Wastage of conserved fodder when feeding livestock

The objective of the present review was to establish levels of conserved fodder wastage when feeding livestock (sheep, beef cattle, dairy cattle) under various conditions and using various feed-out systems, and to determine the factors affecting wastage. The mean wastage of hay recorded in the literature reviewed was 17% of the DM offered, but the range was from 4 to 77%. The main factors affecting the degree of wastage were storage method, packaging method, method of feeding out, amount of fodder on offer and its palatability and/or quality and the impact of wet weather. Although the emphasis was on hay, the principles should also apply to silage. If wastage was 40% rather than 5%, the cost of feeding conserved fodder to livestock would be a third greater than producers might expect or budget on.

Abstract Background Mortality from cryptococcal meningitis remains very high in Africa. In the Advancing Cryptococcal Meningitis Treatment for Africa (ACTA) trial, 2 weeks of fluconazole (FLU) plus flucytosine (5FC) was as effective and less costly than 2 weeks of amphotericin-based regimens. However, many African settings treat with FLU monotherapy, and the cost-effectiveness of adding 5FC to FLU is uncertain. Methods The effectiveness and costs of FLU+5FC were taken from ACTA, which included a costing analysis at the Zambian site. The effectiveness of FLU was derived from cohorts of consecutively enrolled patients, managed in respects other than drug therapy, as were participants in ACTA. FLU costs were derived from costs of FLU+5FC in ACTA, by subtracting 5FC drug and monitoring costs. The cost-effectiveness of FLU+5FC vs FLU alone was measured as the incremental cost-effectiveness ratio (ICER). A probabilistic sensitivity analysis assessed uncertainties and a bivariate deterministic sensitivity analysis examined the impact of varying mortality and 5FC drug costs on the ICER. Results The mean costs per patient were US $847 (95% confidence interval [CI] $776–927) for FLU+5FC, and US $628 (95% CI $557–709) for FLU. The 10-week mortality rate was 35.1% (95% CI 28.9–41.7%) with FLU+5FC and 53.8% (95% CI 43.1–64.1%) with FLU. At the current 5FC price of US $1.30 per 500 mg tablet, the ICER of 5FC+FLU versus FLU alone was US $65 (95% CI $28–208) per life-year saved. Reducing the 5FC cost to between US $0.80 and US $0.40 per 500 mg resulted in an ICER between US $44 and US $28 per life-year saved. Conclusions The addition of 5FC to FLU is cost-effective for cryptococcal meningitis treatment in Africa and, if made available widely, could substantially reduce mortality rates among human immunodeficiency virus–infected persons in Africa.

Cutting pick is a kind of widely-used consumptive mining tool. The traditional producing technics of cutting pick body is foundry, or machining after roughly forging, or machining directly from metal bar. By former technics, the property of products is poor, and by latter, the material availability is low and the cost is high. The patent technology for cutting pick body warm extrusion introduced in this paper can overcome all the disadvantages mentioned above. In this paper, by analyzing the characteristic of cutting pick body warm extrusion, adopting the principle of power balance to solve the approximate solution of strain forces, the approximate calculating formulas of extruding power are deduced. The main factors affecting on extrusion force are determined theoretically. This research can be used as basis to design tooling and choose proper equipment for this new technology.

This study aims to test the influence of the senior management’s support as a moderating variable on the relationship between the independent factors (Training, Empowerment, Motivation and Communication) and the dependent variable (Performance of Employees). (SEM-AMOS) is used to test the impact of the moderating variable. Where it is depended on the method of sampling or analysis of what is known as multiple-groups analysis. The paragraphs of the senior management’s support variable are collected and divided into two groups according to the mean of the total paragraphs. In addition, according to the relative weights given to the paragraphs of the questionnaire, using a five- point’s Likert scale: 1= strongly disagree to 5 = strongly agree. The first group consisted of the grades less than the mean and it is considered as the group which is non-supporters of the existence of support. While the second group consisted of the grades higher than the mean and considered as the group which is a supporter of the existence of support. The study found that the model of study in the presence of the support of the senior management’s is appropriate for the second group and inappropriate in light of the lack of support by the senior management’s support for the first group.

