This paper explores low levels of women's health service utilization for Sexually Transmitted Infections (STIs)in Nepal. We interviewed 120 women individually and 53 in focus groups. Predictors of lower utilisation were self-medication, consultation with faith healers, inadequate knowledge of STIs, beliefs about causes, fear, social taboos and stigmatisation, women's secondary status, and presence of male health professionals. Results indicate the importance of people's beliefs in their decisions about health care. Strategies to improve access to health services in Nepal should systematically investigate the role of all these factors to improve access to and utilisation of health services for STIs.

People from refugee-like backgrounds living in Australia face substantial health challenges that benefit from access to health and social services. Many people from refugee-like backgrounds have frequent contact with education sector staff, who have the potential to act as conduits to health and social services. The aim of this project was to improve access to refugee-focused health services for people from refugee-like backgrounds in south-eastern Melbourne by codesigning and delivering a 1-day education forum to education sector staff. Evaluation of the forum used mixed-methods analysis of data from pre-post and follow-up surveys (n=11; administered before, immediately after and 1 month after the forum respectively), post-forum interviews (n=4) and one school’s referral records. The forum improved attendees’ refugee-focused health service referral knowledge, confidence and behaviour. This was supported by the qualitative findings of high staff motivation, high forum satisfaction and evidence of outcome sustainability. Education staff are an important complement to an integrated model of health care for recently resettled people from refugee-like backgrounds. These results show that a straightforward local intervention can improve the ability of education staff to act as conduits to health services, increasing access to health services for people from refugee-like backgrounds.

Objetiva-se analisar como as mulheres encarceradas percebem o acesso aos serviços de saúde. Estudo descritivo,exploratório e qualitativo realizado com quarenta mulheres em um presídio feminino do Estado do Rio de Janeiro, Brasil.Foi utilizada na coleta de dados, a entrevista semiestruturada, e posterior transcrição na integra. As entrevistas foramsubmetidas à análise de conteúdo na modalidade temática. Antes e durante o encarceramento, as mulheres referiramdificuldades para acessar os serviços de saúde. Entretanto, surgem falas de modo contraditório, inferindo a possibilidadede, após a entrada no sistema penal, essas mulheres conseguem acessar alguns serviços de saúde jamais visitados antesdo período de confinamento. Conclui-se que para garantir a aplicabilidade da política vigente, existe a necessidade deestratégias de atenção integral à saúde dessas mulheres, que assegurem o acesso aos serviços de saúde numa perspectivaampliada de promoção, prevenção, tratamento e reabilitação, antes, durante e depois da prisão.Palavras-chave: Acesso aos Serviços de Saúde; Assistência Integral à Saúde; Mulheres; Prisões. AbstractThe aim is to analyze how incarcerated women perceive access to health services. This descriptive, exploratory andqualitative study was carried out with 40 women in a female prison in the state of Rio de Janeiro, Brazil. It was used indata collection, semi-structured interview, and subsequent transcription in the integrated. The interviews weresubmitted to content analysis in the thematic modality. Before and during incarceration, women reported difficulties inaccessing health services. However, they appear contradictory, inferring the possibility of, after entering the penal system,these women were able to access some health services never visited before the confinement period. It concludes that toensure the applicability of the current policy, there is a need for comprehensive health care strategies for these women,which ensure access to health services in a broader perspective of promotion, prevention, treatment and rehabilitation,before, during and after imprisonment.Keywords: Access to Health Services; Integral Health Assistance; Women; Prisons.

Abstract Background: In Nigeria, adolescents are highly vulnerable to sexually transmitted infections (STIs) which have the potential of jeopardising their future reproductive lives if poorly treated. Objective: To determine the prevalence and health care seeking behaviour for STIs among secondary school adolescents in Ado, South-Western Nigeria. Subjects: Male and female adolescents aged 10–19 years. Methods: The study employed a descriptive cross-sectional design. A pre-tested, self-administered, semi-structured questionnaire was used to elicit information from 560 adolescents selected from public and private secondary schools in Ado Local Government Area (LGA) of Ekiti State, using a multistage sampling technique. The data were analyzed using descriptive and inferential statistics. Analyzed data were presented in the form of tables and charts. Results: Two hundred and fifty-one (47.1%) respondents had previous symptoms of STIs. The commonest symptoms among the males were penile discharge (30.5%), painful micturition (30.5%) and stomach pain with swollen testes (25.4%). The commonest symptoms among the females were genital itching (27.6%), genital sores (14.1%) and painful micturition (13.1%). Only 26.7% sought treatment from a health facility; 37.0% did nothing; 15.9% went to patent medicine stores, 10.0% used herbs; the rest did self-medication or prayed. Conclusion: The prevalence of STI symptoms among adolescents in Ado-Ekiti is high but their health care seeking behaviour is poor. Health education about the dangers of untreated STI and the importance of seeking treatment early, targeted at adolescents, should be intensified.

Patient demand on sexual health services in the United Kingdom is so high that many services have introduced online screening to accommodate more patients. There are concerns that these services may not be accessible to all. This service evaluation was undertaken to determine whether online screening is accessible by those patients most at need by comparing the demographics and number of asymptomatic chlamydial infections detected online and in clinic. No difference was found in the age nor level of deprivation, demonstrating that online services are an accessible way to screen for sexually transmitted infections without overburdening established services.
