Evaluation of pharmacy students' rural placement program: preparation for interprofessional practice

Objective To date, there are few data on pharmacy-specific placement programs and their impact on students’ work after graduation. The present study evaluated an innovative rural pharmacy placement program targeted at influencing students to work and live in rural areas after graduation. A secondary aim of the study was to explore the students’ intentions to come back and practice in rural areas as a result of their involvement in the rural pharmacy program. Methods A questionnaire was distributed, by hand, to a total of 58 third and fourth (final) year pharmacy students undertaking their rural placement in the Gippsland region, in rural Victoria in 2011 and 2012. Results Fifty-seven responses were returned (response rate = 98%). Students stated that understanding pharmacy practice from a rural perspective, visits to rural health professionals and sites and the attitude of their preceptors were essential to their satisfaction with their rural placements. A significant number of students (72%) intend to seek employment in rural areas if opportunities arise as a result of their increased rural awareness. The key components for a successful rural placement program were described by the surveyed students as social awareness, recognising job opportunities and interprofessional learning. Conclusion The evaluation of the rural placement program revealed that students valued their visits to rural sites and their interaction with other rural health professionals the most. What is known about the topic? Rural undergraduate student programs have been initiated as a result of several Australian government strategies to address shortages in rural health workforce. Subsequently, various rural placement programs have been integral parts of several disciplines, including medicine, dentistry, nursing, occupational therapy and pharmacy among others. To date, there are few data on pharmacy-specific placement programs and their impact on students’ work after graduation What does this paper add? The rural pharmacy program is important in influencing students’ perceptions and interest in a career in rural areas. The key components for a successful rural placement program were described by the surveyed students as social awareness, recognising job opportunities and interprofessional learning. Interprofessional learning and collaboration are inevitable due to the shortage of health professionals and the move towards holistic management of patients in healthcare settings. The development of an interprofessional rural education that combines rural medical and pharmacy students together highlights the importance of an interprofessional approach to preparing students to work in rural areas.

Objective.This paper describes stages in the research literature related to recruitment and retention of health professionals to rural health careers. Data sources.Electronic databases accessed included Medline, CINAHL, Social Sciences and Humanities. Key search terms included ‘high school’, ‘career choices’, ‘rural’, ‘attachment’, ‘recruitment’ and ‘retention’. Data synthesis.We identified four stages: (1) making career choices; (2) being attached to place; (3) taking up rural practice; and (4) remaining in rural practice. This is termed the ‘rural pipeline’. However, as some stages of the ‘rural pipeline’ refer specifically to the medical profession, we propose an extension of the notion of the medical ‘rural pipeline’ to include other professions such as nursing, midwifery and allied health. Conclusions.Utilising the ‘rural pipeline’ as a template for medicine, nursing and allied health would strengthen current approaches to the recruitment and retention of professionals in rural areas and provide a consolidated evidence base that would assist in policy development to improve availability and service provision of the rural health workforce. Future research that utilises a multidisciplinary approach could explore how the role and relationship between place and identity shape needs of career choices and would provide important information to advance the practical aspects supporting rural health career pathways. The pipeline, however, refers mainly to the medical profession. The stages in this paper extend the notion of the medical ‘rural pipeline’ to include other professions such as nursing and allied health. This paper focusses on literature concerning developed countries such as Australia, New Zealand, Europe, the USA and Canada and identifies several proposed areas of future research. What are the implications for practitioners?The literature clearly identifies important issues for the rural health workforce. Having an understanding of the key issues underpinning the recruitment and retention of health professionals in rural areas allows the development and enhancement of appropriate workforce strategies. Utilising the ‘rural pipeline’ as a template for medicine, nursing and allied health would strengthen current approaches to the recruitment and retention of professionals in rural areas and provide a consolidated evidence base.

The recent COVID-19 pandemic has revealed flaws in rural settings where most people live without the necessary tools, income, and knowledge to tackle such unprecedented global challenges. Here, I argue that despite the research studies conducted on rural areas, these have not solved rising rural issues, notably poverty and illiteracy. I propound a global institute to be formed by governments that provides a platform for empowering rural communities through better training, skills, and competencies. Such global endeavour will ensure the remaining rural communities withstand future pandemics if they occur.

The European Union (EU) has undergone significant economic crises in recent years. Therein, young people were amongst the hardest hit groups, with youth unemployment rising as high as 50% in some member states. Particularly high rates of youth unemployment were often observed in rural areas, where labour market supply in relation to demand were notably divergent. One of the core pillars of the EU’s agenda is to tackle the persistent problem of youth unemployment. Since the recent crisis, this has been via the “Youth on the Move” initiative, which involves the promotion of intra- and international mobility of young adults in order to gain access to job opportunities. However, what has received little attention so far is the question of what the general willingness of young adults to move is like, and to what extent this varies, for example, depending upon the area they live in. This paper therefore asks if rural youth differ from youth in urban areas in relation to their willingness to move for a job within their country or to another country. Moreover, what influences the general willingness to be mobile? Based on the Cultural Pathways to Economic Self-Sufficiency and Entrepreneurship (CUPESSE) Survey, which includes data on 18–35-year-olds in a sample of 11 European countries, it is shown that living in a rural area is strongly associated with the willingness to move. Furthermore, it shows that rural youth are more willing to move within the country but less willing to move to another country. Based on the presentation of the various factors, which promote or curb mobility readiness, the results make it clear that the success of EU initiatives depends on the preferences and willingness of the target group in question.

