Three Dimensional Echocardiography

Development of interventional methods has revolutionized the treatment of structural cardiac diseases. Given the complexity of structural interventions and the anatomical variability of various structural defects, novel imaging techniques have been implemented in the current clinical practice for guiding the interventional procedure and for selection of the device to be used. Three– dimensional echocardiography is the most used imaging method that has improved the threedimensional assessment of cardiac structures, and it has considerably reduced the cost of complications derived from malalignment of interventional devices. Assessment of cardiac structures with the use of angiography holds the advantage of providing images in real time, but it does not allow an anatomical description. Transesophageal Echocardiography (TEE) and intracardiac ultrasonography play major roles in guiding Atrial Septal Defect (ASD) or Patent Foramen Ovale (PFO) closure and device follow-up, while TEE is the procedure of choice to assess the flow in the Left Atrial Appendage (LAA) and the embolic risk associated with a decreased flow. On the other hand, contrast CT and MRI have high specificity for providing a detailed description of structure, but cannot assess the flow through the shunt or the valvular mobility. This review aims to present the role of modern imaging techniques in pre-procedural assessment and intraprocedural guiding of structural percutaneous interventions performed to close an ASD, a PFO, an LAA or a patent ductus arteriosus.

AbstractUnderstanding the structure and function of vasculature in the brain requires us to monitor distributed hemodynamics at high spatial and temporal resolution in three-dimensional (3D) volumes in vivo. Currently, a volumetric vasculature imaging method with sub-capillary spatial resolution and blood flow-resolving speed is lacking. Here, using two-photon laser scanning microscopy (TPLSM) with an axially extended Bessel focus, we capture volumetric hemodynamics in the awake mouse brain at a spatiotemporal resolution sufficient for measuring capillary size and blood flow. With Bessel TPLSM, the fluorescence signal of a vessel becomes proportional to its size, which enables convenient intensity-based analysis of vessel dilation and constriction dynamics in large volumes. We observe entrainment of vasodilation and vasoconstriction with pupil diameter and measure 3D blood flow at 99 volumes/second. Demonstrating high-throughput monitoring of hemodynamics in the awake brain, we expect Bessel TPLSM to make broad impacts on neurovasculature research.

Positron Emission Tomography (PET) produces a three dimensional spatial distribution of positron-electron annihilations within an image volume. Various positron emitters are available for use in aqueous, organic and liquid metal flows. Preliminary experiments at the University of Tennessee at Knoxville (UTK) injected small flows of PET tracer into a bulk water flow in a four rod bundle. The trajectory and diffusion of the tracer in the bulk flow were then mapped using a PET scanner. A spatial resolution of 1.4 mm is achieved with current preclinical Micro-PET imaging equipment resulting in 200 MB 3D activity fields. A time resolved 3-D spatial activity profile was also measured. The PET imaging method is especially well suited to complex geometries where traditional optical methods such as LDV and PIV are difficult to apply. PET methods are uniquely useful for imaging in opaque fluids, opaque pressure boundaries, and multiphase studies. Several commercial and shareware Computational Fluid Dynamics (CFD) codes are currently used for science and engineering analysis and design. These codes produce detailed three dimensional flow predictions. The models produced by these codes are often difficult to validate. The development of this experimental technique offers a modality for the comparison of CFD outcomes with experimental data. Developed data sets from PET can be used in verification and validation exercises of simulation outcomes.

Abstract Background: MR fingerprinting (MRF) is a novel imaging method proposed for the diagnosis of Multiple Sclerosis (MS). This study aims to determine if MR Fingerprinting (MRF) relaxometry can differentiate frontal normal appearing white matter (F-NAWM) and splenium in patients diagnosed with MS as compared to controls and to characterize the relaxometry of demyelinating plaques relative to the time of diagnosis.Methods: Three-dimensional (3D) MRF data were acquired on a 3.0T MRI system resulting in isotropic voxels (1x1x1mm3) and a total acquisition time of 4min 38s. Data were collected on 18 subjects paired with 18 controls. Regions of interested were drawn over MRF-derived T1 relaxometry maps encompassing selected MS lesions, F-NAWM and splenium. T1 and T2 relaxometry features from those segmented areas were used to classify MS lesions from F-NAWM and splenium with T-distributed stochastic neighbor embedding algorithms (T-SNE). Partial least squares discriminant analysis (PLS-DA) was performed to discriminate NAWM and Splenium in MS compared with controls. Results: Mean out-of-fold machine learning prediction accuracy for discriminant results between MS patients and controls for F-NAWM was 65% and approached 90% for the splenium. There was significant positive correlation between time since diagnosis and MS lesions mean T2 (p=0.015), minimum T1 (p=0.03) and negative correlation with splenium uniformity (p=0.04). Perfect discrimination (AUC=1) was achieved between selected features from MS lesions and F-NAWM.Conclusions: 3D-MRF has the ability to differentiate between MS and controls based on relaxometry properties from the F-NAWM and splenium. Whole brain coverage allows the assessment of quantitative properties within lesions that provide chronological assessment of the time from MS diagnosis.

