Human pregnancy is associated with increased requirements for dietary energy and this increase may be partly offset by reductions in physical activity during gestation. Studies in well-nourished women have shown that the physical activity level (PAL), obtained as the total energy expenditure (TEE) divided by the BMR, decreases in late pregnancy. However, it is not known if this decrease is really caused by reductions in physical activity or if it is the result of decreases in energy expenditure/BMR (the so-called metabolic equivalent, MET) for many activities in late pregnancy. In the present study activity pattern, TEE and BMR were assessed in twenty-three healthy Swedish women before pregnancy as well as in gestational weeks 14 and 32. Activity pattern was assessed using a questionnaire and heart rate recording. TEE was assessed using the doubly labelled water method and BMR was measured by means of indirect calorimetry. When compared to the pre-pregnant value, there was little change in the PAL in gestational week 14 but it was significantly reduced in gestational week 32. Results obtained by means of the questionnaire and by heart rate recording showed that the activity pattern was largely unaffected by pregnancy. The findings support the following conclusion: in a population of well-nourished women where the activity pattern is maintained during pregnancy, the increase in BMR represents approximately the main part of the pregnancy-induced increase in TEE, at least until gestational week 32.

Although water is an important nutrient, there are no recommended intake values. Here, water intake, energy intake, physical activity and water loss was measured over 1 week in summer and in winter. Subjects were healthy volunteers, forty-two women and ten men, mean age of 29 (sd 7) years and mean BMI 21·8 (sd 2·2) kg/m2. Water intake was measured with a 7 d food and water record. Physical activity level (PAL) was observed as the ratio of total energy expenditure, as measured with doubly labelled water, to resting energy expenditure as measured in a respiration chamber. Water loss was measured with the deuterium elimination method. Water loss was highly reproducible and ranged from 0·20 to 0·35 l/MJ, independent of season and activity level, with higher values in women. Water loss was related to water and energy intake in summer (r 0·96, P<0·0001 and r 0·68, P<0·001, respectively) as well as in winter (r 0·98, P<0·0001 and r 0·63, P<0·01, respectively). Water loss was, for men, higher in subjects with a higher physical activity in summer (r 0·94, P<0·0001) and in winter (r 0·70, P<0·05). Normalizing water loss for differences in energy expenditure by expressing water loss in litres per MJ resulted in the same value for men in summer and winter. For women, physical activity-adjusted values of water loss were higher, especially in summer. In men, water turnover was determined by energy intake and physical activity, while seasonal effects appeared through energy expenditure. Women showed a higher water turnover that was unrelated to physical activity.

One of the aims of Identification and Prevention of Dietary- and Lifestyle-Induced Health Effects in Children and Infants (IDEFICS) validation study is to validate field measures of physical activity (PA) and energy expenditure (EE) in young children. This study compared the validity of uniaxial accelerometry with heart-rate (HR) monitoring vs. triaxial accelerometry against doubly labeled water (DLW) criterion method for assessment of free-living EE in young children. Forty-nine European children (25 female, 24 male) aged 4–10 yr (mean age: 6.9 ± 1.5 yr) were assessed by uniaxial ActiTrainer with HR, uniaxial 3DNX, and triaxial 3DNX accelerometry. Total energy expenditure (TEE) was estimated using DLW over a 1-wk period. The longitudinal axis of both devices and triaxial 3DNX counts per minute (CPM) were significantly ( P < 0.05) associated with physical activity level (PAL; r = 0.51 ActiTrainer, r = 0.49 uniaxial-3DNX, and r = 0.42 triaxial Σ3DNX). Eight-six percent of the variance in TEE could be predicted by a model combining body mass (partial r2 = 71%; P < 0.05), CPM-ActiTrainer (partial r2 = 11%; P < 0.05), and difference between HR at moderate and sedentary activities (ModHR − SedHR) (partial r2 = 4%; P < 0.05). The SE of TEE estimate for ActiTrainer and 3DNX models ranged from 0.44 to 0.74 MJ/days or ∼7–11% of the average TEE. The SE of activity-induced energy expenditure (AEE) model estimates ranged from 0.38 to 0.57 MJ/day or 24–26% of the average AEE. It is concluded that the comparative validity of hip-mounted uniaxial and triaxial accelerometers for assessing PA and EE is similar.