Abstract Background Pharmacists are a crucial part of the health workforce and play an important role in achieving universal health coverage. In China, pharmaceutical human resources are in short supply, and the distribution is unequal. This study aimed to identify the key job characteristics that influence the job preferences of undergraduate pharmacy students and to elicit the relative importance of different job characteristics to shed light on future policy interventions. Methods A discrete choice experiment was conducted to assess the job preferences of undergraduate pharmacy students from 6 provinces in mainland China. A face-to-face interview was used to collect data. Conditional logit and mixed logit models were used to analyse data, and the final model was chosen according to the model fit statistics. A series of policy simulations was also conducted. Results In total, 581 respondents completed the questionnaire, and 500 respondents who passed the internal consistency test were analysed. All attributes were statistically significant except for open management. Monthly income and work location were most important to respondents, followed by work unit (which refers to the nature of the workplace) and years to promotion. There was preference heterogeneity among respondents, e.g., male students preferred open management, and female students preferred jobs in public institutions. Furthermore, students with an urban background or from a single-child family placed higher value on a job in the city compared to their counterparts. Conclusion The heterogeneity of attributes showed the complexity of job preferences. Both monetary and nonmonetary job characteristics significantly influenced the job preferences of pharmacy students in China. A more effective policy intervention to attract graduates to work in rural areas should consider both incentives on the job itself and the background of pharmacy school graduates.

During the wake of the COVID-19 pandemic, many schools quickly transitioned their teaching and assessment strategies to online formats. In Spring 2020, a 3-station remote Objective Structured Clinical Examination (OSCE) was implemented for first-year pharmacy students. The day following the remote OSCEs students answered three open-text prompts about the remote OSCE experience: (1) “I liked...”, (2) “I learned…”, and (3) “I suggest…”. Responses were open-coded and frequency counts were calculated to determine the most prevalent codes. Concept maps were created to visualize and explore connections between the codes. Out of 157 students, 156 students completed the reflection assignment, a 99.36% response rate. The three major themes in the Liked data were: Logistics (n = 65, 41.7%), Differences In-person Versus Remote (n = 59, 37.8%), and Skill Development (n = 43, 27.6%). The three major themes in the Learned data were: Technology (n = 66, 42.3%), Communication (n = 58, 37.2%), and Skill Development (n = 56, 35.9%). The three major themes in the Suggest data were: Logistics (n = 89, 57.1%), Technology (n = 31, 19.9%), and Continuation of Remote OSCE (n = 31, 19.9%). Overall, the remote OSCE experience was well-received, and students described it as applicable to their future pharmacy practice. Future work should explore the design, implementation, and outcomes of remote OSCEs.

AbstractOut of 1.21 billion population of India, 69% of them live in rural areas. There is a wide disparity in the distribution of health infrastructure and services in rural areas as compared to that of urban areas. The National Rural Health Mission (NRHM) launched in 2005, aimed to bridge this gap has introduced Accredited Social Health Activists (ASHAs), as health activists into the rural health care. ASHA is an acronym for Accredited Social Health Activists and she has been so far instrumental in facilitating institutionalised delivery, child immunisation, ensuring family planning, besides organising village nutrition day. She has been the vital link between the community and the health care. India, as a nation that is progressing is trying to combat communicable diseases significantly but it is also witnessing the surfacing of a different problem. There is an increasing prevalence of non-communicable diseases (NCDs), including diabetes which poses a big economic burden so much so that NCDs have been labelled as ′a health and developmental emergency′. Diabetes competes with other health concerns in a struggle to secure government health funding. In this resource-limited context, innovative methods are required to reach out to people at grass root levels. ASHA, which means hope in Sanskrit, can be true to her name in providing increased access to diabetes care to the rural population, if adequately trained and empowered. A multi-stakeholder approach through a public-private-people partnership (PPPP) is needed to tackle the issue with this kind of magnitude. The current review focuses on providing suggestions on utilising ASHAs′ services in spreading awareness on diabetes and ensuring that people with diabetes (PWD) receive optimal diabetes care.

Background/Aim. Motivated and job satisfied health professionals represent a basis of success of modern health institutions. The aim of this study was to investigate whether there was a difference in work motivation and job satisfaction between health workers in urban and rural areas in the region of Central Serbia. Methods. The study included 396 health professionals from urban setting, and 436 from a rural area, employed in four randomly selected health facilities. An anonymous questionnaire was used for data gathering. Statistical analysis was performed using ?2, Student t-test, Spearman's correlation coefficient, and logistic regression analysis. Results. Urban health professionals were significantly more motivated and job satisfied than respondents from rural area. In relation to work motivation factors and job satisfaction of health professionals in urban and rural areas, there were no significant differences in working conditions and current equipment, and in terms of job satisfaction there were no significant differences in relation to income either. Conclusion. In order to increase the level of work motivation and job satisfaction of health workers in rural areas, apart from better income, they should get more assistance and support from their supervisors, and awards for good job performance; interpersonal relationships, promotion and advancement opportunities, managerial performance and cooperation at work should be improved; employment security should be provided, as well as more independence at work, with professional supervision of health workers.

Community pharmacies in Nepal and other South Asian countries are in a mediocre state due to poor regulation and the fact that many pharmacies are run by people with insufficient training in dispensing. This has led to the inappropriate use of medicines. The problems due to poor regulation and the mediocre state of community pharmacies in South Asia encompass both academia and clinical practice. In this paper, a 2-week community pharmacy internship programme completed by 2 graduating pharmacy students of Pokhara University (a Nepalese public university) at Sankalpa Pharmacy, Pokhara, Nepal is illustrated. During the internship, they were systematically trained on store management, pharmaceutical care, counselling skills, the use of medical devices, pharmaceutical business plans, medicine information sources, and adverse drug reaction reporting. An orientation, observations and hands-on training, case presentation, discussion, and feedback from 2 senior pharmacists were used as the training method. A proper community pharmacy internship format, good pharmacy practice standards, and a better work environment for pharmacists may improve the quality of community pharmacies.