Aim: To study the medication costs of various topical glaucoma medications using data collected from real world use by patients.Methods: Patients with primary open angle glaucoma treated at glaucoma clinics in 5 hospitals (1 rural and 4 urban) in northern India from 1 January to 30 June 2008 were enrolled. The number of days each bottle of medication lasted was recorded, and the mean cost per day was computed from the maximum retail price and mean number of days each medication lasted.Results: 790 of 801 eligible patients completed the study. The mean number of days that a bottle of medication lasted was found to be highest for Xalatan® and Xalacom® at 35.23 days and 35.00 days, respectively. The brand name prostaglandin analogues all lasted for a mean of more than 30 days: Xalatan, 35.23 days (SD, 4.14 days); Lumigan®, 31.37 days (SD, 5.31 days); and Travatan®, 34.84 days (SD, 6.51 days), while the generic eye drops lasted for about 21 days: latanoprost, 20.69 days (SD, 3.69 days) and bimatoprost, 21.39 days (SD, 4.34 days). The cost of the generic medication was less than the brand name medication in all groups (for example, bimatoprost, Indian rupees 9.76 versus Indian rupees 12.33) except for brimonidine/timolol (Indian rupees 8.73 versus Indian rupees 8.66). Further analysis in 2009 showed that, for latanoprost, brimonidine and brimonidine/timolol, the difference between the brand name and generic medications decreased in 2009 over 2008 (in the latanoprost group, the cost difference over the year reduced from Indian rupees 592 in 2008 to Indian rupees 523 in 2009); the cost difference for bimatoprost increased from 2008 to 2009.Conclusion: When both cost and number of days a bottle lasts were considered over the long term, use of generic medications might not minimise the cost of glaucoma medical management by much when compared with the brand name medication.

A 17.63 m long bore-hole core extracted from the deepest part of Lake Yamanaka, one of the Fuji-five Lakes at the northeasternfoot of Mount Fuji, central Japan, composed of sediment with intercalations of scoria fallout deposits. The sediment of the upper11.4 m was investigated for grain-size distribution by using a laser diffraction particle size analyser. The mean grain-size profileshowed various degrees of fluctuations, both short-and long-terms, and the size-frequency distribution revealed unimodal-trimodalmixing of sediments. Changes in lake size and water depth appear to be the main factors affecting the variability in the grain-sizedistribution and properties. To cope with the increasing healthcare costs brought about by the universal health insurance programme, national volume-based procurement (NVBP) was implemented in China to reduce drug prices. However, the impact of NVBP remains unknown. We reported the effects of the NVBP pilot programme on medication affordability and discussed the challenges and recommendations for further reforms. A total of 25 molecules won the bidding in the NVBP pilot programme, and price cuts ranged from 25% to 96%. Medication affordability was measured as the number of days’ wages needed to pay for a course of treatment, and the medication was identified as affordable if the cost of a treatment course was less than the average daily wage. After the NVBP, the proportion of affordable drugs increased from 33% to 67%, and the mean affordability improved from 8.2 days’ wages to 2.8 days’ wages. Specifically, for rural residents, the proportion of affordable drugs increased from 13% to 58%, and the mean affordability improved from 15.7 days’ wages to 5.3 days’ wages. For urban residents, the proportion of affordable drugs increased from 54% to 71%, and the mean affordability improved from 5.9 days’ wages to 2.0 days’ wages. Implementing the NVBP substantially improved medication affordability. In future reforms, a multifaceted approach addressing all issues in the health system is needed to enhance medicine access.

Purpose This paper aims to model different causal relations among factors interacting with labour productivity in order to recognize the most important factors influencing and influenced by it. Design/methodology/approach Top 60 factors affecting labour productivity were determined and grouped into 5 major groups by reviewing previous research and interviewing relevant experts. The interactions of factors were modelled using system dynamics (SD) approach. The resulting causal loop diagrams obtained from SD were then applied for identifying the most crucial factors influencing and influenced by labour productivity through the decision-making trial and evaluation laboratory (DEMATEL) method. The impact of factors on each other was finally determined based on the opinions of 63 experts selected from the Iranian construction industry. Findings The results indicated that factors such as fatigue, lack of labour motivation and lack of skill are the most influencing, and factors such as schedule delay and inflation in the cost of execution are the most influenced by labour productivity. In the end, a set of recommendations to improve construction labour productivity was also presented. Originality/value The main contribution of the study is proposing a novel method which is capable of providing insights into how causes and effects of construction labour productivity are interrelated. Furthermore, the proposed method makes this study distinct from previous research in the light of prioritizing factors and offering recommendations according to the interrelationships among factors.

