Test of the Assumptions Underlying Comparative Hearing Aid Evaluations

Comparative hearing aid evaluations using NU-6 monosyllabic word lists were administered to adults with predominately high-frequency sensorineural hearing impairments who were randomly assigned to one of two experiments. In the first, three instruments were used that were electroacoustically similar and appropriate to the patients' hearing losses. In the second, the three hearing aids employed were electroacoustically quite different. Following an initial comparative hearing aid evaluation, the patients used the instruments during a trial-use week after which they ranked the aids in terms of benefit provided in daily communication. Following the trial-use week, the comparative hearing aid evaluation was repeated. The results suggest that significant interaid performance differences on the hearing aid evaluation are not likely to occur very often when the aids being evaluated are relatively homogeneous electroacoustically. In contrast, when electroacoustically heterogeneous instruments are evaluated, significant performance differences may occur frequently. Under such circumstance, however, the same instrument(s) would likely provide the best performance to most patients. The results further suggest that the reliability of standard monosyllabic word lists may not be adequate to detect typical interaid differences that occur in a comparative hearing aid evaluation and that the performance hierarchy is likely to change as the patient adjusts amplification. Finally, the comparative hearing aid evaluation will not be a good predictor of success in daily communication unless relatively large performance differences exist among the instruments.

Twenty students between the ages of 6 and 19 years who were receiving services for students with hearing impairments in a metropolitan, inner-city school system were trained to monitor their own hearing aids. This study investigated the effect of this training on the percentage of students who wore functional hearing aids. Ten of the students received fewer than 3 hours of instruction per day in the regular education setting and generally had hearing losses in the severe to profound range. The remaining 10 students received greater than 3 hours of instruction per day in the regular education setting and had hearing losses in the moderate to severe range. The findings indicated improved hearing aid function when students were actively involved in hearing aid maintenance programs. Recommendations are made concerning hearing aid maintenance in the schools.

Two types of special hearing aid have been developed recently to improve the reception of speech by profoundly deaf children. In a different way, each special system provides greater low-frequency acoustic stimulation to deaf ears than does a conventional hearing aid. One of the devices extends the low-frequency limit of amplification; the other shifts high-frequency energy to a lower frequency range. In general, previous evaluations of these special hearing aids have obtained inconsistent or inconclusive results. This paper reviews most of the published research on the use of special hearing aids by deaf children, summarizes several unpublished studies, and suggests a set of guidelines for future evaluations of special and conventional amplification systems.

A custom in-the-ear (ITE) hearing aid fitting was compared to two over-the-ear (OTE) hearing aid fittings for each of 9 subjects with mild to moderately severe hearing losses. Speech intelligibility via the three instruments was compared using the Speech Intelligibility Rating (SIR) test. The relationship between functional gain and coupler gain was compared for the ITE and the higher rated OTE instruments. The difference in input received at the microphone locations of the two types of hearing aids was measured for 10 different subjects and compared to the functional gain data. It was concluded that (a) for persons with mild to moderately severe hearing losses, appropriately adjusted custom ITE fittings typically yield speech intelligibility that is equal to the better OTE fitting identified in a comparative evaluation; and (b) gain prescriptions for ITE hearing aids should be adjusted to account for the high-frequency emphasis associated with in-the-concha microphone placement.

Persons with a high frequency hearing loss are difficult cases for whom to find suitable amplification. We have experienced some success with this problem in our Hearing Clinics using a specially designed earmold with a hearing aid. Thirty-five cases with high frequency hearing losses were selected from our clinical files for analysis of test results using standard, vented, and open earpieces. A statistical analysis of test results revealed that PB scores in sound field, using an average conversational intensity level (70 dB SPL), were enhanced when utilizing any one of the three earmolds. This result was due undoubtedly to increased sensitivity provided by the hearing aid. Only the open earmold used with a CROS hearing aid resulted in a significant improvement in discrimination when compared with the group’s unaided PB score under earphones or when comparing inter-earmold scores. These findings suggest that the inclusion of the open earmold with a CROS aid in the audiologist’s armamentarium should increase his flexibility in selecting hearing aids for persons with a high frequency hearing loss.

Background: Children with steeply sloping sensorineural hearing loss (SNHL) lack access to critical high-frequency cues despite the use of advanced hearing aid technology. In addition, their auditory-only aided speech perception abilities often meet Food and Drug Administration criteria for cochlear implantation. Purpose: The objective of this study was to describe hearing preservation and speech perception outcomes in a group of young children with steeply sloping SNHL who received a cochlear implant (CI). Research Design: Retrospective case series. Study Sample: Eight children with steeply sloping postlingual progressive SNHL who received a unilateral traditional CI at Seattle Children’s Hospital between 2009 and 2013 and had follow-up data available up to 24 mo postimplant were included. Data Collection and Analysis: A retrospective chart review was completed. Medical records were reviewed for demographic information, preoperative and postoperative behavioral hearing thresholds, and speech perception scores. Paired t tests were used to analyze speech perception data. Hearing preservation results are reported. Results: Rapid improvement of speech perception scores was observed within the first month postimplant for all participants. Mean monosyllabic word scores were 76% and mean phoneme scores were 86.7% at 1-mo postactivation compared to mean preimplant scores of 19.5% and 31.0%, respectively. Hearing preservation was observed in five participants out to 24-mo postactivation. Two participants lost hearing in both the implanted and unimplanted ear, and received a sequential bilateral CI in the other ear after progression of the hearing loss. One participant had a total loss of hearing in only the implanted ear. Results reported in this article are from the ear implanted first. Bilateral outcomes are not reported. Conclusions: CIs provided benefit for children with steeply sloping bilateral hearing loss for whom hearing aids did not provide adequate auditory access. In our cohort, significant improvements in speech understanding occurred rapidly postactivation. Preservation of residual hearing in children with a traditional CI electrode is possible.

