Speech Identification Difficulties of Hearing-Impaired Elderly Persons

This study examined the performance of four subject groups on several temporally based measures of auditory processing and several measures of speech identification. The four subject groups were (a) young normal-hearing adults; (b) hearing-impaired elderly subjects ranging in age from 65 to 75 years; (c) hearing-impaired elderly adults ranging in age from 76 to 86 years; and (d) young normal-hearing listeners with hearing loss simulated with a spectrally shaped masking noise adjusted to match the actual hearing loss of the two elderly groups. In addition to between-group analyses of performance on the auditory processing and speech identification tasks, correlational and regression analyses within the two groups of elderly hearing-impaired listeners were performed. The results revealed that the threshold elevation accompanying sensorineural hearing loss was the primary factor affecting the speech identification performance of the hearing-impaired elderly subjects both as groups and as individuals. However, significant increases in the proportion of speech identification score variance accounted for were obtained in the elderly subjects by including various measures of auditory processing.

The role that sensorineural hearing loss plays in the speech-recognition difficulties of the hearing-impaired elderly is examined. One approach to this issue was to make between-group comparisons of performance for three groups of subjects: (a) young normal-hearing adults; (b) elderly hearing-impaired adults; and (c) young normal-hearing adults with simulated sensorineural hearing loss equivalent to that of the elderly subjects produced by a spectrally shaped masking noise. Another approach to this issue employed correlational analyses to examine the relation between audibility and speech recognition within the group of elderly hearing-impaired subjects. An additional approach was pursued in which an acoustical index incorporating adjustments for threshold elevation was used to examine the role audibility played in the speech-recognition performance of the hearing-impaired elderly. A wide range of listening conditions was sampled in this experiment. The conclusion was that the primary determiner of speech-recognition performance in the elderly hearing-impaired subjects was their threshold elevation.

AbstractAuditory processing is affected by advancing age and hearing loss, but the underlying mechanisms are still unclear. We investigated the effects of age and hearing loss on temporal processing of naturalistic stimuli in the auditory system. We analysed neural phase-locking to the fundamental frequency of the voice (f0) in 54 normal-hearing and 14 hearing-impaired adults between 17 and 82 years old. We found that both subcortical and cortical neural sources contributed to the responses. Results indicated that advancing age was related to smaller responses with less cortical response contributions, consistent with an age-related decrease in neural phase-locking ability. Conversely, hearing impaired subjects displayed larger responses compared to age-matched normal hearing controls. This was due to additional cortical response contributions which were stronger for participants with more severe hearing loss. This is consistent with the recruitment of additional cortical sources for auditory processing in persons with hearing impairment.

The purpose of this study was to determine if peripheral hearing loss of varying degrees in elderly subjects affected performance on monotic auditory processing disorder (APD) tests. A battery of monotic APD tests was administered to a group of well-educated and high-functioning older adults who were divided into three subgroups based on hearing acuity but similar in age: (1) normal hearing out to 4000 Hz with a slight high-frequency slope above that point, (2) normal hearing in the speech range but greater high-frequency loss (sloping configuration), and (3) hearing loss in both the low and high frequencies (low/high). The findings documented that subjects with normal hearing in the speech range performed well on all the APD tests. The subjects in the two hearing loss groups, however, performed more poorly on certain tests. The low/high loss subjects did significantly poorer than did the sloping subjects. These data suggest that low/high-frequency peripheral hearing loss is a factor for poor performance on certain monotic APD tests. Results further showed that when cognitive ability and presentation level are held constant, chronological age does not appear to be a contributing factor to performance on the majority of these monotic APD tests. If APD tests are to be administered to elder subjects, peripheral hearing loss configuration needs to be documented. For subjects with low/high-frequency losses, the tester needs to be aware that serious contamination of the results may occur. El propósito de este estudio fue determinar si las pérdidas auditivas periféricas de diferentes grados en sujetos mayores afectaban el desempeño en pruebas monóticas para trastornos de procesamiento auditivo (APD). Una batería de pruebas monóticas para APD fue administrada a un grupo de adultos mayores bien educados y con alto nivel de funcionamiento, quienes fueron divididos en tres subgrupos con base en su agudeza auditiva, pero con edad similares: (1) audición normal hasta 4000 Hz pero con una leve pendiente en las frecuencias agudas por encima de ese punto, (2) audición normal en el rango del lenguaje pero con una pérdida mayor en frecuencias agudas (configuración en caída), y (3) pérdida auditiva tanto en frecuencia agudas como en altas (baja/alta). Los hallazgos documentaron que los sujetos con audición normal en el rango del lenguaje se desempeñaron bien en todas las pruebas APD. Los sujetos en los dos grupos de pérdida auditiva, sin embargo, se desempeñaron más pobremente en ciertas pruebas. Los sujetos de alta/baja rindieron significativamente peor que los sujetos de configuración en caída. Estos datos sugieren que la hipoacusia periférica en frecuencias altas/bajas es un factor de pobre desempeño en las pruebas monóticas en APD. Los resultados mostraron además que cuando la habilidad cognitiva y los niveles de presentación se mantienen constantes, la edad cronológica no parece ser un factor modificador del desempeño en la mayoría de estas pruebas monóticas en APD. Si se han de administrar estas pruebas para APD a adultos mayores, la configuración de la pérdida auditiva periférica debe ser documentada. Para sujetos con pérdidas auditivas en frecuencias bajas/altas el evaluador debe ser conciente de que puede ocurrir una seria contaminación de los resultados.

