Abstract Background.  Herpes zoster, or shingles, is a common, painful reactivation of latent varicella zoster virus infection. Understanding host factors that predispose to herpes zoster may permit development of more effective prevention strategies. Our objective was to examine mitochondrial haplogroups as a potential host factor related to herpes zoster incidence. Methods.  Study participants were drawn from BioVU, a deoxyribonucleic acid (DNA) biobank connected to deidentified electronic medical records (EMRs) from Vanderbilt University Medical Center. Our study used 9691 Caucasian individuals with herpes zoster status determined by International Classification of Diseases, Ninth Revision codes 053–053.9. Cases and controls were matched on sex and date of birth within 5 years. Mitochondrial haplogroups were defined from mitochondrial DNA variants genotyped on the Illumina 660W or Illumina Infinium Human-Exome Beadchip. Sex and date of birth were extracted from the EMR. Results.  European mitochondrial haplogroup H had a protective association with herpes zoster status (odds ratio [OR] = .82; 95% confidence interval [CI], .71–.94; P = .005), whereas haplogroup clade IWX was a risk factor for herpes zoster status (OR = 1.38; 95% CI, 1.07–1.77; P = .01). Conclusions.  Mitochondrial haplogroup influences herpes zoster risk. Knowledge of a patient's mitochondrial haplogroup could allow for a precision approach to the management of herpes zoster risk through vaccination strategies and management of other modifiable risk factors.

Background. The limited donor pool and increasing recipient wait list require a reevaluation of kidney organ suitability for transplantation. Use of higher infectious risk organs that were previously discarded may help improve access to transplantation and reduce patient mortality without placing patients at a higher risk of poor posttransplant outcomes. There is very little data available regarding the safe use of kidney organs from deceased donors with varicella zoster virus infection at the time of organ retrieval. Case Presentation. Here, we report a case of successful transplantation of both kidneys from a deceased donor with active herpes zoster infection at the time of organ retrieval. Recipients were treated preemptively with acyclovir. At 4 months posttransplant, both kidney recipients experienced no infectious complications and were off dialysis with functioning transplant grafts. Conclusions. The use of kidney organs from donors with active herpes zoster infection appears to be a safe option to expand the kidney donor pool.

Subarachnoid hemorrhage (SAH) is a critical illness that may result in patient mortality or morbidity. In this study, we investigated the outcomes of patients treated in medical center and nonmedical center hospitals and the relationship between such outcomes and hospital and surgeon volume. Patient data were abstracted from the National Health Insurance Research Database of Taiwan in the Longitudinal Health Insurance Database 2000, which contains all claims data of 1 million beneficiaries randomly selected in 2000. The International Classification of Diseases, Ninth Revision, subarachnoid hemorrhage (430) was used for the inclusion criteria. We identified 355 patients between 11 and 87 years of age who had subarachnoid hemorrhage. Among them, 32.4% (115/355) were men. The median Charlson comorbidity index (CCI) score was 1.3 (SD ± 0.6). Unadjusted logistic regression analysis demonstrated that low mortality was associated with high hospital volume (OR = 3.21; 95% CI: 1.18–8.77). In this study, we found no statistical significances of mortality, LOS, and total charges between medical centers and nonmedical center hospitals. Patient mortality was associated with hospital volume. Nonmedical center hospitals could achieve resource use and outcomes similar to those of medical centers with sufficient volume.

Background: Although fingertip injuries are common, there is limited literature on the epidemiology and hospital charges for fingertip injuries in children. This descriptive study reports the clinical features of and hospital charges for fingertip injuries in a large pediatric population treated at a tertiary medical center. Methods: Our hospital database was queried using International Classification of Diseases, Revision 9 (ICD-9) codes, and medical records were reviewed. Frequency statistics were generated for 1807 patients with fingertip injuries who presented to the emergency department (ED) at Boston Children’s Hospital (BCH) between 2005 and 2011. Billing records were analyzed for financial data. Results: A total of 1807 patients were identified for this study; 59% were male, and the mean age at time of injury was 8 years. Most commonly, injuries occurred when a finger was crushed (n = 831, 46%) in a door or window. Average length of stay in the ED was 3 hours 45 minutes, 25% of cases needed surgery, and, on average, patients had more than 1 follow-up appointment. About one-third of patients were referred from outside institutions. The average ED charge for fingertip injuries was $1195 in 2014, which would amount to about $320 430 each year (in 2014 dollars) for fingertip injuries presenting to BCH. Conclusion: Fingertip injuries in children are common and result in significant burden, yet are mostly preventable. Most injuries occur at home in a door or window. Although these patients generally heal well, fingertip injuries pose a health, time, and financial burden. Increased awareness and education may help to avoid these injuries.