AbstractSloping hearing loss imposes limits on audibility for high-frequency sounds in many hearing aid users. Signal processing algorithms that shift high-frequency sounds to lower frequencies have been introduced in hearing aids to address this challenge by improving audibility of high-frequency sounds.This study examined speech perception performance, listening effort, and subjective sound quality ratings with conventional hearing aid processing and a new frequency-lowering signal processing strategy called frequency composition (FC) in adults and children.Participants wore the study hearing aids in two signal processing conditions (conventional processing versus FC) at an initial laboratory visit and subsequently at home during two approximately six-week long trials, with the order of conditions counterbalanced across individuals in a double-blind paradigm.Children (N = 12, 7 females, mean age in years = 12.0, SD = 3.0) and adults (N = 12, 6 females, mean age in years = 56.2, SD = 17.6) with bilateral sensorineural hearing loss who were full-time hearing aid users.Individual performance with each type of processing was assessed using speech perception tasks, a measure of listening effort, and subjective sound quality surveys at an initial visit. At the conclusion of each subsequent at-home trial, participants were retested in the laboratory. Linear mixed effects analyses were completed for each outcome measure with signal processing condition, age group, visit (prehome versus posthome trial), and measures of aided audibility as predictors.Overall, there were few significant differences in speech perception, listening effort, or subjective sound quality between FC and conventional processing, effects of listener age, or longitudinal changes in performance. Listeners preferred FC to conventional processing on one of six subjective sound quality metrics. Better speech perception performance was consistently related to higher aided audibility.These results indicate that when high-frequency speech sounds are made audible with conventional processing, speech recognition ability and listening effort are similar between conventional processing and FC. Despite the lack of benefit to speech perception, some listeners still preferred FC, suggesting that qualitative measures should be considered when evaluating candidacy for this signal processing strategy.

AbstractThe American Speech-Language-Hearing Association (ASHA) and American Academy of Audiology (AAA) have created Best Practice Guidelines for fitting hearing aids to adult patients. These guidelines recommend using real-ear measures (REM) to verify that measured output/gain of hearing aid(s) match a validated prescriptive target. Unfortunately, approximately 70–80% of audiologists do not routinely use REM when fitting hearing aids, instead relying on a manufacturer default “first-fit” setting. This is problematic because numerous studies report significant differences in REM between manufacturer first-fit and the same hearing aids using a REM or programmed-fit. These studies reported decreased prescribed gain/output in the higher frequencies for the first-fit compared with the programmed fit, which are important for recognizing speech. Currently, there is little research in peer-reviewed journals reporting if differences between hearing aids fitted using a manufacturer first-fit versus a programmed-fit result in significant differences in speech recognition in quiet, noise, and subjective outcomes.To examine if significant differences were present in monosyllabic word and phoneme recognition (consonant-nucleus-consonant; CNC) in quiet, sentence recognition in noise (Hearing in Noise Test; HINT), and subjective outcomes using the Abbreviated Profile of Hearing Aid Benefit (APHAB) and the Speech, Spatial and Qualities of Hearing (SSQ) questionnaires between hearing aids fit using one manufacturer’s first-fit and the same hearing aids with a programmed-fit using REM to National Acoustic Laboratories Nonlinear Version 2 (NAL-NL2) prescriptive target.A double-blind randomized crossover design was used. Throughout the study, one investigator performed all REM whereas a second investigator measured speech recognition in quiet, noise, and scored subjective outcome measures.Twenty-four adults with bilateral normal sloping to moderately severe sensorineural hearing loss with no prior experience with amplification.The hearing aids were fit using the proprietary manufacturer default first-fit and a programmed-fit to NAL-NL2 using real-ear insertion gain measures. The order of the two fittings was randomly assigned and counterbalanced. Participants acclimatized to each setting for four weeks and returned for assessment of performance via the revised CNC word lists, HINT, APHAB, and SSQ for the respective fitting.(1) A significant median advantage of 15% (p < 0.001; 95% CI: 9.7–24.3%) for words and 7.7% (p < 0.001; 95% CI: 5.9–10.9%) for phonemes for the programmed-fit compared with first-fit at 50 dB sound pressure level (SPL) and 4% (p < 0.01; 95% CI: 1.7–6.3%) for words at 65 dB SPL; (2) No significant differences for the HINT reception threshold for sentences (RTS); (3) A significant median advantage of 4.2% [p < 0.04; 95% confidence interval (CI): −0.6–13.2%] for the programmed-fit compared with the first-fit for the background noise subscale problem score for the APHAB; (4) No significant differences on the SSQ.Improved word and phoneme recognition for soft and words for average speech in quiet were reported for the programmed-fit. Seventy-nine percent of the participants preferred the programmed-fitting versus first-fit. Hearing aids, therefore, should be verified and programmed using REM to a prescriptive target versus no verification using a first-fit.

