Resource barriers to the facilitation of organ and tissue donation reported by Australian emergency clinicians

Objective. To explore emergency department clinicians’ perceived resource barriers to facilitating organ and tissue donation (OTD). Methods. A cross-sectional national online survey of Australian emergency department (ED) clinicians. Results. ED clinicians reported a range of resource barriers that hinder the facilitation of OTD, most notably a lack of time to discuss OTD with a patient’s family (74.6%). Those reporting more resource barriers had been less involved in OTD-related tasks. For example, those reporting a lack of time to assess a patient’s suitability to be a potential donor had less experience with OTD-related tasks in the last calendar year than did those who reported that they often or always have enough time for this (P < 0.01). In addition, ED clinicians working in DonateLife network hospitals were more involved in OTD-related tasks (P < 0.01) and reported fewer resource shortages in the ED and the hospital overall. Conclusions. Resource shortages hinder the facilitation of OTD in the ED and are related to decreased involvement in OTD-related tasks. In addition, ED clinicians working in DonateLife hospitals are more involved in OTD-related tasks and report fewer resource shortages overall. Addressing resource shortages and extending the DonateLife network could benefit OTD rates initiated from the ED. What is known about the topic? Increasing the rate of organ and tissue donation (OTD) has become progressively more urgent as waiting lists for organs and tissues are growing globally. Recently a missed potential donor pool was recognised in emergency departments (EDs) and the Organ and Tissue Authority implemented a ‘clinical trigger’ tool to aid with the identification of potential donors in EDs. However, many Australian studies have reported worsening ED overcrowding and resource shortages in recent years with an adverse effect on patient care and satisfaction as well as on ED clinicians’ work-related stress and satisfaction. International literature has identified that certain resource barriers hinder the facilitation of organ and tissue in EDs. However, there is currently no literature available on how resource barriers in Australian EDs affect the facilitation of OTD. What does this paper add? Our study shows that Australian ED clinicians perceive a range of resource barriers that hinder the facilitation of OTD, most notably a lack of time to discuss OTD with a patient’s family or to identify potential donors. We also found that those reporting more resource barriers had been less involved in OTD-related tasks in the last calendar year. In addition, those that work in hospitals that are part of the DonateLife network, and thus have dedicated staff available for OTD-related tasks, were more involved in OTD-related tasks and reported fewer resource shortages in the ED and the hospital overall. What are the implications for practitioners? To maximize the number of potential donors recognised and referred from the ED, it may be important to decrease the resource barriers identified in this study. Notably, the presence of specialist OTD staff, a function of being part of a DonateLife network hospital, may result in a decreased perception of resource barriers in the ED and more engagement with OTD-related tasks by ED clinicians.

Background: We investigated the risk factors associated with the psychological impact of each healthcare worker group, to help optimize psychological interventions for healthcare workers in countries affected by COVID-19.Methods: Participants for this study comprised 1,783 healthcare workers drawn from two hospitals inKorea. We adopted a cross-sectional online survey design and used an anonymous questionnaire from 20-30 April 2020, with information on demographics, psychiatric history, Stress and Anxiety to Viral Epidemics - 9 (SAVE-9), Patient Health Questionnaire - 9 (PHQ-9), and Generalized Anxiety Disorder 7 (GAD-7) scale.Results: Among the 1,783 healthcare workers, compared to other healthcare workers, nursing professionals had significantly higher levels of depression, general anxiety, and virus-related anxiety symptoms (p &lt; 0.01). In the nursing professionals group, single workers reported more severe depressive symptoms (PHQ-9 ≥ 10) than married workers (20.3% vs 14.1%; p &lt; 0.01), and junior (&lt; 40 years old) workers reported more anxiety about the viral epidemic (p &lt; 0.01). Logistic regression analysis adjusted with age, sex, and work duration identified three factors as significantly associated with healthcare worker's depression (PHQ-9 ≥ 10): being a nursing professional (adjusted odds ratio [OR] 1.44, 95% confidence interval [CI] 1.05 – 1.98), single (adjusted OR 1.36, 95% CI 1.01 – 1.83), and higher stress and anxiety to the viral infection (high SAVE-9 score, adjusted OR 1.19, 95% CI 1.15 – 1.22). Conclusions: Psychological support and interventions for healthcare workers, especially nursing professionals, single, and high SAVE-9 level should be considered.