The auditory system allows the estimation of the distance to sound-emitting objects using multiple spatial cues. In virtual acoustics over headphones, a prerequisite to render auditory distance impression is sound externalization, which denotes the perception of synthesized stimuli outside of the head. Prior studies have found that listeners with mild-to-moderate hearing loss are able to perceive auditory distance and are sensitive to externalization. However, this ability may be degraded by certain factors, such as non-linear amplification in hearing aids or the use of a remote wireless microphone. In this study, 10 normal-hearing and 20 moderate-to-profound hearing-impaired listeners were instructed to estimate the distance of stimuli processed with different methods yielding various perceived auditory distances in the vicinity of the listeners. Two different configurations of non-linear amplification were implemented, and a novel feature aiming to restore a sense of distance in wireless microphone systems was tested. The results showed that the hearing-impaired listeners, even those with a profound hearing loss, were able to discriminate nearby and far sounds that were equalized in level. Their perception of auditory distance was however more contracted than in normal-hearing listeners. Non-linear amplification was found to distort the original spatial cues, but no adverse effect on the ratings of auditory distance was evident. Finally, it was shown that the novel feature was successful in allowing the hearing-impaired participants to perceive externalized sounds with wireless microphone systems.

This study examined auditory temporal sensitivity in young adult and elderly listeners using psychophysical tasks that measured duration discrimination. Listeners in the experiments were divided into groups of young and elderly subjects with normal hearing sensitivity and with mild-to-moderate sloping sensorineural hearing loss. Temporal thresholds in all tasks were measured with an adaptive forced-choice procedure using tonal stimuli centered at 500 Hz and 4000 Hz. Difference limens for duration were measured for tone bursts (250 msec reference duration) and for silent intervals between tone bursts (250 msec and 6.4 msec reference durations). Results showed that the elderly listeners exhibited diminished duration discrimination for both tones and silent intervals when the reference duration was 250 msec. Hearing loss did not affect these results. Discrimination of the brief temporal gap (6.4 msec) was influenced by age and hearing loss, but these effects were not consistent across all listeners. Effects of stimulus frequency were not evident for most of the duration discrimination conditions.

Just as the ear trumpet once symbolized the elderly, so do contemporary approaches to hearing loss in the aged reflect many of the major themes in geriatrics and gerontology today. This paper begins by describing the National Institute on Aging (NIA) with particular emphasis on activities relevant to hearing in the elderly. Next, several areas of research interest, including the typology of presbycusis and related conditions, the epidemiology of auditory impairment in old age, the design of testing and research, and the rehabilitation of the hearing-impaired elderly, are addressed. The NIA and the National Institute of Neurological and Communicative Disorders and Stroke (NINCDS) are coordinating their efforts to stimulate investigation of these and related topics.

The effect of age on accuracy of prediction of hearing impairment with the bivariate-plotting procedure was investigated in 72 normal-hearing subjects aged 20–69 years and in 86 sensorineural hearing-impaired subjects aged 20–83 years. The predictive accuracy with the bivariate-plotting procedure improved markedly when the data from subjects over 44 years of age were excluded from the bivariate plot. The predictive accuracy improved further when the construction of the line segments in the traditional bivariate plot was modified.

