This practical guide is a tool designed for graziers to use in their day-to-day decision-making about livestock nutritional needs, feeding options, condition and health. Pasture supplements are expensive and should not be wasted, yet under-nutrition has an even greater economic effect resulting from low conception and progeny survival rates, poor growth rates, failure to meet market targets and tender fleeces in sheep. ABSTRACT Mycobacterium avium subsp. paratuberculosis (basonym M. paratuberculosis) is the causative agent of paratuberculosis, a chronic enteritis of ruminants. To control the considerable economic effect that paratuberculosis has on the livestock industry, a vaccine that induces protection with minimal side effects is required. We employed transposon mutagenesis and allelic exchange to develop three potential vaccine candidates, which were then tested for virulence with macrophages, mice, and goats. All three models identified the WAg906 mutant as being the most attenuated, but some differences in the levels of attenuation were evident among the models when testing the other strains. In a preliminary mouse vaccine experiment, limited protection was induced by WAg915, as evidenced by a reduced bacterial load in spleens and livers 12 weeks following intraperitoneal challenge with M. paratuberculosis K10. While we found macrophages and murine models to be rapid and cost-effective alternatives for the initial screening of M. paratuberculosis mutants for attenuation, it appears necessary to do the definitive assessment of attenuation with a ruminant model.

Changing the qualification structure of contractors for the subsystem (function) of analytical processing of technical and economic information data (systematic economic analysis, evaluation) at industrial enterprises and scientific and industrial associations, should also strive to ensure that the economic efficiency of economic and analytical work in the sense of its scientific level, results achieved, reliability and timing was highest, and the use of potential staff capabilities was greatest. It is about optimizing the activity of subsystem staff. In this regard, one of the thematic issues of organizational support for the subsystem (function) of analytical processing of technical and economic information data (systematic economic analysis and evaluation) is the development of a system of measures that ensure the subordination of activities of personnel to the goals set both before the subsystem and before the system. Management of an industrial enterprise and a scientific-industrial association as a whole, and of those that certainly contribute to the most cost-effective implementation of this economic activity in set direction of development. The whole range of problems of comprehensive provision in functionally developed systems of strategic management of special purpose scientific and industrial associations and enterprises of the industry cost-effective activities of personnel of the subsystem (function) of analytical processing of technical and economic information data (systematic economic analysis and evaluation) should be solved on the basis of system approach. The shift in the process of designing the center of gravity of research towards the utilization of the capabilities of the human component of the subsystem (function) is one of the most important and indispensable conditions for the effectiveness of its organizational support, and accordingly, to obtain a given economic (economic) effect from the functional development of the subsystem (function) of analytical processing-economic information empiric (data) (systematic economic analysis and evaluation).

Abstract BackgroundIncreasing global wood demands have necessitated an overall push for cost-effective methods of vegetative propagation using cuttings. Vegetative plant propagation through cuttings although a breakthrough is mostly hampered with the inability of cuttings to root effectively to achieve maximum yield. Ten clones each of Eucalyptus grandis x Eucalyptus urophylla (E. grandis x E. urophylla) and Eucalyptus pellita (E. pellita) were treated with Indole-3-butyric acid (IBA) concentrations of 0 ppm (control), 2000 ppm, 3000 ppm and 4000 ppm respectively.ResultsOverall mean survival of cuttings was low (< 50%) for all concentrations of IBA used in this research. Survival rates of individual clones however varied from clone to clone. E. grandis x E. urophylla cuttings had the highest mean survival (43.39%) when treated with 2000 ppm of IBA and lowest mean survival (37.86%) when treated with 0 ppm. Results from the survival rates of E. grandis × E. Urophylla and Eucalyptus pellita cuttings were subjected to an Analysis of Variance test to identify any significant differences between hormone concentration and cutting survival. ANOVA results indicated that differences between cutting–IBA concentrations for E. grandis × E. Urophylla were significant (p=1.85x10-08) which is lower than standard p value (p=0.05) used in the test.For E. pellita (P) cuttings the highest mean survival (27.14%) was achieved when treated with 2000 ppm of Indole-3-butyric acid (IBA). Individual clones however showed significant differences in survival. E. pellita clone P2 achieved the highest survival (52.68%) in the control experiment (0 ppm). Some clones i.e P51 and P82 achieved survivals of less than 10% in all four levels of IBAConclusionThe best surviving Eucalyptus grandis x Eucalyptus urophylla clone (GH11) achieved a mean survival rate of 75% when the GH11 basal cutting was treated with 2000 ppm IBA. The results indicated that the production of clones with survival rates of less than 20% should be discarded since it makes no economic sense clones with survival rates of more than 40% should be planted out in the field to compare field performance with greenhouse survival.