Abstract The self-controlled tree-temporal scan statistic allows detection of potential vaccine- or drug-associated adverse events without prespecifying the specific events or postexposure risk intervals of concern. It thus opens a promising new avenue for safety studies. The method has been successfully used to evaluate the safety of 2 vaccines for adolescents and young adults, but its suitability to study vaccines for older adults had not been established. The present study applied the method to assess the safety of live attenuated herpes zoster vaccination during 2011–2017 in US adults aged ≥60 years, using claims data from Truven Health MarketScan Research Databases. Counts of International Classification of Diseases diagnosis codes recorded in emergency department or hospital settings were scanned for any statistically unusual clustering within a hierarchical tree structure of diagnoses and within 42 days after vaccination. Among 1.24 million vaccinations, 4 clusters were found: cellulitis on days 1–3, nonspecific erythematous condition on days 2–4, “other complications . . .” on days 1–3, and nonspecific allergy on days 1–6. These results are consistent with local injection-site reactions and other known, generally mild, vaccine-associated adverse events and a favorable safety profile. This method might be useful for assessing the safety of other vaccines for older adults.

Previous studies have suggested that mitochondrial metabolism and/or mitochondrial DNA (mtDNA) could be, in conjunction with other genetic or environmental factors, a risk factor for the development of multiple sclerosis (MS). One of these studies establishes that mitochondrial haplogroup JT is a risk factor for developing the disease, in particular the visual manifestations [optic neuritis (ON)]. Nevertheless, as distribution of these haplogroups varies between populations, the observed association may be due to a slanted sample with no physiopathological value. This hypothesis was checked with MS patients, originals from Basque country (this population has peculiar genetic characteristics) and from other Spanish regions. We concluded that such an association does not exist. By contrast, a decrease could be seen in the frequency of the JT haplogroup in the ON group and in the MS-Basque group. That trend could be a protective effect, which needs to be verified in further investigations.

Abstract Bortezomib has been used for patients with refractory and relapsed multiple myeloma, non-Hodgkin lymphoma and leukemia in recent years. It has several complications. Here we report the complication of varicella herpes zoster after using bortezomib, which has higher incidence in Chinese patients. Ten patients were treated with bortezomib in our hematology centre. Among them, seven patients had refractory and relapsed multiple myeloma, two patients had refractory lymphoma (one of T cell lymphoma and one of mantle cell lymphoma) and one patient had refractory acute myeloblastic leukemia. All the patients received bortezomib (1.3 mg/m2) on days 1, 4, 8, and 11 of a 3-week cycle. Patients of multiple myeloma received dexamethasone simultaneously. The combination with liposomal doxorubicin was used on the patients of lymphoma. The patient of acute myeloblastic leukemia received bortezomib in combination with Amsacrine. After one cycle, 6 patients with multiple myeloma responded to treatment (four near complete remission, two partly remission). Two patients with lymphoma reached PR. The patient with leukemia had no response. A remarkable observation in our treatment was the high incidence of varicella herpes zoster. Six out of ten patients developed varicella herpes zoster. Among the six patients, two patients had previous infection of the zoster virus before. Most of the patients had the varicella herpes zoster infection after the complete of one cycle of bortezomib and were disappeared within one month by the use of antiviral treatment. The incidence of varicella hepers zoster after bortezomib was around 13% as reported. But the higher incidence of varicella herpes zoster were observed in our patients. Six of ten patients (60%) developed varicella zoster virus infection during treatment. We considered that the Chinese patients may be more liable to varicella zoster virus infection. And further observations should be made in the future. Prophylactic antiviral medication might be used in predisposed patients who receive bortezomib.

