Objective Physiological bicarbonate/lactate-based solutions may correct acidosis in a better way than standard lactate-based solutions. In this study, a new 25 mmol/L bicarbonate/10 mmol/L lactate peritoneal dialysis (PD) solution was compared with a standard 35 mmol/L lactate solution. Design This was a prospective open label study. All patients had a 2-week baseline period using the standard lactate solution, followed by 8 weeks on the bicarbonate/lactate solution and 2 weeks on the lactate-basedsolution. Setting Four Danish and four Spanish nephrology centers. Patients 40 well-dialyzed (creatinine clearance > 55 L/week/1.73 m2 body surface area) patients on continuous ambulatory PD. Interventions Blood samples were taken for biochemistry (including venous blood gases) at week –2, day 1, weeks 2, 4, and 8, and at follow-up. A physical examination, a peritoneal equilibration test (PET), and quality of life (K/DQOL), ultrafiltration, and adequacy assessments were performed at baseline and at week 8. Vital signs and other safety parameters were followed at each visit. Extraneal (Baxter Healthcare, Castlebar, Ireland) was used by all patients for the long dwell. Main Outcome Measure Effect on the venous plasma bicarbonate level. Results Venous plasma bicarbonate levels rose from 24.4 mmol/L when patients were on the pure lactate to 26.1 mmol/L when using the bicarbonate/lactate solution ( p < 0.001). When patients were using the bicarbonate/lactate solution, 66% of values were maintained within the venous normal range of 24 – 30 mmol/L, versus 46.2% when patients were on the pure lactate solution ( p < 0.001). There were no adverse findings with respect to clinical symptoms, vital signs, or physical examination. The PET and adequacy, ultrafiltration, and K/DQOL assessment results were unchanged. Conclusions The new 25 mmol/L bicarbonate/10 mmol/L lactate solution provided better correction of acidosis than an equivalent 35 mmol/L standard lactate solution, without any safety issues.

Abstrak Analisis karakteristik penderita TB selain berguna untuk pengobatan juga berguna dalam memudahkan penemukan tersangka TB. Penelitian ini bertujuan untuk menganalisis karakteristik penderita TB. Desain studi yang digunakan adalah crossectional dengan sampel sebanyak 40 pasien TB yang berobat 3 bulan terakhir di Puskesmas Seberang Ulu 1 Palembang. Pengumpulan data dilakukan dengan kunjungan rumah ke pasien lalu dilakukan wawancara serta pemeriksaan fisik dan status kesehatan.  Hasil penelitian didapatkan bahwa karakteristik penderita TB paling banyak pada kelompok : usia produktif usia 12 – 35 tahun  dan  rentang usia dewasa 49 -  61 tahun; jenis kelamin laki-laki; pendidikan sekolah tingkat dasar; pekerjaan buruh; sosial ekonomi rendah; perokok aktif. Ditemukan 13.2% pasien TB dengan DM, 45% pasien TB dengan hipertensi, 47.5% pasien TB dengan anemia, 55% pasien TB dengan gizi kurang, 17.5% pasien memiliki riwayat keluarga menderita TB. Pasien kambuh sebanyak 12.5%; pasien yang putus OAT 17.5%; serta ditemukan tidak minum OAT sebanyak 15%.   Kata kunci: Karakteristik, Deskriptif, Tuberkulosis,                            Abstracts Analysis of characteristics TB disease besides useful for treatment is also useful to finding TB cases. This study aims to analyze characteristic of TB patients. This research used cross sectional study design with 40 TB patients treated last 3 months at Puskesmas Seberang Ulu 1 Palembang. Data collection was done with patient home visits, interview, physical examination health status. The results was most of characteristic TB patients in productive age group, maximum age 35 years, male gender, primary education, occupation was coolie, low socioeconomic, active smokers. TB Patients was founded 13.2% with DM; 45% TB patients with hypertension; 47.5% TB patients with anemia; 55% TB patients with malnutrition; 17.5% of patients had a family history of TB. Patients relapsed were 12.5%; OAT dropout patients were founded 17.5%; 15% patients were not OAT.   Keywords: Characteristics, Descriptive, Tuberculosis

