Carcinoma originating from a thyroglossal duct cyst is a rare entity that is often identified on postoperative histopathology after Sistrunk procedure. In this procedure the thyroglossal duct cyst along with the entire tract and some part of hyoid bone are excised to prevent recurrence. We present the case of a 39-year-old male with midline neck swelling who was preoperatively diagnosed of locally advanced papillary carcinoma originating in a submental thyroglossal duct cyst and the presence of cervical lymph node metastasis.

Sistrunk Procedure

Thyroglossal duct cyst (TDC) is a cystic expansion of a remnant of the thyroglossal duct tract. Carcinomas in the TDC are extremely rare and are usually an incidental finding after the Sistrunk procedure. In this report, an unusual case of a 36-year-old woman with concurrent papillary thyroid carcinoma arising in the TDC and on the thyroid gland is presented, followed by a discussion of the controversies surrounding the possible origins of a papillary carcinoma in the TDC, as well as the current management options.

Thyroglossal duct cysts are most common congenital anomalies in thyroid development and are usually presented with midline neck swelling. The co-existence of carcinoma in thyroglossal duct cyst’s is extremely rare. We, herein present a case of primary papillary carcinoma arising from thyroglossal duct cyst in a 45 year old woman.
