A further passage shows the imagination and craft of Achilles Tatius taken to a still greater degree, as he depicts the hero Cleitophon at the terrible crisis of believing his beloved to be dead. The narrative is suspended as the writer develops, at remarkable length, a psychological and physiological observation on delayed effects. Where Heliodorus had heaped up imagery with abundance, Achilles develops with skilled organization a single line of thought and imagery, developing it and enriching it as he goes. Rhythm plays a vital part in this remarkable union of tight order and inventive imagination. The expansion and the deployment of science have a Plutarchan element; but the fantasy and the tautness create something quite different out of rhythm.

Data on the use of Granulocyte colony-stimulating factor (G-CSF) in pregnant cancer patients are scarce. The International Network of Cancer, Infertility and Pregnancy (INCIP) reviewed data of pregnant patients treated with chemotherapy and G-CSF, and their offspring. Among 2083 registered patients, 42 pregnant patients received G-CSF for the following indications: recent chemotherapy induced febrile neutropenia (5; 12%), dose dense chemotherapy (28, 67%), poly chemotherapy (7, 17%), or prevention of neutropenia at delivery (2; 5%). Among 24 women receiving dose dense chemotherapy, three (13%) patients recovered from asymptomatic neutropenia within 5 days. One patient developed pancytopenia following polychemotherapy after which the pregnancy was complicated by chorioamnionitis and intrauterine death. Nineteen singleton livebirths (49%) were born preterm. Sixteen neonates (41%) were admitted to the Neonatal Intensive care Unit (NICU). No neonatal neutropenia occurred. Two neonates had congenital malformations. Out of 21 children in follow-up, there were four children with a motor development delay and two premature infants had a delay in cognitive development. In conclusion, the rate of maternal and neonatal complications are similar to those described in (pregnant) women treated with chemotherapy. Due to small numbers and limited follow-up, rare or delayed effects among offspring exposed to G-CSF in utero cannot be ruled out yet.

Background: To the authors’ knowledge, there have been no studies comparing the acute responses to “all out” efforts in concentric (isoinertial) vs. eccentric (isovelocity) cycling. Methods: After two familiarization sessions, 12 physically active men underwent the experimental protocols consisting of a 2-min warm-up and 8 maximal efforts of 5 s, separated by 55 s of active recovery at 80 rpm, in concentric vs. eccentric cycling. Comparisons between protocols were conducted during, immediately after, and 24-h post-sessions. Results: Mechanical (Work: 82,824 ± 6350 vs. 60,602 ± 8904 J) and cardiometabolic responses (mean HR: 68.8 ± 6.6 vs. 51.3 ± 5.7% HRmax, lactate: 4.9 ± 2.1 vs. 1.8 ± 0.6 mmol/L) were larger in concentric cycling (p < 0.001). The perceptual responses to both protocols were similarly low. Immediately after concentric cycling, vertical jump was potentiated (p = 0.028). Muscle soreness (VAS; p = 0.016) and thigh circumference (p = 0.045) were slightly increased only 24-h after eccentric cycling. Serum concentrations of CK, BAG3, and MMP-13 did not change significantly post-exercise. Conclusions: These results suggest the appropriateness of the eccentric cycling protocol used as a time-efficient (i.e., ~60 kJ in 10 min) and safe (i.e., without exercise-induced muscle damage) alternative to be used with different populations in future longitudinal interventions.

Most reports on the effects of epidermal growth factor (EGF) on gluconeogenesis have indicated that such effects depend on the substrate used and are only observable after a lag time of 30-40 min. Recently, an immediate and transient effect of EGF on glucose synthesis was described in a perfused liver system. Here we extend the study of the effect of EGF on gluconeogenesis to isolated hepatocytes from fasted rats. The delayed effect of EGF on gluconeogenesis was studied by adding the substrate 40 min after the peptide. Under these conditions EGF increased glucose synthesis from pyruvate, decreased it when the substrate was lactate or glycerol and did not modify gluconeogensis from fructose or dihydroxyacetone. EGF did not affect the metabolic flux through glycolysis, determined as the production of lactate+pyruvate from 30 mM glucose. Furthermore, EGF did not modify the metabolic flux through pyruvate kinase, determined as the production of lactate+pyruvate from 1 mM dihydroxyacetone. The differing effects of EGF on gluconeogenesis depending on the substrate used can be explained by the effects of EGF on the cytosolic redox state (measured as the lactate/pyruvate ratio). About 20 min after the addition of EGF, the mitochondrial redox state (measured as the 3-hydroxybutyrate/acetoacetate ratio) decreased. This effect of EGF was blocked by ammonium, which also abolished the effect of the peptide on gluconeogenesis. Thus the effect of EGF at the mitochondrial level appears to be necessary for its effects on gluconeogenesis. Taken together, our results indicate that the delayed effects of EGF on gluconeogenesis are secondary to the effects of the peptide at both the mitochondrial and cytosolic levels. In addition to these delayed effects, we observed that EGF rapidly and transiently stimulated glucose synthesis from lactate, decreased the cytosolic redox state and increased oxygen consumption. All of these rapid effects required the presence of extracellular calcium and disappeared in the presence of rotenone, suggesting that this rapid effect of EGF on gluconeogenesis is secondary to the stimulation of mitochondrial respiration.