Body fatness and the components of energy expenditure in children aged 5–14 years were investigated. In a group of seventy-nine healthy children (thirty-nine female, forty male), mean age 10·0 (sd 2·8) years, comprising twenty-seven Maori, twenty-six Pacific Island and twenty-six European, total energy expenditure (TEE) was determined over 10 d using the doubly-labelled water method. Resting metabolic rate (RMR) was measured by indirect calorimetry and physical activity level (PAL) was calculated as TEE:RMR. Fat-free mass (FFM), and hence fat mass, was derived from the 18O-dilution space using appropriate values for FFM hydration in children. Qualitative information on physical activity patterns was obtained by questionnaire. Maori and Pacific children had a higher BMI than European children (P<0·003), but % body fat was similar for the three ethnic groups. The % body fat increased with age for girls (r 0·42, P=0·008), but not for boys. Ethnicity was not a significant predictor of RMR adjusted for FFM and fat mass. TEE and PAL, adjusted for body weight and age, were higher in Maori than European children (P<0·02), with Pacific children having intermediate values. PAL was inversely correlated with % body fat in boys (r −0·43, P=0·006), but was not significantly associated in girls. The % body fat was not correlated with reported time spent inactive or outdoors. Ethnic-related differences in total and activity-related energy expenditure that might account for higher obesity rates in Maori and Pacific children were not seen. Low levels of physical activity were associated with increased body fat in boys but not in girls.

Dance is an enjoyable health-promoting physical activity that many people worldwide incorporate into their lifestyles today. Therefore, dance improves your heart health, overall muscle strength, balance, and coordination, and reduces depression. We aim to gain insights into the effects of samba dance on the cardiorespiratory and metabolic response during an acute session of this dance style for 40-minutes. The study was carried out on 20 female samba dancers. All of them performed two procedures: (1) a cardiopulmonary exercise test on a treadmill for physical fitness aptitude verification and (2) a 40-minutes of samba dancing monitored by analysis of expired gases. The results were: At peak exercise: oxygen uptake (VO2) =32.7 mL.kg-1.min-1; heart rate (HR) =183 bpm; the pulse of oxygen (PO2) =10.9 mL.HR-1, energy expenditure (EE) = 9.9 kcal.min-1and metabolic equivalent (METs)=13.5.Overall, each session of the dance of 40-minutes was performed at a mean VO2 of 22.8 mL.kg-1.min-1 (70%VO2max), heart rate of 162 bpm (89%HRmax), energy expenditure (EE) of 6.5 kcal.min-1 (66%max), metabolic equivalent (METs) of 6.3 (62%max), and the rate of perceived exertion (RPE) of 11.8/20. In conclusion: The findings of the current investigation might suggest that the moderate and vigorous-intensity of the exercise verified in an acute samba dance session is enough to induce a chronic training effect recommended by the criteria established by the American College of Sports Medicine for improving cardiorespiratory fitness. This information may lead to a better understanding of the energy expenditure of samba dance and add to the compendium of physical activity.

Objective: The management of atypical antipsychotic-induced weight gain is a significant challenge for people with mental illness. Fundamental research into energy metabolism in people taking atypical antipsychotic medication has been neglected. The current study of men with schizophrenia taking clozapine aimed to measure total energy expenditure (TEE) and energy expended on physical activity – activity energy expenditure (AEE) and to consider the clinical implications of the findings. Method: The well-established reference method of doubly labelled water (DLW) was used to measure TEE and AEE in men with schizophrenia who had been taking clozapine for more than 6 months. Resting energy expenditure was determined using indirect calorimetry. Results: The TEE was 2511 ± 606 kcal day−1 which was signifcantly different to World Health Organization recommendations (more than 20% lower). The Physical activity level (PAL) was 1.39 ± 0.27 confirming the sedentary nature of people with schizophrenia who take clozapine. Conclusions: The findings support the need for weight management strategies for people with schizophrenia who take clozapine to focus on the enhancement of energy expenditure by increasing physical activity and reducing inactivity or sedentary behaviours, rather than relying primarily on strategies to reduce energy intake.