Over the last decades, the emphasis on the health of dairy cows has changed from an individual to a herd level. In this scenario, the role played by the recording system and its interpretation by veterinarians has gained primordial importance. The records of productive and reproductive performance and of sanitary status from a southern Brazilian dairy cattle herd have been presented and discussed. The period of study was 2000-2009. Mean values per lactation period were 349D 8436M 290F 275P 201SCS (D: days in lactation, M: kg of milk yield, F: kg of fat, P: kg of protein and SCS: somatic cell score in 1000 cells/ml of milk). Major indexes of reproductive efficiency included age at first calving (31 months), services per conception (2.1), intercalving interval (428 days), calving to conception interval (146 days), mean annual rates of parturitions (76.2%), fetal losses (9.8-19.0%), and stillbirths (3.6%), apart of voluntary waiting period (94 days). Main information on sanitary status of the herd was associated with the mean prevalence of common disorders of dairy cattle such as anaplasmosis (29.8%), mastitis (27.8%), digital diseases (26.3%), ovarian cysts (21.3%), placental retention (19.7%), postpartum uterine infections (10.6%), and calf diarrhea (23.7%) and pneumonia (16.8%), among others. In addition, culling reasons (low reproductive performance [56.3%] and udder/mastitis problems [33.6%]), causes of cattle deaths (anaplasmosis [16.4%] and leukosis [11.4]), and the impact of cattle diseases such as tuberculosis, leukosis, and neosporosis on the herd have also been presented and succinctly discussed. Numbers between brackets represent rates accumulated in the 10-year period.

Blood flow has been extensively studied because of its close relationship with cardiovascular disease. Heart valves blood flow analysis is particularly complex due to the high mobility of its leaflets, a fact that has stimulated the development of computational models aimed to its better understanding. For studying heart valves blood flow, we developed a mathematical model derived from clinical observations based on echocardiographic images, which describe valve leaflets motion and its influence on blood flow. This work presents a concentrated-parameters-based model of heart valves blood flow that takes into consideration five main factors affecting such a flow in the mitral and aortic valves. This model considers factors that are related to blood fluid and valve leaflets characteristics. Considering the main factors involved, it was found that blood flow exhibit an abnormal behavior in response to small variations (less than 10%) in blood pressure gradient or in leaflets stiffness. Likewise, after changing the roughness of the leaflets, the impact is smaller, only slightly affecting blood flow behavior with changes beyond 30%. Moreover, it was observed that the influence of fluid vortices originated behind the valves can be disregarded and the kinetic energy induced by them is almost negligible.

Aim. To assess the degree of satisfaction among hemodialysis patients and the factors influencing this satisfaction.Methods. Patients were recruited from 3 Saudi dialysis centers. Demographic data was collected. Using 1 to 10 Likert scale, the patients were asked to rate theoverall satisfactionwith, andthe overall impactof, their dialysis therapy on their lives and to rate the effect of the dialysis therapy on 15 qualities of life domains.Results. 322 patients were recruited (72.6% of the total eligible patients). The mean age was 51.7 years (15.4); 58% have been on dialysis for 3 years. The mean Charlson Comorbidity Index was 3.2 (2), and Kt/V was 1.3 (0.44). The mean satisfaction score was (7.41 2.75) and the mean score of the impact of the dialysis on the patients' lives was 5.32 2.55. Male patients reported worse effect of dialysis on family life, social life, energy, and appetite. Longer period since the commencement of dialysis was associated with adverse effect on finances and energy. Lower level of education was associated with worse dialysis effect on stress, overall health, sexual life, hobbies, and exercise ability.Conclusion. The level of satisfaction is affected by gender, duration on dialysis, educational level, and standard of care given.
