During the past several years, major changes have occurred in the way in which health care is provided and financed. One of the outcomes of these changes has been a greater emphasis on providing quality health care at a low cost. This has led many healthcare facilities to question the necessity of all expenditures and to reduce spending when at all possible. Many healthcare facilities have solicited the services of consulting groups to provide recommendations regarding cost-containment strategies appropriate for their particular institution.

Managed health care has changed the way health services are provided and paid for. It is still evolving. Many pharmacists have already felt the impact of these changes. This continuing feature illuminates the many facets of managed care with special emphasis placed on how these changes may affect pharmacists working in health systems. The expertise provided by pharmacists will be needed to fulfill the potential of affordable, comprehensive, and quality health care as promised by managed care. Pharmacists must understand what is happening, why it is happening, and what is likely to happen in the future. To be an active and effective player, you must understand what is happening on the field.

The recent health care reform is one of the biggest changes that the health care industry has ever faced. This reform represents the paradigm changing opportunities and challenges for the company providing health insurance in a managed care environment. The CIO of a premier managed care health insurance provider (“ABC Company”) wants to take advantage of the new environment using Information Technology. He and his management teams have determined, using primary research, that the customer perception of the health care company’s cost and accessibility to the quality health care are the most important factors to their customers in the new market. They are aware that even though they have been able to use information technology to predict customer reaction to the changes in cost, and perception of quality, it will be very difficult to deliver new systems and processes that support ABC Company’s to the new realities it is facing the market.

On February 2,2004,U.S. District Judge Federico Moreno issued the final order and judgment authorizing a settlement between the HMO CIGNA Healthcare and the physicians who treated patients covered by CIGNA, ending the companys involvement in the larger class action In re Managed Care Litigation, which stil includes eight other HMOs. The settlement, estimated by plaintiffs experts to be worth 1.3 billion, mandates changes in the companys business and disclosure practices, establishes a non-profit foundation dedicated to the promotion of high quality health care, and guarantees at least 70 million in payments to the physicians. Similar in scope to the settlement approved between the physician class and Aetna Healthcare in October 2003,the settlement could potentially set a new standard for transparency and physician involvement in billing practices; however, much depends on the outcome of the remainder of the class action, which is set to go to trial later this year.