Background The incidence of cardiac arrests per year in the United States continues to increase, yet in-hospital cardiac arrest survival rates significantly vary between hospitals. Current methods of training are expensive, time consuming, and difficult to scale, which necessitates improvements in advanced cardiac life support (ACLS) training. Virtual reality (VR) has been proposed as an alternative or adjunct to high-fidelity simulation (HFS) in several environments. No evaluations to date have explored the ability of a VR program to examine both technical and behavioral skills and demonstrate a cost comparison. Objective This study aimed to explore the utility of a voice-based VR ACLS team leader refresher as compared with HFS. Methods This prospective observational study performed at an academic institution consisted of 25 postgraduate year 2 residents. Participants were randomized to HFS or VR training and then crossed groups after a 2-week washout. Participants were graded on technical and nontechnical skills. Participants also completed self-assessments about the modules. Proctors were assessed for fatigue and task saturation, and cost analysis based on local economic data was performed. Results A total of 23 of 25 participants were included in the scoring analysis. Fewer participants were familiar with VR compared with HFS (9/25, 36% vs 25/25, 100%; P<.001). Self-reported satisfaction and utilization scores were similar; however, significantly more participants felt HFS provided better feedback: 99 (IQR 89-100) vs 79 (IQR 71-88); P<.001. Technical scores were higher in the HFS group; however, nontechnical scores for decision making and communication were not significantly different between modalities. VR sessions were 21 (IQR 19-24) min shorter than HFS sessions, the National Aeronautics and Space Administration task load index scores for proctors were lower in each category, and VR sessions were estimated to be US $103.68 less expensive in a single-learner, single-session model. Conclusions Utilization of a VR-based team leader refresher for ACLS skills is comparable with HFS in several areas, including learner satisfaction. The VR module was more cost-effective and was easier to proctor; however, HFS was better at delivering feedback to participants. Optimal education strategies likely contain elements of both modalities. Further studies are needed to examine the utility of VR-based environments at scale.

Background: Sorghum-Soy Blend (SSB) and Sorghum-Cowpea Blend (SCB) fortified blended food aid porridge products were developed as alternatives to Corn-Soy Blend Plus (CSB+) and Super Cereal Plus (SC+), the most widely used fortified blended food aid products. However, the cost and nutrient cost-effectiveness of these products procured from different geographical areas have not been determined. Objective: The objective of this study is to determine the nutrient cost-effectiveness of SSB and SCB compared to existing fortified blended foods. Methods: Nutritional data as well as ingredient, processing, and transportation cost for SSB, SCB, and existing fortified blended foods were compiled. Using the omega value, the ratio of the fortified blended food’s Nutrient Value Score to the total cost of the fortified blended food divided by an identical ratio of a different fortified blended food or the same fortified blended food produced in a different country and the nutrient cost-effectiveness of each of the fortified blended foods procured in the United States and several African countries were determined. Results: Both CSB+ and SC+ are less expensive than SSB and SCB, but they also have lower Nutrient Value Scores of 7.7 and 8.6, respectively. However, the omega values of CSB+ and SC+ are all above 1 when compared to SSB and SCB, suggesting that the existing fortified blended foods are more nutrient cost-effective. Conclusions: Comparing the nutrient cost-effectiveness of various food aid products could provide valuable information to food aid agencies prior to making procurement decisions.