Abstract. Background: Chronic work stress is a problem which persists among nurses for several reasons and studies suggest that both levels of stress and depression remain on an ongoing high level. However, not much is known about the impact of differential aspects of chronic work-related stress on levels of depression when investigating geriatric and registered nurses. Aim: The aim of this study was to analyse the relationship between various aspects of chronic work stress and levels of depression in geriatric and registered nurses. Methods: A cross-sectional online survey among 370 nurses was carried out. Besides demographic information, several aspects of chronic work stress were assessed with the Trier Inventory of Chronic Stress and levels of depression with the Beck Depression Inventory-II. Statistical analyses included multiple linear regression. Results: Most important work-related predictors of depression scores in nurses were ’Work dissatisfaction’ (ß = .237; 95 %-CI: [.194; .280]) and ’Work overload’ ( ß = .161; 95 %-CI: [.124; .198]) within blockwise multiple regression analysis. A previously diagnosed psychiatric disorder ( ß = –.268; 95 %-CI: [–.313; –.223]) proved to be the strongest predictor of nurses’ depression experience. Our model explained 37 % of variance in depression scores. However, being a geriatric or registered nurse did not predict levels of depression. Conclusions: Different aspects of chronic work stress seem to be associated with levels of depression in nurses. This should be taken into account when working on the improvement of working conditions for nurses.

Budaya keselamatan merupakan kunci untuk mendukung tercapainya peningkatan keselamatan dan kesehatan kerja dalam organisasi. Upaya membangun budaya keselamatan merupakan langkah pertama dalam mencapai keselamatan pasien. Terdapat beberapa faktor yang berkontribusi dalam perkembangan budaya keselamatan yaitu; sikap baik individu maupun organisasi, kepemimpinan, kerja tim, komunikasi dan beban kerja. Penelitian ini bertujuan mengetahui faktor determinan yang berhubungan dengan terciptanya budaya keselamatan pasien di RS Pemerintah Kabupaten Kuningan. Teknik pengambilan sampel yang digunakan incidental sampling 88 orang perawat pelaksana. Rancangan penelitian menggunakan survey analitik dengan pendekatan cross sectional, uji hipotesis digunakan Chi Square dan regresi logistik ganda. Hasil penelitian menunjukan terdapat pengaruh yang signifikan antara persepsi terhadap manajemen (p 0.0005, odd rasio 21.3), dukungan tim kerja (p 0.0005, odd rasio 13.34), stress kerja (p 0.006, odd rasio 3.94), kepuasan kerja (nilai p 0. 002) dengan budaya keselamatan pasien. Tidak terdapat pengaruh yang signifikan kondisi kerja dengan budaya keselamatan pasien dengan nilai p 0.507. Berdasarkan analisis multuvariat diperoleh persepsi terhadap manajemen menjadi factor determinan dengan nilai p 0.000 < α 0.05. Simpulan; unsur pimpinan memiliki pengaruh yang signifikan dalam menciptakan budaya keselamatan pasien. Pimpinan memiliki kewenangan dalam menerapkan system yang berlaku dalam organisasi, oleh karena itu gaya kepemimpinan, teknik komunikasi serta kemampuan manajerial merupakan suatu hal yang sangat perlu diperhatikan dalam menciptakan atmosfer kerja yang kondusif sebagai upaya terciptanya budaya keselamatan pasien. Berdasarkan hasil penelitian bahwa model kepemimpinan transformasional merupakan model yang sesuai diterapkan untuk meningkatkan budaya keselamatan pasien, pelatihan keterampilan komunikasi efektif serta pengembangan model pendidikan antar profesi sebagai upaya peningkatan kemampuan kolaborasi.Kata kunci:Budaya keselamatan pasien, stress kerja, kepuasan kerja.Determinant factors that are Influencing Patient Safety Culture in a Government-owned Hospitals in Kuningan Regency AbstractSafety culture is a key to support the achievement of occupational health and safety in an organization. An effort to build safety culture is the first step in ensuring patient safety. There are some factors that contribute in the development of safety culture, namely, individual and organizational attitude, leadership, team work, communication, and work load. This study aimed to identify the determinant factors that are related to achievement of patient safety culture in a government-owned hospital in Kuningan Regency. Eighty eight samples of nurses were recruited using incidental sampling technique. The research design was using cross sectional study, the hypothesis testing were using Chi Square and multiple logistic regression. The results showed that there were significant influenced between perception towards management (p= 0.0005, odd rasio 21.3), team work support (p= 0.0005, odd rasio 13.34), work-related stress (p= 0.006, odd rasio 3.94), work satisfaction (p= 0. 002) with patient safety culture. There was not significant influenced between work condition and patient safety (p= 0.507). The multivariate analysis showed that perception towards management was the determinant factor for patient safety culture (p 0.000 < α 0.05). In conclusion, leaders have significant influence in creating patient safety culture. Leaders have authority to implement systems in the organization. Therefore, leadership style, communication technique, and managerial ability are important in order to create a conducive atmosphere for developing patient safety culture. As recommendation, transformational leadership is a model that is appropriate to be applied in order to increase patient safety culture, trainings of effective communication and inter-professional education model are also needed to increase the collaboration skills among health professionals.Keywords:Patient safety culture, work-related stress, work satisfaction.

