate the average internal and external lead time. Standardized format was used to retrieve data from the procurement records. Average lead time between the preparation of indent and receipt of drugs was observed to be 162 days, out of which 117 days (71%) accounted for internal lead time and external lead time was 47 days (29%). Internal lead time was approximately four times that of external lead time. Various reasons were noted for longer duration of internal lead time and every effort should be made to bring it down to the minimum, so as to ensure the availability of the right medicine in the right quantity, in the right time at the right place. Background: Pharmacovigilance is proven as an effective monitoring mechanism for safety and efficacy of pharmaceutical products with the assistance of physicians, pharmacists, nurses, and other healthcare professionals to avoid undue physical, mental and financial suffering of patients. Thus, this study was conducted to assess awareness of pharmacovigilance among the healthcare service providers to evaluate the acquaintance, approach application (3A) of judicial reporting of ADRs and pharmacovigilance in a tertiary care teaching hospital in Dhaka.Methods: A pre-tested questionnaire-based study was done among the 5th year medical students, interns, doctors and nurses of Holy Family Red Crescent Medical College, Dhaka, Bangladesh to assess the overall status of acquaintance (knowledge), approach (attitude) and application (practice) pharmacovigilance. Total 417 questionnaires were distributed and 389 were included as valid, compiled and analysed using SPSS version 25.0.Results: Among the respondents, almost 78% of the nurses responded the right answers and doctors responded the lowest 29% on average. The average percentage of approach and application of pharmacovigilance was low in all the respondent groups. The poorest outcome was observed about reporting an adverse drug reactions (ADR) form by all respondents as 01% to 08%.Conclusions: The overall status of pharmacovigilance in a tertiary teaching hospital was found paradoxically low, that revealed the necessity of much more initiatives at the undergraduate and postgraduate academic curriculum and intensive motivation, training, monitoring should be addressed to ensure the safety of medication, rationality of drug use and accomplish the national pharmacovigilance programs.
