Background. Demonstrating differences between euthymic bipolar subjects and healthy controls in response to positive (happy) mood induction may help elucidate how mania evolves. This pilot study evaluates the Go task in a reward paradigm as a method for inducing a happy mood state and compares the response of euthymic bipolar subjects and healthy controls.Method. The Sense of Hyperpositive Self Scale, the Tellegen positive and negative adjectives, the Global-Local task and a visual analogue scale for measuring positive affect were administered to 15 euthymic bipolar subjects and 19 age-and-sex-matched healthy control subjects before and after they had performed the Go task in a reward paradigm.Results. Significant differences were found between subjects and controls on several measures at each time-point but there were no differences across the groups across time except for the visual analogue scales, where subjects had a more sustained duration in self-reported happiness compared with controls.Conclusions. This pilot study has shown that a positive affect can be induced in bipolar subjects and controls which can be demonstrated by changes in scores on several tasks. However, only the visual analogue scales showed a significant difference between cases and controls over time. Such tests may prove valuable in furthering understanding about the evolution of manic mood states.

Background/aimsDry eye disease (DED) questionnaires would ideally be easy and fast to answer and explore the main aspects of disease burden and satisfaction (efficacy and tolerability) with treatment. This pilot study evaluates the Pentascore questionnaire for routinely assessing DED.MethodsThe Pentascore combines five visual analogue scales (VAS) to assess the intensity and frequency of ocular pain/discomfort, the impact of DED on daily activities and visual tasks and the efficacy and tolerability of ongoing DED treatment(s). This retrospective study compared Pentascore to the Ocular Surface Disease Index (OSDI) questionnaire, fluorescein tear break-up-time, corneal staining and Schirmer I test.ResultsFor 161 DED patients, the algebraic mean (±SE) for the Pentascore was 52.6±1.8, the mean standardised area of the radar graph was 32.1±1.7 (out of 100) and the mean score for the OSDI was 52.6±1.8. Both questionnaires were highly statistically correlated (R=0.74 for both algebraic score and radar area, p<0.001), and each of five Pentascore VAS was significantly correlated with the OSDI (p<0.05). Corneal staining score (CSS) was correlated with two Pentascore VAS (impact of DED on daily activities and visual tasks), and there was a trend towards a correlation between CSS and the area of the radar graph (p=0.09).ConclusionsThis pilot study indicates that the Pentascore can rapidly and effectively assess the burden of DED and satisfaction with treatments. Compared with the algebraic mean, the estimation of the area of the radar graph likely improves the sensitivity for detecting differences/changes in symptoms and treatment follow-up.

AbstractIntroductionPRN (as required) medication is commonly used in inpatient psychiatry but little is known about staff opinions concerning its use.Aims &amp; objectivesToexplore views about PRN medication among nurses and psychiatrists working in PICUs or secure care at a charitable hospital.MethodA structured questionnaire with visual analogue scales was used to examine staff views about PRN medication. Participants were: 21 PICU nurses, 20 nurses working on medium secure admission units (MSAUs) and 27 psychiatrists. Inquiry was made about preferences and avoidance of PRN drugs and when PRN administration becomes rapid tranquillisation (RT).ResultsNurses working in PICUs and MSAUs held similar views about PRN medication. Nurses were more in favour of PRN medication than psychiatrists, whereas psychiatrists felt more strongly that PRN sedation was sometimes over-used. PICU staff favoured promethazine whereas non-PICU staff preferred lorazepam/lorazepam and haloperidol combined. There was no consensus as to when PRN administration becomes RT.ConclusionsNurses hold more positive views about PRN sedation than psychiatrists. Despite the ECG requirement, many staff favour haloperidol. The NICE definition of RT was not quoted by most participants. RT and PRN medication is a grey area which deserves further exploration.
