People seeking asylum (PSA) are recommended to undertake a comprehensive risk-based health assessment within 1 month of arrival in Australia. The Asylum Seeker Resource Centre (ASRC) offers health services to PSA in Victoria, through the ASRC nurse-led clinic. A healthcare assessment is conducted by nurses using a Refugee Health Assessment (RHA) tool. A process evaluation was conducted to assess if the adapted 2016 version of the RHA tool was able to appropriately identify, describe and prioritise the needs of PSA. Twelve ASRC nurses who conducted assessments were interviewed. All interviews were audio recorded, transcribed and analysed. The adapted RHA tool was considered as appropriate for identifying, describing and prioritising the needs of PSA. Three key interconnected themes emerged influencing the appropriateness of the tool: the tool; users’ experiences; and the individual characteristics of the PSA undergoing the assessment. Key tool limitations included: the structure not being user-friendly; variability in users’ sensitivity to using the tool; and the limited feedback from PSA on the adapted RHA tool. Given the high number of people seeking asylum in Australia and the complexity of their healthcare needs, this research provides preliminary guidance on ways to improve the appropriateness of the ASRC-adapted RHA tool.

ABSTRACT The World Health Organization (WHO) planned a new functioning assessment tool - the WHODAS 2.0 (World Health Assessment Disability Schedule 2.0) - a user-friendly application instrument based on the theoretical framework of the ICF. The aim of this article is to introduce the Brazilian version of WHODAS to clinical and academic users interested in the assessment of functioning. The process was conducted according to the Translation Package offered by WHO, in addition to the permission for translating the tool. Three translators worked in this research to produce the definitive version of the manual and a set of linguistic evaluation forms. Finally, the Brazilian version was approved by WHO. All documents about WHODAS were translated and are free to download on the internet. WHODAS 2.0 addresses functioning according to the ICF domains and keeps the multifactorial and biopsychosocial perspective of the phenomenon. As it has an ICF differentiate presentation, the data collection is faster (20 minutes, on average) and more objective. Besides, it was planned to be a generic and transcultural tool. WHODAS was translated and is available on Internet to use. It is expected that the use of WHODAS will be widespread in Brazil, in clinical and research settings, aiming at the biopsychosocial approach of the functioning, as endorsed by WHO.

Many individuals who experience mental and emotional problems prefer support from clergy instead of formal mental health services. Because clergy are often sought out by those with mental illnesses, it is essential to understand their perspectives about mental and emotional problems and identify the individual characteristics that influence these perspectives. This study utilized qualitative data from a conversation among pastors in an online social networking group to examine the characteristics of clergy who held affirming attitudes toward professional mental health services. Thirty-five pastors participated in the discussion, generating more than 140 comments over 13 days in response to the question: “If the church is where we are to come for healing, how do we handle people who are depressed, suicidal, suffering from PTSD or anxiety?” Thematic analysis was conducted, and results suggest four characteristics common among clergy: personal experiences with mental and emotional problems, transparency in sharing their stories, personal help-seeking, and humility. These four characteristics may influence clergy members’ present-day understanding and responses to mental and emotional problems. Implications for spiritual care professionals and mental health professionals are presented.

Leadership has an important role in the organization as leader is in charge of supervising and controlling the course of an organization. Ability to lead in mobilizing and empowering employees will affect the performance of the organization. This behavior has a significant impact on the attitudes, behavior and performance of employees. The results of the study showed there is a significant relationship between characteristics such as age (p = 0.004), education(p = 0.034), work experience (p = 0.000), the experience of the organization (p = 0.000), and educational hierarchy (p =0.000) for leadership flexibility. Sex variable is not significant to the leadership flexibility (p = 0.801). There is a relationship with the flexibility of directive leadership style (p = 0.027), supportive leadership style (p = 0.046), and participative leadership style (p = 0.009) with the flexibility of leadership. There is aso a relationship between achievement-oriented leadership style and leadership flexibility (p = 0.000). There is a relationship between the individual characteristics of leadership style and versatility with variable: educational level of leadership (p = 0.021; OR = 19.265). The result suggests that we need more organized seminars / work shop / scientific studies that stimulate the realization of learning about leadership flexibility in improving the performance of the District/City Health Office and it is necessary to study theperformance of the head of the Department of Health assessment intensively and periodically

Caregiver well-being plays an important role in children's development and a number of factors have been found to impact distress levels among caregivers of children and youth referred for mental health services. Further, caregiver distress impacts youth psychopathology, its acuity as well as related mental health interventions. The purpose of this study was to develop and validate an algorithm for identifying caregivers who are at greatest risk of experiencing caregiver distress. This algorithm was derived from, and will be embedded in, existing comprehensive interRAI child and youth instruments. Ontario data based on the interRAI Child and Youth Mental Health assessment instruments (ChYMH and ChYMH-DD) were analyzed to identify predictors of distress among caregivers of children and youth ages 4–18 years. Starting with proactive aggression, the algorithm uses 40 assessment items to assign one of 30 nodes that are grouped into five levels of risk. The interRAI ChYMH Caregiver Distress (iCCareD) algorithm was validated using longitudinal data from mental health agencies across Ontario and was found to be a good predictor among this sample with a c-statistic of 0.71 for predicting new or ongoing caregiver distress and 65% for both sensitivity and specificity using algorithm values of 3 or greater. This algorithm provides an evidence-based decision-support tool embedded within a comprehensive assessment tool that may be used by clinicians to inform their selection of supports and services for families.

Abstract Background: The prevention and control of hypertension should be an effective way to reduce deaths and it has been a high priority in China. The Chinese government issued the National Essential Public Health Services Package (NEPHSP) in 2009; this initiative provides free public health services to meet the challenges posed by hypertension. It includes health education, regular health checkups, and regular follow-ups provided to patients with hypertension aged ≥ 35. This study explored the influence of the NEPHSP on outpatient and inpatient expenditure among patients with hypertension. Methods: Data were mined from the 2011–2015 Harmonized China Health and Retirement Longitudinal Study. The dependent variables were the outpatient and inpatient expenditure of patients with hypertension. The independent variable was defined as whether covered by the NEPHSP in 2013 or 2015. Using propensity score matching (PSM) to match the individual characteristics of hypertension in the NEPHSP-covered group and the NEPHSP-uncovered group, Tobit regression models with difference-in-differences (DID) were used to analyze the outcomes. Results: After PSM, of the 1,956 hypertensive participants, 369 had physical exams covered by the NEPHSP in 2013 and 2015. In 2013 and 2015, the outpatient and inpatient expenses of patients with hypertension increased compared with 2011. A DID estimate for the NEPHSP-covered service was associated with a marginal significant decrease of RMB 319.79 (p = 0.586) and RMB 1072.02 (p = 0.068) in hypertension inpatient expected expenditure in 2013 and 2015, respectively. The DID estimate showed no significant change among outpatient expected expenditure. Conclusions: The NEPHSP may reduce inpatient expenditure among patients with hypertension. Further strengthening of the NEPHSP may reduce their burden.
