Background:Educating trainees during cardiology fellowship is a process in constant evolution, with program directors regularly adapting to increasing demands and regulations as they strive to prepare graduates for practice in today’s healthcare environment.Methods and Results:In a 10-year follow-up to a previous manuscript regarding fellowship education, we reviewed the literature regarding the most topical issues facing training programs in 2010, describing our approach at The Ohio State University.Conclusion:In the midst of challenges posed by the increasing complexity of training requirements and documentation, work hour restrictions, and the new definitions of quality and safety, we propose methods of curricula revision and collaboration that may serve as an example to other medical centers.

Small- and medium-sized family farms are places to live and sources of income for about half of the population. The aim of this analysis was to determine the relationship between eco-efficiency and human capital efficiency on small- and medium-sized family farms. The analysis was carried out using an economic measure (value of agricultural production per work hour calculated per hectare) and two synthetic measures (human capital and environmental measures). The synthetic measures were determined using the TOPSIS-CRITIC method by defining weights for variables used in the measures. The analysis covered five countries: Lithuania (960 farms), Moldavia (532 farms), Poland (696 farms), Romania (872 farms) and Serbia (524 farms). All of these countries are characterised by a high fragmentation of agricultural holdings. The analysis allowed us to formulate the following conclusions: eco-efficiency and human capital efficiency indices increased with area for small- and medium-sized family farms. An increase in the eco-efficiency index with an increase in farm area suggests that the smaller the farm area, the more extensive the agricultural production that was carried out. In addition, an increase in human capital efficiency with an increase in farm area indicates that there was inefficiency in the utilisation of human capital resources on the agricultural farms studied.

Objective To create a high-quality annotated online surgical video atlas of key indicator otolaryngology cases and assess its use and overall journal trends over time. Methods Videos are recorded from multiple viewpoints within the operating room and compiled into a single stream. Postediting includes chaptering videos and overlaying relevant text annotations. Videos are published online and viewership trends analyzed. Results Over 3 years, 29 otolaryngology videos were published out of 161 journal publications (18%). Eight of the 14 key indicator procedures are included (57%). From the beginning of 2017 to the end of 2019, viewership of otolaryngology pages increased from 548 to 11,139 views per month, totaling >150,000 views. These now represent 10% of the total journal monthly views and 10% of the overall views. Users originate from the United States and from >10 other countries. Discussion Residents and faculty face challenges of providing the highest standard of clinical care, teaching, and learning in and out of the operating room. Inherent difficulties of surgical training, high-fidelity surgical simulation, and imposed work hour restrictions necessitate additional, more efficient and effective means of teaching and learning. Surgical videos demonstrating key anatomy, procedural steps, and surgical dexterity with hand positioning are increasing in their popularity among learners. Implications for Practice Surgical video atlases provide a unique adjunct for resident education. They are enduring and easily accessible. In a climate of work hour restrictions or elective case reduction, they may supplement how residents learn to operate outside the operating theater.

Precarious work, which has become more prevalent in the United States in recent decades, is disproportionately experienced by workers of lower socio-economic classes, and research suggests that the erosion of worker power has contributed to this class polarization in precarity. One dimension of precarious work of growing interest to scholars and policymakers is instability faced by workers in the amount and regularity of their work hours. However, we know little about the magnitude of month-to-month or week-to-week (intra-year) volatility in hours worked, the extent of class-based polarization in this measure of job quality, and whether worker power moderates this polarization. In this paper, we make novel use of the panel nature of the nationally-representative Current Population Survey (CPS) to estimate intra-year volatility in the actual hours respondents report working in the previous week across four consecutive survey months. Using this new measure, we then show that, net of demographic characteristics and controls for occupation and industry, low-wage workers experience disproportionately greater work hour volatility. Finally, we find evidence that reductions in marketplace bargaining power--as measured by higher state-level unemployment rates--increase wage- and education-based polarization in work hour volatility, while increases in associational power--as measured by union coverage--reduce wage-based polarization in work hour volatility.
