Objective.To describe the control of an outbreak of infection and colonization with the New York/Japan methicillin-resistantStaphylococcus aureus(MRSA) clone in multiple healthcare facilities, and to demonstrate the importance of making an MRSA management policy involving molecular typing of MRSA into a statewide public health responsibility.Setting.A range of healthcare facilities, including 2 metropolitan teaching hospitals and a regional hospital, as well as several community hospitals and long-term care facilities in a nonmetropolitan healthcare region.Interventions.A comprehensive, statewide MRSA epidemiological investigation and management policy.Results.In May 2005, there were 3 isolates referred to the Western Australian Gram-Positive Bacteria Typing and Research Unit that were identified as the New York/Japan MRSA clone, a pandemic MRSA clone with the ability to spread and replace existing clones in a region. Subsequent investigation identified 28 additional cases of infection and/or colonization dating from 2002 onward, including 1 involving a colonized healthcare worker (HCW) who had previously been hospitalized overseas. Of the 31 isolates detected, 25 were linked epidemiologically and via molecular typing to the isolate recovered from the colonized HCW. Four isolates appeared to have been introduced separately from overseas. Although the isolate from the single remaining case patient was genetically indistinct from the isolates that spread within Western Australia, no specific epidemiological link could be established. The application of standard outbreak management strategies reduced further spread.Conclusions.The elimination of the New/York Japan MRSA clone in a healthcare region demonstrates the importance of incorporating MRSA management policy into statewide public health programs. The mainstays of such programs should include a comprehensive and effective outbreak identification and management policy (including pre-employment screening of HCWs, where applicable) and MRSA clone identification by multilocus sequence typing.

This chapter looks at how two newspapers used unnamed sources in reports leading up to the invasion of Iraq in March 2003. When Iraq's weapons of mass destruction failed to materialize, critics on the left and from within journalism chastised the New York Times and Washington Post for overly credulous, unnamed source-laden investigative reporting appearing on their front pages in the buildup to the war. The newspapers responded by revisiting their unnamed sourcing practices, but not until more than a year after the invasion. These self-assessments generated attention around two problems negatively impacting prewar coverage: the calculated press management strategies of the Bush administration, and the willingness of the competing newspapers to reproduce official statements anonymously. The complex problems marking the journalist-unnamed source exchange come to light through these efforts to attach blame both to the sources and the journalists.

Chronic congestive heart failure (CHF) is a complex disorder characterized by inability of the heart to keep up the demands on it, followed by the progressive pump failure and fluid accumulation. Although the loop diuretics are widely used in heart failure (HF) patients, both pharmacodynamic and pharmacokinetic alterations are thought to be responsible for diuretic resistance in these patients. Strategies to overcome diuretic resistance include sodium intake restriction, changes in diuretic dose and route of administration and sequential nephron diuretic therapy. In this review, we discuss the definition, prevalence, mechanism of development and management strategies of diuretic resistance in HF patients.

This paper discusses issues encountered in developing a retrofit project for a roadway in a densely developed, commercialized area. The project was initiated, directed, and funded by the New York State Department of Transportation (NYSDOT). Numerous major shopping centers and a high volume of through traffic produce congestion and affect safety. NY-27, also known as Sunrise Highway, is a principal arterial in suburban Long Island. The conditions along NY-27 reflect what happens when development approvals are granted by local land use agencies without sufficient consideration of whether the transportation system can accommodate the associated traffic. NYSDOT, in this case, was put in the position of attempting to implement a retrofit project to mitigate the traffic and safety conditions. The proximity of shopping center driveways to each other and to nearby intersections results in weaving movements and heavy turning-movement volumes that block the through travel lanes and cause overlapping conflicts. The significant traffic congestion and high accident rate reflect the problems caused by inadequate access spacing and need for improved access management. The improvement options identified represented a wide range of access management strategies and configurations. The options were screened and modified to reflect community desires and business concerns. This paper reviews operational and safety problems, identifies a theoretical access configuration along the NY-27 corridor that could mitigate these problems, and reviews the alternative that was approved and is being progressed. The approved alternative reflects real-world considerations that affect a retrofit project.

Fungal diseases have their greatest impact on citrus in Florida by reducing tree vigor, fruit yield, and quality. Given the complex etiology of these diseases, this software was developed to facilitate diagnosis of symptoms and to explain the dynamics of Alternaria brown spot of mandarins, greasy spot, melanose, Phytophthora brown rot, post-bloom fruit drop, and sour orange scab. CITPATH includes a diagnostic key to identify symptoms of the major fungal diseases of citrus foliage and fruit in Florida and a hypertext program containing a description and graphic display of symptoms, maps of geographic occurrence, diagrams of disease development, and management strategies. Users can also consult a list of citrus cultivars susceptible to specific diseases and a reciprocal list of diseases affecting specific cultivars. Chemical control methods are discussed briefly with reference to the current Florida Citrus Spray Guide, a hardcopy of which is included with the software purchase. Developed for commercial growers, county extension programs, citrus horticulture classes, and master gardeners, this software is available on CD-ROM disks containing other citrus databases and as a separate disk for MS-DOS-based computers.

Stemphylium leaf blight (SLB), caused by Stemphylium vesicarium, is a foliar disease of onion worldwide, and has recently become an important disease in the northeastern United States and Ontario, Canada. The symptoms begin as small, tan to brown lesions on the leaves that can progress to defoliate plants. Crop loss occurs through reduced photosynthetic area, resulting in smaller, lower-quality bulbs. Leaf necrosis caused by SLB also can compromise bulb storage, as green leaves are required for the uptake of sprout inhibitors applied prior to harvest. The pathogen can overwinter on infested onion residue and infected volunteer plants. Asymptomatic weedy hosts near onion fields may also be a source of inoculum. Production of ascospores of the teleomorph (Pleospora allii) peaks in early spring in northeastern North America, often before the crop is planted, and declines rapidly as daily mean air temperatures rise. Conidia usually are present throughout the growing season. Application of fungicides is a standard practice for management of the complex of fungi that can cause foliar diseases of onion in this region. Recent assessments have shown that populations of S. vesicarium in New York and Ontario are resistant to at least three single-site mode-of-action fungicides. Three disease prediction systems have been developed and evaluated that may enable growers to reduce the frequency and/or number of fungicide applications, but the loss of efficacious fungicides due to resistance development within S. vesicarium populations threatens sustainability. The lack of commercially acceptable onion cultivars with sufficient resistance to reduce the number of fungicides for SLB also limits the ability to manage SLB effectively. Integrated disease management strategies for SLB are essential to maintain profitable, sustainable onion production across eastern North America.