e19022 Background: The burden of pediatric cancer is more than 300.000 cases per year in children aged 0-19 years globally. Turkey faces almost 4000 cases per yr in the same age group. The survival rates exceeded 80% in high income countries although it is still a challenge in LMICs at different level of low survivals. The availability, accessibility & affordability of advanced techniques such as interventional procedures are major challenges for successful treatment of pediatric cancers in LMICs. Methods: 279 sequentially diagnosed cancer in children aged 0-18 years in a single institution in 2018 were included. Tumors were classified according to the ICCC-3. Leukemias were not included. The types of interventional & surgical procedures used for diagnosis were recorded. The distribution of procedures according to the type of malignancies were examined & descriptive characteristics of patients were compared. Results: The median age was 58/12yrs, M/F=153/126. The most common tumors were CNS tumors followed by lymphomas & neuroblastoma. Among all, surgery was performed in 189 (67.8) children whereas 45 (16.1%) patients were diagnosed by interventional radiology procedures. Radiological diagnosis was made in 42 (15.1%) children. Conclusions: Early diagnosis & access to best available care is the most essential part of the pediatric cancer control. Current global initiatives have a focus on how to improve the survival in LMICs. Here we report the use of surgery vs interventional radiological procedures for the diagnosis of pediatric tumors. The efficient & timely use of the diagnostic procedures save the patients lives in a cost effective ways with less morbidity & mortality. Since there is limited data from the LMICs, we suggest to the pediatric oncology community to do more detailed evidenced based research for the diagnosis pediatric cancers. [Table: see text]

Vasa praevia is described as the unprotected fetal vessels traversing through the placental membranes over the cervical os, below the fetal presenting part and unprotected by placental tissue or the umbilical cord. It is often not detected antenatally and presents with painless bleeding and rapid fetal compromise after spontaneous or artificial rupture of the membranes, which causes trauma to these vessels. It is a rare condition affecting one in 2500 pregnancies and has a reported perinatal mortality rate of up to 60%. More than 80% of cases of vasa praevia have at least one risk factor for the condition and it has been shown that identification of vasa praevia antenatally results in fetal survival rates of up to 97%. Risk factors include placenta praevia, velamentous cord insertion, bi-lobed placenta, succenturiate lobe, assisted reproduction and multiple pregnancy. Screening for vasa praevia with transvaginal ultrasound has been shown to be sensitive and cost effective when used in a targeted population where risk factors are present. Here, we present the antenatal assessment and management of a case of vasa praevia detected during the routine 20-week anatomy scan of a healthy primigravida, which resulted in elective caesarean section delivery of a healthy baby at 36 weeks' gestation. The pathophysiology of vasa praevia is discussed and the methods and role of screening for this rare but serious condition are appraised.

Carcinoma of the lung is the leading cause of death from malignancy in this country. Twenty percent of patients with lung carcinoma are asymptomatic early in the course of the disease. Survival rates are much better when lung carcinoma is discovered at the asymptomatic or radiographically occult stage, therefore improved screening methods should be investigated. Many tumors in the tracheobronchial tree shed malignant cells into the bronchi and sputum cytology has been of value in the early diagnosis of carcinoma of the tracheobronchial tree. Thousands of patients are intubated daily. During endotracheal anesthesia, secretions are stimulated in the tracheobronchial tree and endotracheal or tracheostomy tubes are coated with these secretions. In a preliminary study, the cytology examination of the secretions adherent to 50 endotracheal tubes suggests that endotracheal and tracheostomy tube cytology will prove to be a reliable, noninvasive and cost-effective means of screening high risk patients for occult primary carcinomas of the lung and upper aerodigestive tract.