Nurses working in palliative care are at risk of burnout. The Copenhagen Burnout Inventory was used to determine burnout levels of nurses working in the Portuguese national network of palliative care. We evaluated the contribution of personal, work, and COVID-19 variables in three burnout subclasses: personal, work, and patient-related. A cross-sectional, exploratory, and quantitative design was employed and participants were sampled using convenience and snowball technique. An online survey was conducted and 153 nurses participated in our study. Socio-demographic characterization was conducted, levels of burnout and determinants were explored through multiple linear regression models for its three dimensions. High levels of personal, working, and patient burnout were present in 71 (46%), 68 (44%), and 33 nurses (22%), respectively. Most of them agreed that COVID-19 had an impact on their activities. Significant personal and work related burnout factors found were specialization in palliative care, self-perceived health status, unit type, weekly hours of work, and allocation to COVID-19 units. Gender was found to be a significant factor in patient-related burnout. There is a high level of burnout among nurses working in the Portuguese national network of palliative care. Measures for identification and mitigation of burnout are necessary to protect health care professionals.

Background. Workplace mental health (WMH) is an important and expanding practice area for occupational therapists. Purpose. This cross-sectional study explores the current practices and support needs of Ontario-based occupational therapists in WMH. Method. Registrants from the provincial association who provide mental health and/or work-related services were invited to participate. Ninety-three therapists completed an online survey; 10 participated in follow-up focus groups. Analysis involved descriptive statistics of the survey data and content analysis of the focus group data. Findings. Return to work, reactivation, and functional cognitive assessment were the most commonly reported services. Key challenges were limited funding and training and lack of awareness of the occupational therapy role. Therapists emphasized the importance of raising the competence, credibility, and profile of occupational therapy in WMH. Implications. Defining the role of occupational therapy, developing training and resources, and advocacy are needed to advance occupational therapy practice within this rapidly changing field.

