ABSTRACT INTRODUCTIONInsomnia has negative health effects and may indicate underlying serious conditions, but is underdiagnosed and often not discussed with a doctor. AIMThis study aimed to explore the utility and workability in New Zealand community pharmacies of a 23-question sleep-screening tool adapted from the Short Auckland Sleep Questionnaire. METHODSA multidisciplinary advisory group (sleep specialist, general practitioner and pharmacists) discussed the tool, pharmacists’ capability in managing insomnia and training needs for pharmacists, and recommended management strategies, including referral points. Twelve community pharmacists piloted the tool with people with insomnia who presented in pharmacies, recording the time it took to administer the tool. The pharmacists were then surveyed about their experiences with the tool and possible improvements. RESULTSTen pharmacists took an average of 12.4 min (range 4–35 min) for each use of the screening tool with 62 people with insomnia. Most pharmacists found the screening tool easy to administer, organised and easy to follow and nine of 10 said it provided better information than their usual consultation. Seven of 10 pharmacists would use it again. Time limitations and low recruitment were potential barriers to usage especially for pharmacy owners. DISCUSSIONThe screening tool could provide a useful addition to pharmacists’ toolkits, aiding information gathering and better than usual practice. The tool was acceptable to most pharmacists, but its use takes time and remuneration needs consideration.

Managing medicine shortages consumes ample time of pharmacists worldwide. This study aimed to explore the strategies and resources being utilized by community pharmacists to tackle a typical shortage problem. Qualitative face-to-face interviews were conducted. A total of 31 community pharmacists from three cities (Lahore, Multan, and Dera Ghazi Khan) in Pakistan were sampled, using a purposive approach. All interviews were audio taped, transcribed verbatim, and subjected to thematic analysis. The analysis yielded five broad themes and eighteen subthemes. The themes highlighted (1) the current scenarios of medicine shortages in a community setting, (2) barriers encountered during the shortage management, (3) impacts, (4) corrective actions performed for handling shortages and (4) future interventions. Participants reported that medicine shortages were frequent. Unethical activities such as black marketing, stockpiling, bias distribution and bulk purchasing were the main barriers. With respect to managing shortages, maintaining inventories was the most common proactive approach, while the recommendation of alternative drugs to patients was the most common counteractive approach. Based on the findings, management strategies for current shortages in community pharmacies are insufficient. Shortages would continue unless potential barriers are addressed through proper monitoring of the sale and consumption of drugs, fair distribution, early communication, and collaboration.

The value judgments about medicine are contributed to by the public image. Formerly this has been one of a devoted, caring, self-sacrificing, somewhat unworldly group of people, dedicated to their work for the suffering and diseased. But the doctors are not separate from society and they are affected by its values. These have been adopted by the profession so that it is coming to be seen as no worse and no better than any other group of comparable education and training. Its status has diminished and this has called into question its compensation at a high level. Status cannot be maintained when its base has been eroded.

Barotrauma is one of the most common medical problems associated with aviation and has been a causal factor for significant loss of trained aircrew hours. The occurrence of barotrauma has been reported not only in flight but also during simulated exposures to changing pressures as a part of aeromedical evaluation and training. This occurrence is despite the screening of individuals by otoscopy and tympanometry before exposure to barometric changes. In such cases, baro-challenge-induced eustachian tube (ET) dysfunction has been identified as the most probable cause, leading to decreased ability to equalize pressures across the middle ear. This paper describes a paradigm approach to the prediction of baro-challenge-induced ET dysfunction using the nine-step test and ET dysfunction questionnaire. The inclusion of the test as a screening tool before exposure of aircrew to baro-challenge after recovery from their ear, nose, and throat ailments is recommended to prevent occurrence/ reoccurrence of barotrauma.

FIFA 15 and Pro Evolution Soccer (PES) 15 are soccer games that are popular in Indonesia. Usability testing needs to be done to assess user interest and satisfaction with both and provide an overview of the comparison of them. The framework used for testing is McCall’S. The test combines operability matrix and training matrix to determine software quality. McCall’S was chosen because it has a reliable and comprehensive quality factor indicators. The results of the tests carried out were data on the operability level of PES15 games of 76.81% ± 15.76% and FIFA15 games of 70.65% ± 20.73%. Testing of training matrices produced 15.96 ± 21.74 seconds for PES15 and 78.29 ± 25.73 seconds for FIFA15 game training matrix. The data shows that reusability of PES15 is better than FIFA15.