AbstractBackgroundThe PheCode system was built upon the International Classification of Diseases, Ninth Revision, Clinical Modification (ICD-9-CM) for phenome-wide association studies (PheWAS) in the electronic health record (EHR).ObjectiveHere, we present our work on the development and evaluation of maps from ICD-10 and ICD-10-CM codes to PheCodes.MethodsWe mapped ICD-10 and ICD-10-CM codes to PheCodes using a number of methods and resources, such as concept relationships and explicit mappings from the Unified Medical Language System (UMLS), Observational Health Data Sciences and Informatics (OHDSI), Systematized Nomenclature of Medicine - Clinical Terms (SNOMED CT), and National Library of Medicine (NLM). We assessed the coverage of the maps in two databases: Vanderbilt University Medical Center (VUMC) using ICD-10-CM and the UK Biobank (UKBB) using ICD-10. We assessed the fidelity of the ICD-10-CM map in comparison to the gold-standard ICD-9-CM→PheCode map by investigating phenotype reproducibility and conducting a PheWAS.ResultsWe mapped >75% of ICD-10-CM and ICD-10 codes to PheCodes. Of the unique codes observed in the VUMC (ICD-10-CM) and UKBB (ICD-10) cohorts, >90% were mapped to PheCodes. We observed 70-75% reproducibility for chronic diseases and <10% for an acute disease. A PheWAS with a lipoprotein(a) (LPA) genetic variant, rs10455872, using the ICD-9-CM and ICD-10-CM maps replicated two genotype-phenotype associations with similar effect sizes: coronary atherosclerosis (ICD-9-CM: P < .001, OR = 1.60 vs. ICD-10-CM: P < .001, OR = 1.60) and with chronic ischemic heart disease (ICD-9-CM: P < .001, OR = 1.5 vs. ICD-10-CM: P < .001, OR = 1.47).ConclusionsThis study introduces the initial “beta” versions of ICD-10 and ICD-10-CM to PheCode maps that will enable researchers to leverage accumulated ICD-10 and ICD-10-CM data for high-throughput PheWAS in the EHR.

Herpes zoster represents a latent reactivation of the varicella zoster virus infection with a dermatomal pattern of eruption and complicated by post-herpetic neuralgia. Ramsay hunt syndrome is a rare complication of herpes zoster and it is not just a syndrome but it's rather an infectious disease. This syndrome is characterized by peripheral facial nerve palsy associated with an erythematous vesicular rash on the ear. It is characterized by unilateral pattern of facial involvement and presence of vesicles helps in early diagnosis and distinguish the syndrome with diseases mimicking other severe neurological diseases. This article reports a case of 56-year-old male patient who reported with a complaint of severe toothache, which serves as severe prodrome for reactivation of herpes zoster virus which later leads to Ramsay Hunt syndrome. This case report highlights about the management of herpes zoster and its complication and emphasizes on prevention of post herpetic neuralgia complication.

Abstract Background Headache, especially migraine, has been associated with various vestibular symptoms and several vestibular syndromes. In addition, cochlear disorders, such as tinnitus and hearing loss, have recently been reported to be more prevalent among migraine patients. However, whether headaches, including migraine or non-migraine headaches, are associated with vestibular and cochlear disorders remains unclear. Thus, the possible associations between headaches, including migraine and non-migraine headaches, and vestibulocochlear disorders were investigated in the present study. Methods Clinical data were analyzed from the Smart Clinical Data Warehouse of Hallym University Medical Center from 2011–2021. In patients with migraine and non-migraine headaches, data on Meniere`s disease (MD), benign paroxysmal positional vertigo (BPPV), vestibular neuronitis (VN), and cochlear disorders such as sensorineural hearing loss (SNHL) and tinnitus, were collected and compared with clinical data from controls without headache who had health check-ups. In addition, the presence of comorbidities were defined according to the International Classification of Diseases, tenth revision codes in the database and analyzed. Results The study included 15,128 participants with migraines and 76,773 with non-migraine headaches. Controls were identified based on propensity score matching (PSM). After PSM, the adjusted odds ratios (ORs) in subjects with migraine versus controls were 2.597 for MD (95% CI, 2.047–3.295; p < 0.001), 2.045 for BPPV (95% CI, 1.816–2.302; p < 0.001), 2.976 for VN (95% CI, 2.636–3.360; p < 0.001), 1.739 for SNHL (95% CI, 1.404–2.156; p < 0.001), and 1.970 for tinnitus (95% CI, 1.658–2.341; p < 0.001). The adjusted ORs for MD (1.771; 95% CI, 1.560–2.011; p < 0.001)), BPPV (1.731; 95% CI, 1.637–1.831; p < 0.001), VN (2.048; 95% CI, 1.935–2.168; p < 0.001), SNHL (1.396; 95% CI, 1.273–1.531; p < 0.001), and tinnitus (1.693; 95% CI, 1.569–1.826; p < 0.001) in patients with non-migraine headache versus controls were also high after PSM. Conclusion The present study findings indicated that migraine and non-migraine headaches are associated with an increased risk of cochlear disorders, such as SNHL and tinnitus, in addition to vestibular disorders.