This study examined the characteristics of lower limb muscle activity in elderly persons after ergometric pedaling exercise for 1 month. To determine the effect of the exercise, surface electromyography (SEMG) of lower limb muscles was subjected to Daubechies-4 wavelet transformation, and mean wavelet coefficients were compared with the pre-exercise coefficients and the post-exercise coefficients in each wavelet level. The characteristics of muscle activity after pedaling exercise were also compared between the elderly subjects and young subjects. For the elderly subjects, the mean wavelet coefficients were significantly decreased in the tibialis anterior and the gastrocnemius medialis at wavelet levels of 3, 4, and 5 (125–62.5, 62.5–31.25, and 31.25–15.625 Hz, respectively), by pedaling exercise. However, the mean power of wavelet levels of 2 and 3 (250–125 and 125–62.5 Hz) within the rectus femoris and the biceps femoris were significantly increased in the young subjects. The effect of pedaling exercise is different from the effects of heavy-resistance training. It was suggested that the muscle coordination, motor unit (MU) firing frequency, and firing fiber type of lower limb muscles are changed with the different characteristics between elderly and young persons by pedaling exercise for 1 month.

The Modified Rhyme Test (MRT), recorded using natural speech and two forms of synthetic speech, DECtalk and Votrax, was used to measure both open-set and closed-set speech-recognition performance. Performance of hearing-impaired elderly listeners was compared to two groups of young normal-hearing adults, one listening in quiet, and the other listening in a background of spectrally shaped noise designed to simulate the peripheral hearing loss of the elderly. Votrax synthetic speech yielded significant decrements in speech recognition compared to either natural or DECtalk synthetic speech for all three subject groups. There were no differences in performance between natural speech and DECtalk speech for the elderly hearing-impaired listeners or the young listeners with simulated hearing loss. The normal-hearing young adults listening in quiet out-performed both of the other groups, but there were no differences in performance between the young listeners with simulated hearing loss and the elderly hearing-impaired listeners. When the closed-set identification of synthetic speech was compared to its open-set recognition, the hearing-impaired elderly gained as much from the reduction in stimulus/response uncertainty as the two younger groups. Finally, among the elderly hearing-impaired listeners, speech-recognition performance was correlated negatively with hearing sensitivity, but scores were correlated positively among the different talker conditions. Those listeners with the greatest hearing loss had the most difficulty understanding speech and those having the most trouble understanding natural speech also had the greatest difficulty with synthetic speech.

Consonant recognition was measured as a function of the degree of spectral resolution of the speech stimulus in normally hearing listeners and listeners with moderate sensorineural hearing loss. Previous work (Turner, Souza, and Forget, 1995) has shown that listeners with sensorineural hearing loss could recognize consonants as well as listeners with normal hearing when speech was processed to have only one channel of spectral resolution. The hypothesis tested in the present experiment was that when speech was limited to a small number of spectral channels, both normally hearing and hearing-impaired listeners would continue to perform similarly. As the stimuli were presented with finer degrees of spectral resolution, and the poorer-than-normal spectral resolving abilities of the hearing-impaired listeners became a limiting factor, one would predict that the performance of the hearing-impaired listeners would then become poorer than the normally hearing listeners. Previous research on the frequency-resolution abilities of listeners with mild-to-moderate hearing loss suggests that these listeners have critical bandwidths three to four times larger than do listeners with normal hearing. In the present experiment, speech stimuli were processed to have 1, 2, 4, or 8 channels of spectral information. Results for the 1-channel speech condition were consistent with the previous study in that both groups of listeners performed similarly. However, the hearing-impaired listeners performed more poorly than the normally hearing listeners for all other conditions, including the 2-channel speech condition. These results would appear to contradict the original hypothesis, in that listeners with moderate sensorineural hearing loss would be expected to have at least 2 channels of frequency resolution. One possibility is that the frequency resolution of hearing-impaired listeners may be much poorer than previously estimated; however, a subsequent filtered speech experiment did not support this explanation. The present results do indicate that although listeners with hearing loss are able to use the temporal-envelope information of a single channel in a normal fashion, when given the opportunity to combine information across more than one channel, they show deficient performance.