ABSTRAKPerikanan  tangkap  di  kawasan  Taman  Nasional  Karimunjawa  dikelola  oleh  lebih  dari  satu  instansipengelola.  Setiap  instansi  diduga  mempunyai  peran  masing-  masing  sesuai  tupoksinya.  Saat  ini  belum diketahui secara nyata instansi apa saja yang mengelola beserta perannya. Tujuan penelitian ini adalah untuk mengidentifikasi instansi pengelola yang berperan dalam pengelolaan perikanan tangkap di kawasan TNKJ, persepsi,  aspirasi  dan  partisipasi  nelayan  terhadap  pengelolaan  perikanan  tangkap  serta  bagaimana  strategi pengelola  dalam  pengawasan  perikanan  tangkap.  Metode  penelitian  ini  adalah  studi  kasus  dengan  analisis deskriptif, dimana pengumpulan data melalui wawancara dan studi pustaka. Penentuan responden menggunakan metode purposive sampling, responden terdiri dari nelayan dan instansi pengelola. Hasil penelitian menunjukan bahwa  instansi yang  mengelola  perikanan tangkap adalah Unit  Pelaksana  Teknis   Pelabuhan Perikanan Pantai Karimunjawa, Balai Taman Nasional Karimunjawa, Dinas Perikanan, Polisi Air   dan Tentara Nasional Indonesia Angkatan Laut dan Satuan Kerja Pengawasan Sumberdaya Kelautan dan   Perikanan. Pembagian peran masing- masing instansi pengelola sudah sesuai dengan bidang masing-masing  serta tidak terjadi tumpang tindih kewenangan. Sinkronisasi dan koordinasi sudah dilakukan, hal ini terbukti   dengan  adanya  Nota  Kesepakatan Bersama. Sebanyak 80% nelayan setuju dengan peraturan yang diterapkan. Partisipasi nelayan terhadap sosialisasi dan pelatihan rendah yaitu 35% dan 19%. Strategi pengelola dalam pengawasan perikanan tangkap adalah dengan membentuk dan memberdayakan Pengawas   Perikanan  dan  Kelompok  Masyarakat  Pengawas  secara  sinergi. Strategi untuk  meningkatkan partisipasi  nelayan  dalam  sosialisasi  dan  pelatihan  adalah  dengan  memberikan penyadaran  motivasi dan  apresiasi berupa penghargaan dan pendampingan. ABSTRACTCapture   fisheries   in Karimunjawa National   Park   are   managed by   more   than one   management institutions.  Each  institution  is  assumed  to  have  their  respective  roles  according  to  their  basic  tasks  andfunctions. It is not yet known exactly what institutions are managing along with their roles. The purposes of  this study  were to know what institutions  were involved  in the management of capture fisheries in TNKJ  areas, perceptions, aspirations and participation of fishermen on the management of capture fisheries and  how the management strategy in the supervision of capture fisheries. This research method was case study with descriptive analysis, where data collected by interview and literature study. Determination of  respondents used purposive sampling method, respondents consist of fishermen and manager institution. The results showed that the institutions that managed capture fisheries were Technical Implementation Unit  Karimunjawa Fishery Port, Karimunjawa National Park Office, Fisheries Official, Water Police and Indonesian National Army – Navy and Work Unit of Supervision of Marine and Fisheries Resources. The  division of roles of each institution agency was in accordance with their respective fields and there was no  overlapping authority. Synchronization and coordination had been done, it was proved by the Memorandum of Understanding (MoU). As many as 80% of fishermen agree with the regulations applied. The  participations of fishermen on socialization and training were low at 35% and 19%. The management strategies in the supervision of capture fisheries were by establishing and empowering Fisheries Supervisor  and a group of supervisor community (PokMasWas). The strategies to increase the participation of fishermen in socialization and training were by giving awareness of motivation and appreciation in the form  of recognition and assistance.

The effectiveness of simulations for teaching statistical concepts was compared to the effectiveness of a textbook. The variable Medium (simulation versus textbook) and Question specificity (Specific versus Non-specific), were manipulated factorially. Question specificity was defined as follows: Subjects were presented with a scenario in which gumballs were sampled from an urn. Subjects in the “Specific” condition were given a specific question about the outcome of the sampling procedure to consider; subjects in the “Non-specific” condition were asked generally to consider what would happen. A no-treatment control was included. The subjects consisted of 115 college students. The dependent variable was performance on problems requiring subjects to apply what they learned to ill defined everyday problems. Subjects trained by simulation performed significantly better than those trained with a textbook. Subjects in the “Specific” condition performed better than those in the “Non-specific” condition, although the difference did not reach conventional levels of significance. These results support the increasing use of simulation in education and training.

PURPOSE: The use of telemedicine expanded dramatically in March 2020 following the COVID-19 pandemic. We sought to assess oncologist perspectives on telemedicine's present and future roles (both phone and video) for patients with cancer. METHODS: The National Comprehensive Cancer Network (NCCN) Electronic Health Record (EHR) Oncology Advisory Group formed a Workgroup to assess the state of oncology telemedicine and created a 20-question survey. NCCN EHR Oncology Advisory Group members e-mailed the survey to providers (surgical, hematology, gynecologic, medical, and radiation oncology physicians and clinicians) at their home institution. RESULTS: Providers (N = 1,038) from 26 institutions responded in Summer 2020. Telemedicine (phone and video) was compared with in-person visits across clinical scenarios (n = 766). For reviewing benign follow-up data, 88% reported video and 80% reported telephone were the same as or better than office visits. For establishing a personal connection with patients, 24% and 7% indicated video and telephone, respectively, were the same as or better than office visits. Ninety-three percent reported adverse outcomes attributable to telemedicine visits never or rarely occurred, whereas 6% indicated they occasionally occurred (n = 801). Respondents (n = 796) estimated 46% of postpandemic visits could be virtual, but challenges included (1) lack of patient access to technology, (2) inadequate clinical workflows to support telemedicine, and (3) insurance coverage uncertainty postpandemic. CONCLUSION: Telemedicine appears effective across a variety of clinical scenarios. Based on provider assessment, a substantial fraction of visits for patients with cancer could be effectively and safely conducted using telemedicine. These findings should influence regulatory and infrastructural decisions regarding telemedicine postpandemic for patients with cancer.