Introdução: O aumento da população idosa em todo o mundo exige a capacitação de profissionais para o atendimento nesta área. Metodologia: Este é um estudo qualiquantitativo do tipo exploratório-descritivo realizado através da entrevista e exame físico feito nos idosos. Objetivos: O objetivo geral deste trabalho foi aplicar  o processo de enfermagem nos idosos com alzheimer participantes do projeto da UNICRUZ. Os objetivos específicos foram:  Realizar a avaliação do estado de saúde atual destes idosos; Levantar os principais diagnósticos de enfermagem encontrados; Proporcionar aos sujeitos cuidados de enfermagem integral através da realiazação das prescrições de enfermagem. Resultados: Após a coleta de dados foram levantadas as informações em comum obtidas na entrevista e exame físico que apontam idade média dos sujeitos de 76 anos, 4 idosos têm o diagnóstico de alzheimer há 5 anos; 3 deles são hipertensos; 5 deles têm distúrbios do sono; 3 têm irmãos com alzheimer e todos têm falha de memória e limitação de amplitude de movivento. Dos diagnósticos de enfermagem, pode-se destacar -  tensão do papel do cuidador e risco para o trauma. Dentre as  principais prescrições de enfermagem estão - estimular cognição e memória, atividade física e participação social. Conclusão: Foi possível aplicar o processo de enfermagem nos idosos e com isso obter um maior conhecimento do estado de saúde deles,  descrever os diagnósticos de enfermagem e levantar pontos de intervenção através da prescrição de enfermagem para promover cuidados como o estímulo à participação em grupos e encaminhamento ao odontólogo que poderão auxiliar no tratamento dos indivíduos e previnir complicações, oferecendo-os assistência e orientações de enfermagem e transdiciplinar. palavras chave Processo de enfermagem. Methodology: This qualitative-quantitative study is of the exploratory-descriptive type conducted by interview and physical examination in the elderly. Objectives: The aim of this work was to apply the nursing process in the elderly with Alzheimer's UNICRUZ project participants. The specific objectives were to perform the evaluation of current health status of elderly; Raise the main nursing diagnoses found; subject providing comprehensive nursing care through nursing realization prescriptions. Results: After data collection, it was raised together the information obtained in the interview and physical examination suggest that the average age of subjects 76 years, has four seniors diagnosed with Alzheimer's five years, three of them are hypertensive, five of them have disorders sleep, have three siblings with Alzheimer's and all have memory impairment and limitation of range of movivento. Of nursing diagnoses, can be highlighted - the role of caregiver stress and risk for trauma. The main requirements are nursing - stimulate cognition and memory, physical activity and social participation. Conclusion: It was possible to apply the nursing process in the elderly and therefore gain a greater understanding of their health status, describing nursing diagnoses and raise points of intervention through the prescription of nursing care and to promote the stimulation of participation in groups and referral to a dentist that can help treat and prevent complications of individuals, offering them assistance and guidance to nursing and transdisciplinary. Dr. Anderson has questioned our study on two counts. He asked whether the infants in our study could be considered normal when normality was based oniy on physical examination, hematocrit, and arterial blood gases and pH. This is an important point, but we think it is adequately covered in the Discussion section of our report. He also questioned whether the indications for umbilical arterial catheterization in some infants were valid in view of the potential hazards associated with the procedure.

Background and Aim: Since mental skills of nurses are effective on the quality of their activities, this research was conducted with the aim of determining the mental skills of physical examinations of children by nurses and related agents at the Children's Medical Education Center in Rasht. Methods: The current cross sectional study was performed on 127 female nurses working in pediatric wards of the 17th Shahrivar hospital in Rasht in 2018 by enumeration sampling method. A valid and reliable questionnaire comprised of two sections of demographic information and information for physical examination of children was used. Data were analyzed by SPSS software v.21. P value less than 0.05 was considered significant. Results: The highest number of nurses (71.7%) had a moderate mental skills. There were a significant relationship between the mean of mental skills with the service ward (p <0.001), interest in the current service ward (p = 0.031), and the history of retraining related to health status assessment (p = 0.034). While, there were not a significant relationship between the mean of mental skills with age (p = 0.847), total score at graduation date (p = 0.052), marital status (p = 0.812), number of children (p = 0.501), educational status (p = 0.797), employment status (p = 0.505), classification of clinical experience (p = 0.644), classification of work experience in the current ward (p = 0.127), shift work (p = 0.443), the degree of interest in the current field and job (p = 0.439). Conclusion: The results of this study indicated that the nurses had moderate mental skills, which the mean of mental skills were related to the total score at the date of graduation, the name of the service ward, the level of interest in the current ward and the history of retraining related to health status assessment. Therefore, considering the factors affecting the mental skills, this skill can be increased, which should be considered in future researches.