Accurate and easy-to-use methods to assess free-living energy expenditure in response to physical activity in young children are scarce. In the present study, we evaluated the capacity of (1) 4 d recordings obtained using the Actiheart (mean heart rate (mHR) and mean activity counts (mAC)) to provide assessments of total energy expenditure (TEE) and activity energy expenditure (AEE) and (2) a 7 d activity diary to provide assessments of physical activity levels (PAL) using three sets of metabolic equivalent (MET) values (PALTorun, PALAdolphand PALAinsworth) in forty-four and thirty-one healthy Swedish children aged 1·5 and 3 years, respectively. Reference TEE, PALrefand AEE were measured using criterion methods, i.e. the doubly labelled water method and indirect calorimetry. At 1·5 years of age, mHR explained 8 % (P= 0·006) of the variation in TEE above that explained by fat mass and fat-free mass. At 3 years of age, mHR and mAC explained 8 (P= 0·004) and 6 (P= 0·03) % of the variation in TEE and AEE, respectively, above that explained by fat mass and fat-free mass. At 1·5 and 3 years of age, average PALAinsworthvalues were 1·44 and 1·59, respectively, and not significantly different from PALrefvalues (1·39 and 1·61, respectively). By contrast, average PALTorun(1·5 and 3 years) and PALAdolph(3 years) values were lower (P< 0·05) than the corresponding PALrefvalues. In conclusion, at both ages, Actiheart recordings explained a small but significant fraction of free-living energy expenditure above that explained by body composition variables, and our activity diary produced mean PAL values in agreement with reference values when using MET values published by Ainsworth.

The doubly labelled water method for the measurement of total daily energy expenditure (TDEE) over 1–3 weeks under daily living conditions is the indicated method to study effects of exercise and extreme environments on energy balance. Subjects consume a measured amount of doubly labelled water (2H218O) to increase background enrichment of body water for 18O and 2H, and the subsequent difference in elimination rate between 18O and 2H, as measured in urine, saliva or blood samples, is a measure for carbon dioxide production and thus allows calculation of TDEE. The present review describes research showing that physical activity level (PAL), calculated as TDEE (assessed with doubly labelled water) divided by resting energy expenditure (REE, PAL = TDEE/REE), reaches a maximum value of 2·00–2·40 in subjects with a vigorously active lifestyle. Higher PAL values, while maintaining energy balance, are observed in professional athletes consuming additional energy dense foods to compete at top level. Exercise training can increase TDEE/REE in young adults to a value of 2·00–2·40, when energy intake is unrestricted. Furthermore, the review shows an exercise induced increase in activity energy expenditure can be compensated by a reduction in REE and by a reduction in non-exercise physical activity, especially at a negative energy balance. Additionally, in untrained subjects, an exercise-induced increase in activity energy expenditure is compensated by a training-induced increase in exercise efficiency.

AbstractObjectivesTo describe issues related to energy requirements of free living adults and discuss the importance of basal metabolic rate (BMR) and their relationships to total energy expenditure (TEE ) and physical activity level (PAL, derived as TEE/BMR) and to determine the influence of body weight, height, age and sex.DesignBased on a review of the literature, this paper examines the variability in BMR due to methodology, ethnicity, migration and adaptation (both metabolic and behavioural) due to changes in nutritional status. Collates and compiles data on measurements of TEE in free living healthy adults, to arrive at limits and to compare TEE of populations with different life-styles.Results and ConclusionsThe constancy of BMR and its validity as a reliable predictor of TEE in adults as well as the validity of PAL as an index of TEE adjusted for BMR and thus its use to categorise the physical activity pattern and lifestyle of an individual was confirmed. The limits of human daily energy expenditure at around 1.2×BMR and 4.5×BMR based on measurements made in free living adults have been reported in the literature. A large and robust database now exists of energy expenditure measurements obtained by the doubly labelled water method in the scientific literature and the data shows that, in general, levels of energy expenditure are similar to the recommendations for energy requirements adopted by FAO/WHO/UNU (1985). The review also confirms that metabolic adaptation to energy restriction is not an important factor that needs to be considered when recommending energy requirements for adults in developing countries.
