Abstract Background There is wide variation in clinical practice for the early detection of prostate cancer, not least because of the ongoing debate about the benefits of prostate-specific antigen (PSA) testing. In this study, we aimed to assess the approaches, attitudes, and knowledge of general practitioners (GPs) regarding PSA testing in primary care in the Netherlands, particularly regarding recommendations for prostate cancer. Methods Questionnaire surveys were sent to 179 GPs in the north-east of the Netherlands, of which 65 (36%) were completed and returned. We also surveyed 23 GPs attending a postgraduate train-the-trainer day (100%). In addition to demographic data and practice characteristics, the 31-item questionnaire covered the attitudes, clinical practice, adherence to PSA screening recommendations, and knowledge concerning the recommendations for prostate cancer early detection. Statistical analysis was limited to the descriptive level. Results Most GPs (95%; n = 82) stated that they had at least read the Dutch GP guideline, but just half (50%; n = 43) also stated that they knew the content. Almost half (46%; n = 39) stated they would offer detailed counseling before ordering a PSA test to an asymptomatic man requesting a test. Overall, prostate cancer screening was reported to be of minor importance compared to other types of cancer screening. Conclusions Clinical PSA testing in primary care in this region of the Netherlands seems generally to be consistent with the relevant guideline for Dutch GPs that is restrictive to PSA testing. The next step will be to further evaluate the effects of the several PSA testing strategies.

It would be a brave health promotion coordinator who would suggest to Primary Care Partnership health promotion sub-committee members that just providing "information" to "people who are problems" is a "best-practice" approach (Legge et aI., 1996). One, and only one, of the many reasons for the change has been the creation and dissemination of the Men's Health Promotion Strategic Framework (Hayes, 1999). This framework was developed through a project funded by the Victorian Health Promotion Foundation and commissioned by the North East Health Promotion Centre (NEHPC), Melbourne.

To objective of this project was to reduce unplanned hospital admission rates in children related to asthma to the Newcastle upon Tyne Hospitals National Health Service Trust (NUTH).Multiple educational interventions were introduced both locally and regionally including: a collection of educational materials aimed at young people and families, schools, primary care and secondary care on the website www.beatasthma.co.uk; regional training days; a nurse-led one-stop clinic; a new pathway following an acute attendance to hospital with an asthma attack; a local asthma service and cascade training for schools.The primary outcome measure was reduction in unplanned hospital admission rates in children due to asthma to the NUTH.Results showed that admission rates had been increasing at a sustained rate of approximately 30% each year in the 3 years prior to our intervention. After the Beating Regional Asthma Through Health Education interventions, unplanned admissions to NUTH reduced by 29% and this reduction has been sustained for the last 3 years. This compares with a regional increase of 10% over the same time period.In conclusion, simple but effective educational interventions resulted in a significant and sustained reduction in unplanned asthma admissions to NUTH. Further work is underway to extend the reach of these interventions into primary care and schools.

The Impact

The aim of this study was to explore the reactions of children aged 10–11 years towards being weighed and measured and subsequently told their correct weight as part of the National Child Measurement Programme (NCMP). The study was undertaken in primary schools in a Primary Care Trust (PCT) in the North East of England, UK. One-to-one semi-structured interviews were conducted post-NCMP measurement (and the feedback) with a sample of 21 children, aged 10–11 years, conveniently sampled from the primary schools in the PCT. Findings from the study indicate that the NCMP, especially the weight feedback letter caused annoyance, panic and worry among children who were indicated to have weight problems and oversensitised all children about weight issues, regardless of their weight category. The NCMP is a potentially useful ‘wake-up call’ to the fact that something needs to be done for children identified to be overweight or obese but needs to be delivered with a sensitivity to the impact of the news on the child.