There is widespread evidence that beef cattle land managers in Queensland are using stocking rates for perennial pastures that are substantially higher than recommended guidelines, and some indication that these decisions are motivated by perceived financial and economic benefits. Considerable effort has been, and is currently being, applied by public-sector organisations to encourage producers to reduce grazing pressure from beef cattle across Queensland’s pastoral lands. A better understanding of the relationships among stocking rate, land condition and profitability of beef-grazing enterprises is imperative to better inform cattle producers and policy makers. The present study assessed the effect of grazing pressure and land condition on the productivity and profitability of a steer-turnover enterprise utilising buffel grass (Cenchrus ciliaris) pastures in central Queensland. A property-level, regionally relevant herd model was used to determine whole-of-business productivity and profitability over a 30-year investment period. Growth paths for steers from weaning to marketing were developed for 16 scenarios encompassing a range of pasture-utilisation rates (30%, 35% and 50% of annual biomass growth), land condition (A, B and C) and market targets (feedlot entry at 474 kg or slaughter at 605 kg). The economic effect of each scenario was assessed by comparison to a base scenario of 30% pasture utilisation and turn-off of slaughter steers. Our analyses demonstrated a large economic advantage from increasing grazing pressure above 30% utilisation for buffel grass pastures, even with assumptions of declining land condition and animal performance. For instance, producing slaughter steers under a 50% pasture-utilisation regime with a continuous decline in land condition from A to C (and, hence, productivity) over Years 10–30 was AU$21 772/annum more profitable than was a 30% pasture-utilisation strategy, which is widely recommended as closer to a long-term, safe utilisation rate. The present research has provided insights into the relationship between grazing pressure and economic returns of beef producers over the medium term. However, it should be considered as a scoping study due to the paucity of data for effects of utilisation rate on the productivity of buffel grass pastures and, hence, on land-condition rating. Further research is required to better understand the effects of utilisation rate of buffel grass, and other sown pasture grass and legume species, on plant biomass production, plant-diet quality for cattle, land-condition decline and cattle productivity.

ObjectiveTo assess the efficacy and cost-effectiveness of modulated electrohyperthermia (mEHT) concurrent to dose-dense temozolomide (ddTMZ) 21/28 days regimen versus ddTMZ 21/28 days alone in patients with recurrent glioblastoma (GBM).DesignA cohort of 54 patients with recurrent GBM treated with ddTMZ+mEHT in 2000–2005 was systematically retrospectively compared with five pooled ddTMZ 21/28 days cohorts (114 patients) enrolled in 2008–2013.ResultsThe ddTMZ+mEHT cohort had a not significantly improved mean survival time (mST) versus the comparator (p=0.531) after a significantly less mean number of cycles (1.56 vs 3.98, p<0.001). Effect-to-treatment analysis (ETA) suggests that mEHT significantly enhances the efficacy of the ddTMZ 21/28 days regimen (p=0.011), with significantly less toxicity (no grade III–IV toxicity vs 45%–92%, p<0.0001). An estimated maximal attainable median survival time is 10.10 months (9.10–11.10). Cost-effectiveness analysis suggests that, unlike ddTMZ 21/28 days alone, ddTMZ+mEHT is cost-effective versus the applicable cost-effectiveness thresholds €US$25 000–50 000/quality-adjusted life year (QALY). Budget impact analysis suggests a significant saving of €8 577 947/$11 201 761 with 29.1–38.5 QALY gained per 1000 patients per year. Cost-benefit analysis suggests that mEHT is profitable and will generate revenues between €3 124 574 and $6 458 400, with a total economic effect (saving+revenues) of €5 700 034 to $8 237 432 per mEHT device over an 8-year period.ConclusionsOur ETA suggests that mEHT significantly improves survival of patients receiving the ddTMZ 21/28 days regimen. Economic evaluation suggests that ddTMZ+mEHT is cost-effective, budget-saving and profitable. After confirmation of the results, mEHT could be recommended for the treatment of recurrent GBM as a cost-effective enhancer of ddTMZ regimens, and, probably, of the regular 5/28 days regimen. mEHT is applicable also as a single treatment if chemotherapy is impossible, and as a salvage treatment after the failure of chemotherapy.