Imaging methods are of great importance for diagnosis and treatment in dentistry. With technological advances, great progress has been made in these methods. Over time, 3-dimensional (3-D) imaging has replaced 2-dimensional, thereby providing examination of objects in all directions. Of these methods, which play an important role in the clinical evaluation of patients, cone-beam computed tomography (CBCT) is the newest and most advanced imaging method. This method will revolutionize dental in comparison with conventional CT, it has several advantages, including a shorter scanning time, low radiation dose, low cost and the acquisition of high-resolution images. With 3-D imaging technology, this method has introduced the possibility of applying several procedures from diagnosis in the maxillofacial region to operative and surgical procedures. AbstractThree-dimensional (3D) structures of the genome are dynamic, heterogeneous and functionally important. Live cell imaging has become the leading method for chromatin dynamics tracking. However, existing CRISPR- and TALE-based genomic labeling techniques have been hampered by laborious protocols and low signal-to-noise ratios (SNRs), and are thus mostly applicable to repetitive sequences. Here, we report a versatile CRISPR/Casilio-based imaging method, with an enhanced SNR, that allows for one nonrepetitive genomic locus to be labeled using a single sgRNA. We constructed Casilio dual-color probes to visualize the dynamic interactions of cohesin-bound elements in single live cells. By forming a binary sequence of multiple Casilio probes (PISCES) across a continuous stretch of DNA, we track the dynamic 3D folding of a 74kb genomic region over time. This method offers unprecedented resolution and scalability for delineating the dynamic 4D nucleome.One Sentence SummaryCasilio enables multiplexed live cell imaging of nonrepetitive DNA loci for illuminating the real-time dynamics of genome structures.

Despite advances in diagnosis, imaging methods, and medical and surgical interventions, prosthetic valve endocarditis (PVE) remains an extremely serious and potentially fatal complication of heart valve surgery. Characteristic changes of PVE are more difficult to detect by transthoracic echocardiography (TTE) than those involving the native valve. We reviewed advances in transesophageal echocardiography (TEE) in the diagnosis of PVE. Three-dimensional (3D) TEE is becoming an increasingly available imaging method combined with two-dimensional TEE. It contributes to faster and more accurate diagnosis of PVE, assessment of PVE-related complications, monitoring effectiveness of antibiotic treatment, and determining optimal time for surgery, sometimes even before or without previous TTE. In this article, we present advances in the treatment of patients with mitral PVE due to 3D TEE application.

✓ The utility of three-dimensional anisotropy contrast (3DAC) magnetic resonance (MR) axonography, a method sensitive to neuronal fibers and their directionality, was investigated in the clinical setting using a 3-tesla MR imaging system based on a General Electric Signa platform. The study focused on healthy volunteers and patients with common structural central nervous system disorders, namely chronic infarction, brainstem cavernous hemangioma, supratentorial meningioma, and astrocytoma. Three orthogonal anisotropic diffusion-weighted images were first obtained. Three primary colors were each assigned to a diffusion-weighted image, respectively, and the images were subsequently combined into a single-color image in full-color spectrum (3DAC MR axonography image). Fiber-tract definition in the cerebral peduncle of the midbrain of healthy volunteers showed intersubject variation, with two general patterns recognized: dispersed (60% of cases) and compact (40% of cases). Pathological alterations in the fiber tracts were readily identified in cases involving wallerian degeneration of the pyramidal tract, as illustrated in the cases of chronic infarction. Displacement of major tracts, such as the medial lemniscus or corticospinal tract, as well as fiber directionality, was also easily recognized in cases of mass lesions. As an imaging method uniquely capable of providing information regarding axonal connectivity, 3DAC MR axonography appears to have promising potential for routine clinical application.

In order to study the bubble morphology, a novel experimental and numerical approach was implemented in this research focusing on the analysis of a transparent throttle valve made by Polymethylmethacrylate (PMMA) material. A feature-based algorithm was written using the MATLAB software, allowing the 2D detection and three-dimensional (3D) reconstruction of bubbles: collapsing and clustered ones. The valve core, being an important part of the throttle valve, was exposed to cavitation; hence, to distinguish it from the captured frames, the faster region-based convolutional neural network (R-CNN) algorithm was used to detect its morphology. Additionally, the main approach grouping the above listed techniques was implemented using an optimized virtual stereo vision arrangement of one camera and five plane mirrors. The results obtained during this study validated the robust algorithms and optimization applied.