Background Like other health professionals, community pharmacists are exposed to stress factors (being efficient, avoiding mistakes and bearing emotional load), but they are also under the pressure of entrepreneurial responsibilities. The main objective was to assess the level of work-related stress in French community pharmacies. The other objectives of the study were to assess the associated comorbidities and causes of work-related stress. Methods This observational cross-sectional study was sent to all French community pharmacies by email. The survey was anonymous and designed to collect the following items: socio-demographic factors, professional status, characteristics of community pharmacy, work-related stress (visual analogic scale—VAS), fatigue (VAS), sleep disturbances (questions), anxiety and depression symptoms (hospital anxiety and depression scale), medical consultation for work-related stress, medication use for work related stress, psychoactive drug-use and causes of work-related stress. Participants were included in the survey if they were pharmacists (owner or assistant) or pharmacy technicians working in a community pharmacy at the time of the survey. Exclusion criteria were defined as follows: pharmacy students or other professionals involved in a community pharmacy (e.g. dietician, beautician) and lack of professional status information. There was no age limitation. Results After three months of data collection, 1,339 participants answered the survey and 1,272 participants were included in conformity with the inclusion and exclusion criteria, and to avoid missing data on the primary endpoint. Work-related stress was detected in 32.8% (417/1,272) of individuals (scores ≥70/100). Men were significantly more affected than women and there was no difference between professional statuses and no relation with the age of the participants. Work-related stress was significantly associated with anxiety, depression, fatigue, sleep disturbances, medical consultations, medication use, alcohol consumption above the WHO recommendations for men and psychoactive drug use. Three causes of stress were clearly identified and related to stress levels, workload, working atmosphere and deterioration of work quality. However, causes of work-related stress were significantly different among professionals, for example: entrepreneurial burden for pharmacists-in-charge and workload for employees (assistant pharmacists and pharmacy technicians). Discussion Work-related stress has a very strong impact in French community pharmacies. This stress was associated with several comorbidities and induces health resource consumption. Several causes of work-related stress have been identified such as workload, working atmosphere and deterioration of work quality; however, these causes could be detected and managed to improve stress levels. We recommend developing individual and organizational stress management in French community pharmacies.

Background. Work is reported as one of the main sources of psychological stress. Because of its role in the onset of burnout and impact on economic and health systems, work-related stress (WS) has become an issue of much concern. Among modifiable factors capable of reducing WS, two categories of physical activity (PA), namely leisure-time and moderate-to-vigorous physical activity (LTPA and MVPA), show promising evidence. Previous findings suggest that LTPA and MVPA allow adults to experience psychological detachment from job demands and restore their depleted resources at work. However, the optimal independent doses of LTPA and MVPA associated with a lower WS level has not yet been established. Methods. The aim of this study was therefore to address this gap using a cross-sectional, nationally representative sample of 4 200 Canadian workers. MVPA was measured through accelerometry and a self-reported assessment was conducted to collect data on WS and LTPA. Results. Generalized additive models indicated that one hour a day on average spent doing a LTPA of 8.5 METs-hour was associated with the highest benefits on WS (p < 0.001, Adjusted R2 = 0.04) while the optimal average daily dose of MVPA was around 90 minutes (p < 0.001, Adjusted R2 = 0.04). Noteworthy, first signs of WS reduction appear long before optimum is reached (e.g. 30 minutes of MVPA), stressing the relevance of merely doing an LTPA/MVPA regardless of the dose. Conclusion. Findings offer practical recommendations for public health policies on the optimal doses of MVPA/LTPA associated with decreased WS.

Background: Despite the prevalence of obesity and its increases in associated diseases such as diabetes and cardiovascular disease, no studies have addressed the spread of obesity among police officers working in stressful environment. Therefore, the present study conducted to access the prevalence of work related stress in police personnel.  Aims and Objective: To access the prevalence of obesity and stress burden among police workers and to see association of stress burden of police workers with their obesity.   Material and Method: This cross-sectional observational study done on 245 police workers in PAC, Sitapur, India. Subjects having more than ten years of working experience were included in study. Anthropometric measurements of subject recorded by following standard protocol. Standardized stress scale was used to access the degree of stress in police workers. Available data analyze and expressed in percentage, mean with SD and chi square test to see the significance of association.   Results: According to result of body mass index 77.14% of subjects were obese and 9.8% of subject was overweight. According to waist circumference 82.04% of subjects were obese. Result of stress score showed that 31.84% of subjects having mild stress and 16.73% of subjects having moderate stress. 30.35% of subjects having abdominal obesity were suffered from mild stress and there association was highly significant (p=0.02).   Conclusion: The study recommends health education programs to combat obesity and overweight. Regular counseling should be conducted to alleviate work related stress.