The SIR test was created for use in hearing aid comparisons. The test protocol obtains listener judgments of the intelligibility of connected speech passages. This study was conducted to evaluate the effectiveness of the SIR test in differentiating among hearing aids Specific research questions were (a) Is the sensitivity of the SIR test sufficient for differentiating among very similar and slightly dissimilar hearing aids? (b) Does the SIR test result In reliable heanng aid rankings? and (c) What are the effects of using shortened connected speech passages? Ten listeners with hearing impairments rated the intelligibility of both full-length and shortened SIR test passages while wearing each of four individually selected hearing aids representing three different frequency/gain prescriptions. Results suggested that the SIR test is capable of differentiating among slightly dissimilar hearing aids and that hearing aid rankings resulting from speech Intelligibility ratings were reliable. The decision to use full-length or shortened SIR test passages depends on the outcome the user wishes to maximize Under the conditions used n this study, maximum sensitivity was achieved with ratings from five shortened passages, whereas maximum reliability was obtained with three full-length passages.

Intelligibility scores on Harvard Phonetically Balanced (PB-50) Word Lists, Central Institute for the Deaf (W-22) Monosyllabic Word Lists, and a sample of continuous discourse were compared. Speech samples were recorded on magnetic tape under seven high frequency filtering conditions. One-hundred and seventy-five normally hearing subjects were divided into seven groups, each group listening to all speech samples under one particular condition of filtering. Word lists were scored in terms of number of words correct and the continuous discourse in terms of items correct on a test covering information presented in the continuous discourse. The following results were obtained: (1) Consistently higher scores were achieved with the W-22 word lists than with the PB-50 word lists, even when the influence of dissimilar recordings was eliminated. Such differences were attributed to a combination of differences in list content and recording conditions. (2) Monosyllabic word lists and continuous discourse are affected in the same way by frequency distortion; that is, errors increase as distortion increases. (3) No accurate intelligibility score prediction for the continuous discourse could be made on the basis of scores obtained on the PB-50 and W-22 lists.

Background: Frequency transposition has gained renewed interest in recent years. This type of processing takes sounds in the unaidable high-frequency region and moves them to the lower frequency region. One concern is that the transposed sounds mask or distort the original low-frequency sounds and lead to a poorer performance. On the other hand, experience with transposition may allow the listeners to relearn the new auditory percepts and benefit from transposition. Purpose: The current study was designed to examine the effect of linear frequency transposition on consonant identification in quiet (50 dB SPL and 68 dB SPL) and in noise at three intervals—the initial fit, after one month of use (along with auditory training), and a further one month of use (without directed training) of transposition. Research Design: A single-blind, factorial repeated-measures design was used to study the effect of test conditions (three) and hearing aid setting/time interval (four) on consonant identification. Study Sample: Eight adults with a severe-to-profound high-frequency sensorineural hearing loss participated. Intervention: Participants were fit with the Widex m4-m behind-the-ear hearing aids binaurally in the frequency transposition mode, and their speech scores were measured initially. They wore the hearing aids home for one month and were instructed to complete a self-paced “bottom-up” training regimen. They returned after the training, and their speech performance was measured. They wore the hearing aids home for another month, but they were not instructed to complete any auditory training. Their speech performance was again measured at the end of the two-month trial. Data Collection and Analysis: Consonant performance was measured with a nonsense syllable test (ORCA-NST) that was developed at this facility (Office of Research in Clinical Amplification [Widex]). The test conditions included testing in quiet at 50 dB SPL and 68 dB SPL, and at 68 dB SPL in noise (SNR [signal-to-noise ratio] = +5). The hearing aid conditions included no transposition at initial fit (V1), transposition at initial fit (V2), transposition at one month post-fit (V3), and transposition at 2 months post-fit (V4). Identification scores were analyzed for each individual phoneme and phonemic class. Repeated-measures ANOVA were conducted using SPSS software to examine significant differences. Results: For all test conditions (50 dB SPL in quiet, 68 dB SPL in quiet, and 68 dB SPL in noise), a statistically significant difference (p < 0.05 level) was reached between the transposition condition measured at two months postfitting and the initial fitting (with and without transposition) for fricatives only. The difference between transposition and the no-transposition conditions at the 50 dB SPL condition was also significant for the initial and one-month intervals. Analysis of individual phonemes showed a decrease in the number of confusions and an increase in the number of correct identification over time. Conclusions: Linear frequency transposition improved fricative identification over time. Proper candidate selection with appropriate training is necessary to fully realize the potential benefit of this type of processing.