BackgroundPeople with dementia and their families need support in different forms, but currently services are often fragmented with variable quality of care. Case management offers a way of co-ordinating services along the care pathway and therefore could provide individualised support; however, evidence of the effectiveness of case management for dementia is inconclusive.ObjectiveTo adapt the intervention used in a promising case management project in the USA and test its feasibility and acceptability in English general practice.DesignIn work package 1, a design group of varied professionals, with a carer and staff from the voluntary sector, met six times over a year to identify the skills and personal characteristics required for case management; protocols from the US study were adapted for use in the UK. The feasibility of recruiting general practices and patient–carer dyads and of delivering case management were tested in a pilot study (work package 2). An embedded qualitative study explored stakeholder views on study procedures and case management.SettingFour general practices, two in the north-east of England (Newcastle) one in London and one in Norfolk, took part in a feasibility pilot study of case management.ParticipantsCommunity-dwelling people with dementia and their carers who were not already being case managed by other services.InterventionA social worker shared by the two practices in the north-east and practice nurses in the other two practices were trained to deliver case management. We aimed to recruit 11 people with dementia from each practice who were not already being case managed.Main outcome measuresNumbers of people with dementia and their carers recruited, numbers and content of contacts, needs identified and perceptions of case management among stakeholders.ResultsRecruitment of practices and patients was slow and none of the practices achieved its recruitment target. It took more than 6 months to recruit a total of 28 people with dementia. Practice Quality and Outcome Framework registers for dementia contained only 60% of the expected number of people, most living in care homes. All stakeholders were positive about the potential of case management; however, only one of the four practices achieved a level of case management activity that might have influenced patient and carer outcomes. Case managers’ activity levels were not related solely to time available for case management. Delivery of case management was hindered by limited clarity about the role, poor integration with existing services and a lack of embeddedness within primary care. There were discrepancies between case manager and researcher judgements about need, and evidence of a high threshold for acting on unmet need. The practice nurses experienced difficulties in ring-fencing case management time.ConclusionsThe model of case management developed and evaluated in this feasibility study is unlikely to be sustainable in general practice under current conditions and in our view it would not be appropriate to attempt a definitive trial of this model. This study could inform the development of a case management role with a greater likelihood of impact. Different approaches to recruiting and training case managers, and identifying people with dementia who might benefit from case management, are needed, as is exploration of the scale of need for this type of working.Trial registrationCurrent Controlled Trials ISRCTN74015152.FundingThis project was funded by the National Institute for Health Research (NIHR) Health Technology Assessment programme and will be published in full inHealth Technology Assessment; Vol. 18, No. 52. ObjectiveTo explore the attitudes towards, and perceptions of, primary care healthcare staff and patients, regarding the role of clinical pharmacists in the provision of oral health advice and collaboration with dentists in general practice.DesignInterpretivist methodology using qualitative semi-structured interviews and focus groups.Participants22 participants; 10 pharmacists; 3 general practitioners; 2 nurses; 1 practice manager; 6 patients.SettingPrimary care general medical practices in the North East of England and the University of Sunderland Patient Carer and Public Involvement group.MethodsOne-to-one semi-structured interviews were performed with primary care healthcare staff. An iterative approach using constant comparative analysis facilitated the ongoing enrichment of data; salient themes were identified using Framework Analysis and related back to extant literature. A focus group was held with patients to further explore key themes.ResultsFour salient and inter-related themes emerged: enhanced clinical roles; indicating rapidly changing roles of pharmacists working in general practice, increased responsibility and accountability of pharmacist prescribers and the delivery of advanced clinical services; limited knowledge; indicating basic understanding of appropriate oral health advice, but limited insight and provision of advice to patients with regards to links with systemic diseases and medication; geographical/situational isolation of the dental team; indicating the disparate contexts and challenges of multidisciplinary working in oral health, and patients’ attitudes towards dental care; integration of oral health advice; indicating the potential of pharmacists to integrate oral health advice into current roles and to target specific patient groups in practice.ConclusionsThe lack of integration between oral and general healthcare services potentially impacts negatively on patient care, requiring further interprofessional oral health education. The developing role of the pharmacist in general practice represents an opportunity to integrate oral health advice and/or interventions into the management of patients in this setting.

This paper aims to present information about the provision of gingival prostheses or gingival veneers. These appliances are most commonly provided for patients with gingival recession and attachment loss. The paper will highlight the indications and advantages for providing prostheses, and inform the reader of the risks or complications that may arise. The clinical and laboratory stages will be explained and advice provided on maintenance. A case series will demonstrate some of the patient-related benefits and difficulties that can be encountered. A final aspect to the paper will present the results of a survey undertaken in the north-east of England, exploring the provision of these appliances in primary care. CPD/Clinical Relevance: Good aesthetics relating to the gingival margin can be difficult to achieve and the use of gingival veneers may provide a solution.

Background: Depression is a common and serious disorder that impairs quality of life. Since general practitioners (GP) are considered gatekeepers to secondary care, the choice of interventions offered in primary care can have a significant impact not only on patients’ quality of life, but also on health service demands. Objective: To evaluate the confidence of GPs in diagnosing and managing depression; and, to assess the factors influencing their strategy in treating depression. Methods: A cross-sectional study was carried out among GPs working in the North-East of England, UK. The survey questionnaire consisted of mostly close-ended questions with some allowing for free-text comments (see Appendix 1). The responses obtained were analysed using Microsoft Excel. Results: Among the total of 63 respondents, most GPs were comfortable diagnosing depression. Most would consider combining talking therapies with antidepressants (68.3%) at presentation, followed by referral to talking therapies alone (41.2%). In only 14.3% of cases would antidepressant therapy alone be considered. For those patients non-responsive to initial treatment, 25.4% considered offering a different antidepressant or adjunct medication (such as a sedative, anxiolytic, or beta-blocker), and another 25.4% of GPs considered a combination with another antidepressant. 46.0% of participants were not comfortable prescribing dual antidepressants due to concerns about side effects, lack of experience, paucity of guidelines, and lack of timely access and guidance from the local mental team. Nearly all (98.4%) GP participants would agree to prescribe dual antidepressants on advice of the mental health team with telephone advice being the preferred means of communication in 65.1% of cases. Conclusion: The results of this study can help to develop closer co-operation between primary and secondary care by not only upskilling GPs through various means (educational events, training posts, etc.), but by also creating better communication channels at the interface between those two services.
